SPECIALIST REGISTRAR POST IN JOB DESCRIPTION 2021

APPOINTMENT

This job description covers a full-time non-resident Specialist Registrar post in Orthodontics. The duties of this post will be split between Kettering General Hospital and the Eastman Dental Institute, with three days spent in Kettering and two days in the Eastman Dental Institute. This post will be based administratively at the Trent Deanery, Sheffield.

The Postgraduate Dental Dean has approved this post for training on advice from the SAC in Orthodontics. The posts have the requisite educational and staffing approval for specialist training leading to a CCST and Specialist Registration with the General Dental Council.

The successful candidate will be required to register on the joint training programme leading to the M.Orth.RCS and the MClin.Dent. in Orthodontics at the (unless they already hold equivalent qualifications). This appointment is for one year in the first instance, renewable for a further two years subject to annual review of satisfactory work and progress.

QUALIFICATIONS/EXPERIENCE REQUIRED

Applicants for specialist training must be registered with the GDC, fit to practice and able to demonstrate that they have the required broad-based training, experience and knowledge to enter the training programme.

Applicants for orthodontics will be expected to have had a broad based training including a period in secondary care, ideally in Oral and Maxillofacial surgery and paediatric dentistry and to have completed a 2 year period of General Professional Training. Normally they will have passed the MFDS, MJDF or equivalent examination as evidence of satisfactory completion of GPT. Those undertaking the MJDF examination will have satisfactorily completed a portfolio of evidence summating their work-based experience, and including certificates of attainment. Evidence of experience in Orthodontics during GPT will not be essential but would be advantageous. Those without a College examination may be allowed entry to training if they old a higher degree in an appropriate and relevant subject area, or can demonstrate an appropriate level of knowledge and prior experience.

THE HOSPITALS

ORTHODONTIC DEPARTMENT MAXILLOFACIAL UNIT KETTERING GENERAL HOSPITAL NHS FOUNDATION TRUST

Kettering is situated in the North of Northamptonshire and the surrounding area includes the market towns of Corby, Rushden and Wellingborough, together with a variety of smaller towns and attractive villages. Northants the “Rose of the Shires” is in the heart of England between the industrial Midlands and East Anglia. The A14, which links the M1 and A1, passes near the hospital and provides easy access to M1, M6 and M11, and London, Leicester, Birmingham and Cambridge. There is a frequent fast train service to London (under 1 hour to St. Pancras International station), and there are three international airports within an hour’s drive. In the locality there is a mixture of urban and rural population centred on Kettering and extended to the towns of Rushden, Wellingborough and Corby. The area offers a variety of housing types from village to town and there is a good range of schools. The surrounding countryside is well preserved and offers access to many outdoor activities including sailing on Rutland Water.

The Kettering area is well supplied with leisure facilities (sailing, riding, fishing, swimming, and cycling). House prices are comparatively low compared with other English Counties. There are plenty of sports facilities including several private health clubs, as well as active rugby, hockey and football clubs. There are also several private and public golf clubs in and around the area.

The Hospital covers the Northamptonshire. There is a growing population of, at present, 747,622. The hospital draws referrals from the surrounding districts. The Trust has approximately 600 beds distributed between the specialities. There are 3 main buildings making up the hospital, with the Radiology Department lying centrally. The hospital is made up of 5 developments dating from the beginning of the last century, the l960’s, 1970’s l980’s and present. Within these areas, significant developments and refurbishments have taken place.

There is considerable site development at Kettering General Hospital. The first phase was the completion of a 12 million pound, state of the art, day case, diagnosis and treatment centre which fully opened in 2007. A dedicated Paediatric unit was completed in 2014 and a new Obstetrics and gynaecology unit in late 2015.

Most recently the Maxillofacial Unit itself has been redeveloped at a cost of £1.8 million.

