Request for Bandwidth Upgrade Or Cutover

Request for bandwidth upgrade or cutover

This form is used to request an upgrade or cutover to your school’s bandwidth.

Cutover refers to changing from one data carriage to another (e.g. ADSL 1500K to GWIP+ 4MB).

Upgrade refers to changing the speed with the same data carriage (e.g. ADSL 512K to ADSL 1500K).

Instructions:

Email evidence showing discussion and approval from both your regional technology manager and principal/approving officer is required to be attached when creating your job in the Service Centre’s Service Now job logging system .

This form authorises Education Queensland to direct debit the cost of the upgrade/cutover.

Financial implications  An upgrade in school bandwidth as a result of this request will result in a variation to the school’s annual Web and Internet Access Service (WIAS) contribution. Annual contribution rates are outlined online.

 Schools will also need to budget for a pro rata contribution to cover that period between the month of upgrade and the end of the current financial year.

 A decision not to proceed with an upgrade may incur cancellation fees for work undertaken by service providers.

 Contact the Service Centre on 1800 680 445 or email [email protected] if you have any questions while completing this form.

Please be aware of the following timeframes:

Upgrades: it will take vendor a minimum of four weeks.

Cutovers: it will take vendor a minimum of six weeks.

(Install time starts when CITEC submits the data service application to vendor install as is dependent on vendor: resources, change freeze periods, weather infrastructure capability and public holidays). All fields are mandatory.
