JOB DESCRIPTION
CARER SUPPORT WORKER (Transitions Project)
Post Title: Carer Support Worker Responsible to: Manager Salary: AP3 pro rata based on 37hours Hours: 35 hours per week
“A carer is someone who is supporting a relative, child, friend, partner or neighbour with an illness, frailty, a disability or addiction issue.”
Purpose of Post:
To review and develop the information, support and advice we currently provide Carers of young people moving to adult services and Young Adult Carers entering our own adult Carer service.
Applicants must be committed to Inverclyde Carers Centre values which set out a picture of how we will strive to work with all people involved with our service.
Key Responsibilities:
To identify and increase engagement with Young Adult Carers aged between 18 and 25 years. To work with Young Adult Carers to better understand their needs. To build relationships with other Young Carer projects across the country. To identify good practice and work alongside Young Adult Carers and other organisations to develop a range of information, advice and support in Inverclyde. To improve the information, advice and support we provide to carers of children transitioning to adult services. To work intensively with Carers of young people to better inform planning of Inverclyde services making them more responsive to family needs.
Generic Tasks:
To provide information, advice and support to Carers in all settings.
To support Carers to identify their own needs through Carers Self Assessment, agree
outcomes and action plans.
To undertake home visits to Carers when required. To liaise with staff from other organisations in providing support to Carers
To accurately record relevant information about Carers
To inform the wider community of Carers services and issues
To maintain accurate records of service delivery
To monitor and evaluate the outcomes of the service through report writing and oral
presentation
To contribute to the development of services and information for Carers.
To attend meetings, conferences and training relevant to the role.
To keep abreast of new developments and legislation affecting Carers.
To support Carers to express their views in relation to planning and delivery of Care as
Equal Partners.
To carry out such work as assigned by the Manager. Personal Specification:
Essential Skills, Experience and Qualities
Empathy with Young Adult Carers and excellent understanding their needs
Empathy with Carers of children and young people and excellent understanding their needs
Good understanding of carers rights, services and support available to them
Experience of collaborative work with a range of agencies
Excellent communication and IT skills
Experience of data collection, collation and report writing
Confidence, enthusiasm and creativity to develop ideas and carry them forward.
Excellent team member
Ability to work on own initiative
Experience of caring situations in a personal or professional capacity
HNC Social Care, Health, Community Development, Youth Work or experience in a
relevant post with transferable skills
Desirable Skills, Experience and Qualities
Working knowledge of voluntary and statutory organisation
Experience of support work
Full driving licence and access to car
Experience of developing engagement and consultation processes and opportunities
Experience of facilitation and group work