Kettering General Hospital has excellent library facilities, with a recently extended Postgraduate Centre. The Trust also offers a wide range of staff benefits including discounts within retail and sport and leisure facilities, a holiday play scheme and nursery on site. Kettering town is situated in the midst of the attractive Northamptonshire countryside with excellent rail and road links.

A six-bedded combined ITU/HDU is in close proximity to the main operating theatres. There are fully supporting Pathology and Radiology services. Radiology provides ultrasound, nuclear medicine, MRI and computerised axial tomography on site. There is a CT scanner and MRI scanner on site. Radiographic services are fully digitised.

Maxillofacial Unit

The unit is situated on the ground floor of the Cave Block and comprises of 2 surgical treatment suites, 2 orthodontic surgeries, one trauma room, 2 consulting rooms, a wellbeing room, a recovery room and a laboratory treatment room. There is a maxillofacial laboratory within the unit as well as digitised radiography facilities including DPT and cone beam CT.

The medical staff comprises two part-time Consultant Orthodontists, four Consultant Maxillofacial Surgeons, one orthodontic StR, two DCT grade staff, three Oral Surgery Specialty Doctors, , one Orthodontic Therapist and one Orthodontic Clinical Assistant. The nursing staff comprise of Sister, and Deputy Sister, three Staff Nurses??, nine Dental Nurses??, two Health Care Assistants??, two Laboratory Technicians, four receptionists, one clerical officer and three secretaries.

A full range of maxillofacial procedures is undertaken, including cancer surgery and reconstruction, orthognathic and dento-alveolar surgery. Joint cleft clinics are run once monthly in the Maxillofacial Unit attended by the full cleft team, based at Addenbrookes Hospital, Cambridge. Joint orthognathic clinics are run monthly and attended by Maxillofacial and Orthodontic Consultants.

Orthodontics

A full range of orthodontic techniques is undertaken. Most cases are treated with 2-arch fixed appliances, with mainly the Straight-wire system being used. A significant number of cases are treated with a range of functional appliances and is entirely up to date with the latest digital technology’s including onsite radiography, CBCT, 3D printing and 3D surgical planning software.

Clinics dedicated to intra-oral scanning and photography support the use of the technology.

The Radiology Department is situated near the Outpatient department and has full digital radiological facilities including OPT and Lateral Ceph. machines. Digital intraoral radiographs are carried out in the Maxillofacial Unit.

Clinical photography is undertaken by the laboratory technicians, although the trainee will be encouraged to take their own clinical photographs. The department has clinical cameras and computerised data archiving. The orthodontic technicians have received additional training in cephalometric tracing and PAR scoring. Departmental computers are set up for digitised tracing and planning as well as photography using the Dolphin system.

Eastman Directorate & Eastman Dental Institute The Eastman Directorate lies within the Specialist Hospitals Board. The activities of the Directorate include outpatient care, inpatient care as well multidisciplinary and interface clinics within the Directorate as well as the Trust and other providers. The services delivered by the Eastman Dental Hospital take place at its main site on Huntley Street in London for the majority of outpatient care and all inpatient and day case work being undertaken on the main UCLH campus. The Directorate also provides trauma cover for surrounding hospitals, namely the , the , St Mary’s Hospital and Great Ormond Street Hospital for Children. The Eastman provides a number of outreach Restorative Services due to the increasing demand within this discipline.

The Eastman, comprising of the Eastman Dental Hospital & Institute, is a unique postgraduate centre for oral health care sciences offering first class research, teaching and clinical training. The Institute is part of University College London and the Hospital is part of University College London Hospitals NHS Trust. Together they are able to enhance the experience of those working and training at the forefront of clinical research and patient care. Last year approximately 80,000 patients were seen as outpatients in the Eastman Directorate. The Eastman Directorates vision is to deliver a world class oral health service with patient care forming the core of its education and research activities.

