OSP Application 2015-2019 p.1 SECTION I: Program and Administration

Please provide one-paragraph responses to each of the following questions. Use as many pages as needed. Pages must be standard MS Word electronic file documents, 1.5 or double-spaced in 12-point font. All MS Word files must have a filename that includes an abbreviation of the organization’s name and an indication of the file’s content, referring to the section and letter below, or other clear indication of content. Examples: SMTheaterBalanceSheet.doc or Art_Project_Inc_Section_I-A.doc, etc.

A. Artistic and Cultural Achievements and Aspirations (1) Describe the organization’s key artistic or cultural aspirations and achievements. (2) Summarize the training, experience and other qualifications of the key artistic/cultural personnel. (3) Indicate the agency’s policy with regard to compensation of artists. (4) Provide a concise list of the applicant’s planned exhibitions, performances, programs and special activities for the twelve months from July 1, 2015 to June 30, 2016

B. Administrative and Fiscal Achievements and Aspirations (1) Summarize the strong points of the applicant’s leadership, fundraising and administrative capacities. (2) Does the organization maintain written policies and procedures for the administration of programs, bookkeeping and the supervision of employees? Is so, please describe. (3) Does the Board of Directors help fund the organization? What percentage of members contribute? (4) How does the organization address strategic planning? Describe any written strategic plan and/or planning process in place. (5) What evaluation processes does the organization use to measure the effectiveness and reach of its programs? (6) If the applicant has an endowment, what are the restrictions, if any, on the use of the funds? Does the applicant have a cash reserve or any plans to develop one?

OSP Application 2015-2019 p.2 C. Audience, Community and Partnerships (1) Describe the impact(s) of the applicant’s programs on Santa Monicans. Provide evidence, both quantitative and qualitative, of those impacts. Include attendance figures and the percentage of audience members with Santa Monica zip codes. If the organization does not collect address data from audiences or participants, explain why. (2) Describe how the organization defines and engages underserved audiences. (3) Indicate any significant partnerships or engagements between the applicant and other Santa Monica organizations.

SECTION II: Financials

1. List on a separate page all major donors and contributors and the amount of their support.

2. Attach a Balance Sheet (Statement of Financial Position) showing the organization’s assets, net assets and liabilities.

A. All applicants must attach a balance sheet from March 2015 signed by the Executive Director, Board Treasurer or President.

B. In addition:  Organizations of $500,000 annual expenditures or greater must attach their most recent audited Balance Sheet signed by the Executive Director, Treasurer or President.  Organizations of less than $500,000 annual expenditures must attach a review of their financial statements conducted by a CPA and signed by the Executive Director, Treasurer or President. The review must be no more than two years old – i.e. dated no earlier than March 2013.

Annual expenditures amounts are deemed to be as reported on Line 45 “Total expense not including in-kind” of the Financial Activity Detail of their most recent Cultural Data Project report submitted for this application.

3. If the applicant is planning or executing capital improvements, indicate the amount of capital-restricted assets currently held.

OSP Application 2015-2019 p.5 SECTION V: Required Supplementary Materials Every organization is required to include the following with their application:

1. A copy of the organization’s Cultural Data Project OSP Report showing the most recent three fiscal years’ data. The “most recent” year is defined as the year ending June 2014 for organizations on July-June fiscal years, or the year ending December 2013 for organizations whose fiscal year coincides with the calendar year. To obtain the City of Santa Monica OSP report, log on to www.caculturaldata.org/home.aspx and go to Funder Reports.

2. A copy of the Board’s (or governing body’s) current bylaws.

3. Proof of nonprofit 501(c)3 status.

4. Artistic Documentation. Artistic documentation is crucial for evaluating the artistic standards of the organization. Applicants must submit either:  Two video samples  Two musical-audio samples  Up to forty (40) visual images in jpg format, with separate index

All samples should be recent, of high quality, and as relevant to the organization’s mission and aspirations as possible. All documentation must be digital and uploaded to the internet. The entire submitted sample, not just a selected segment, is considered part of the application and may be reviewed. Panelists generally spend no more than three-to- five minutes reviewing work samples for each application.

Do not submit marketing/promotional materials as artistic documentation. See Optional Marketing Materials below for instructions.

Instructions for Artistic Documentation To submit video or audio samples for performing arts, media arts, music, or heritage organizations: applicants may submit up to five minutes of material. Panelists generally prefer the five minutes to consist of one or a few substantive excerpts of works rather than short clips edited together. Upload your artistic samples to youtube, vimeo, SoundCloud, or a similar website that is accessible to any viewer with the URL. City staff and panelists must be able to click on the link you provide and view the artistic samples. Passworded site pages are acceptable if the password is included in the sample index below. Samples must be labeled clearly with the corresponding work sample letter (A or B) from the Artistic Documentation Links and Index.

To submit still images for visual arts, heritage organizations, etc.: applicants may submit up to forty (40) images. Upload a folder containing the images to Dropbox. An MS Word document titled Org_Name_Image_Index.doc, which serves as an index to the images, must be included in the folder. In the index document, also provide a brief statement describing the samples’ relationship to the organization’s mission and artistic aspirations. The folder should be titled “OrganizationName_OSP_Samples” or a similarly clear title.

Submit a maximum of three examples of scanned or online marketing materials to demonstrate the quality of the organization’s marketing and outreach (optional). Label each sample A, B or C according to the index below. Also include the name of your organization in the filename for each sample. Materials may include press releases, reviews, scans of season brochures or other printed materials, community postings and/or other marketing and outreach related instruments.
