GRANT QUESTIONNAIRE

We endeavor to make the submission process simple and flexible for eligible charitable organizations. First time applicants (or those who have not applied for some time) are strongly encouraged to visit with executive director Terri Yoho, to discuss whether your request fits Kohler Foundation guidelines.

Submission Instructions:

1. We prefer applications to be submitted via email, in order to save postage and paper. Email submissions to Kohler Foundation at: [email protected].

2. Fill out the entire Grant Questionnaire, in order, starting with Name of Organization, and include a detailed project budget.

3. K-12 Schools are also required to include a cover letter from the Principal of the School, explaining how the request fits into the school's overall strategic goals and priorities.

6. Description of Project, including: Why is it needed? How many people will benefit? What is the timetable, and when is the funding actually needed?

7. Project Budget, including: How much will the project cost? What other funding sources are supporting the project? (An attachment is allowed)

8. Total Annual Budget of the organization, including: What are the broad categories of revenue & expense for your organization, and percentage of each.

9. How will you know if this project is successful? How will you measure and communicate that success? Do you have a plan for this program’s future growth and sustainability? (Please indicate whether such a plan has been developed. Do not include the plan in detail.)

10. Project Manager name and credentials:

Updated 1/10/17 *If applicant is a K-12 School, a Cover Letter from the Principal of the School is required. Letter should express support for the project as part of the school's overall priorities/needs.