Clinical Departments

The clinical departments are arranged on a Divisional structure. There are 3 divisions which are:

• Growth and Development: comprising of the Department of Orthodontics and the Department of Paediatric dentistry. • Diagnostic, surgical and medical sciences: comprising of the Departments of Oral Surgery and Oral Medicine (including special care dentistry and facial pain). • Restorative Sciences: comprising of Department of Endodontics, Department of , Department of and includes Implantology as well as the Schools providing DCP training.

The clinical Departments cover all aspects of dentistry and medically related fields. In addition to the traditional subjects, novel academic departments supporting clinical advancements and new areas of training include:

Research Facilities

The Institute has extensive dedicated research facilities on site readily accessible to clinicians with an interest in research as well as those employed as researchers. Collaborative research is encouraged within UCL and beyond.

Other Facilities The Institute and Hospital have been steadily improving facilities and technology to enhance the learning environment. As part of UCL there are also close links with other departments within the university. The following are available on site: ⚫ An Information Centre with 34 computers for access to academic networks and the world wide web, a wide range of contemporary audio-visual materials and an extensive collection of books and journals. ⚫ Lecture theatres and seminar rooms with contemporary audio-visual facilities. ⚫ A refectory for staff, students and patients.

The Eastman Continuing Professional Development Unit is a dedicated training facility based just a short walk away from the main Eastman building at 123 Grays Inn Road. Challenging short courses and longer programmes are available for all members of the dental team in modern teaching facilities, which include lecture theatres, seminar rooms and two state of Page 6 of 13

the art hands-on skills laboratories. The Trust also now has a dedicated Education centre based on 250 .

Work and Interests of the Department of Orthodontics

The Department of Orthodontics forms part of the Division of Growth and Development. It has a first-class reputation for its patient care, teaching and research activities both nationally and internationally. The department provides advice and treatment often of an advanced nature in the field of Orthodontics for patients referred by general dental practitioners, specialist practitioners, other departments within the Eastman and other hospitals. The department fosters a close and integrated working relationship with the other departments, thus facilitating patient care to its optimum standards. The Orthodontic Department has close links with several hospitals including Mayday University Hospital, Kingston Hospital, Royal Surrey County Hospital, Watford General Hospital, Chichester Hospital, Kettering Hospital, Peterborough Hospital, The John Radcliffe Hospital and Great Ormond Street Hospital for Children.

A central focus of the Unit is to provide high quality patient care through the educational programme for Specialty Trainees and postgraduates in Orthodontics at Masters and Specialist levels who enroll onto our programme from across the world. The Eastman Orthodontic Department is devoted solely to postgraduate education. The Unit has a national and international reputation which comes from 50 years’ experience of specialist level training. The postgraduates and staff work together within the ethos of a team of over 90 individuals. The programme as a whole, including its content and its teaching staff, is appraised frequently enabling a constantly evolving process to ensure it meets the highest quality standards and is appropriate for modern orthodontics.

All aspects of Orthodontic care are provided, and the special interests of the department include management of patients with complex malocclusions, particularly those requiring multidisciplinary care. The following multidisciplinary clinics are held: Craniofacial, Hypodontia, Orthodontic-Paediatric Dentistry, Orthodontic-Periodontal, Orthodontic- Restorative and Orthognathic.

The department occupies 22 clinical chairs. The clinical activity is supported by a full complement of nurses, reception staff and secretaries and there are approximately 25,000 patient attendances annually. Regular departmental meetings are held, and the department has a focus on using audit to improve quality and patient care.

The Orthodontic Department actively collaborates with other specialties within the Eastman Dental Hospital and Institute, in particular with Prosthodontics, Oral and Maxillofacial Surgery, Biomaterials and Tissue Engineering and Microbiology.

THE TRAINING PROGRAMME

This training programme is run over three years and involves a mixture of didactic and clinical chair-side teaching. The post holder will also enrol with the University of London for a two-year MClinDent. in orthodontics, (unless they already hold an equivalent qualification).

The curriculum and training programme have been designed to comply with recommendations set by the SAC in Orthodontics of the Royal College of Surgeons with learning outcomes expected of an MOrth graduate.

