Permission to Use Image or Audio/Video Recording

The College of Earth and Mineral Sciences of the Pennsylvania State University (University) wishes to produce an image or digital video and/or audio recording of the following presentation or performance by the following author:

University wishes to disseminate the recording as a courseware module that can be viewed on computers and other digital media played. “Courseware module” is defined in Penn State policy RA17 as “computer-based materials intended to enhance or supplement a Penn State residential or on-line course.” The courseware module to which this agreement refers will enhance and supplement the College’s education and outreach programs, but will not be a required component of an entire courseware product.

If Author is a Penn State employee, the presentation or performance to be recorded is above and beyond the scope of Author’s normal duties. Whether or not Author is a Penn State employee, no extra compensation is provided for the presentation or recording. The courseware module is therefore not a commissioned work. University asserts no claim of copyright over the courseware module.

Author hereby grants a no-cost, non-exclusive, royalty-free license to University to distribute the courseware module freely via computer networks, CD-ROM, broadcast, or other media. Author attests that the content of his or her presentation is original, or that he or she has acquired permissions to use content that is copyrighted by third parties.  Yes  No ______(Initials)