Clinical Training

The trainee will undertake clinics at both the Kettering General Hospital and the Eastman Dental Institute. Each trainee has their own treatment clinics and attends New Patient clinics, where an initial patient assessment is performed. New patient clinics allow the trainee to gain experience in diagnostics and improve their treatment planning skills. In the treatment clinics, the trainee has an appropriate case load of patients hand-picked for their suitability for examinations and training. During these clinics the Consultants provide 1:1 chairside teaching.

The Maxillofacial unit at KGH has dedicated nursing, I.T. facilities and cameras available for to support their training. Dolphin software provide storage and display of clinical photographs as well as enabling digitised cephalometric analyses to be performed. Trainees will be encouraged to undertake Workplace Based Assessments and audit. The trainee will be allocated an Educational Supervisor and through a series of appraisals, the trainee’s development can be nurtured.

Academic Training

There is a comprehensive modular academic programme involving seminars and lectures, which is delivered on Fridays when all the trainees are at the Eastman dental institute. A weekly Journal club allows the latest literature to be reviewed and assessed with the trainee learning the importance critical appraisal and interpretation of new literature. A reading list will be provided to guide the trainee throughout the training period and essays will be set for selected topics.

Research

Evidence-based practice is at the centre of all training. The post holder will be expected to carry out a supervised research project during their training. This allows the trainee to learn the importance of understanding the research process, the value of advancing research and the interpretation of that new knowledge. The project will be overseen by two supervisors, one at KGH and the other at EDI. Full statistical support is provided at EDI. This research project is to be submitted as a dissertation/thesis in partial fulfilment of the MClinDent. The project should result in a peer-reviewed publication.

Health and Safety All Trusts, singularly and severally, recognise their duties under the Health and Safety at Work Act (1974) to ensure that, as far as is reasonably practical, the health, safety and welfare at work of all their employees, and, in addition, the business of the Trusts, shall be conducted so as to ensure the patients, their relatives, contractors, voluntary workers, visitors, and members of the public who have access to the Trusts’ premises and facilities are not exposed to risk to their health and safety. All medical and dental staff under contract to the Trusts will be expected to comply with national health and safety policies and also those specific to the individual Trusts.

Conditions of Service The appointments are subject to the Terms and Conditions of Service of Hospital Medical and Dental Staff (England and Wales) as amended from time-to-time, and also such other policies and procedures as may be agreed. The successful candidates will work in the appropriate Units as detailed above. As a consequence, they are expected to be aware of local policies and procedures and take note of the standing orders and financial instructions of that Authority. The post is full-time and non-resident.

Salary The Specialist Registrar salary scale progresses by annual increments in accordance with the current nationally agreed salary scale.

Health clearance Potential applicants should be aware of the Department of Health and GMC/GDC requirements with regard to HIV/AIDS and Hepatitis viruses.

Candidates are advised that they will be asked at interview whether they are aware of anything preventing them from fulfilling their professional commitment and this includes health.

Annual leave

Specialist Registrars on the minimum, 1st or 2nd incremental points of their payscale, shall be entitled to leave at the rate of 5 weeks a year. Specialist Registrars on the 3rd or higher incremental point on their payscale shall be entitled to leave at the rate of 6 weeks a year.

Study leave

Defined as leave taken to attend a formal course of study designed to further the employee’s career or to enable the employee to obtain his/her professional qualification or gain the necessary expertise in order to carry out his/her job.

This is granted in accordance with the recommended allowance subject to the maintenance of the service and to prior formal approval by the Clinical Tutor at the appropriate administrative Trust. Application for study leave must be made at least six weeks prior to the period of absence and ALL consultants involved in training during the planned dates must be consulted.

Notification of Termination of Employment Specialist Registrars are required to give individually to the relevant Trusts a minimum of three months’ notice of termination of employment. This appointment is for one year in the first instance, renewable for a further two years, subject to satisfactory work and progress.
