(a) Identify each source and provide its estimated contribution to total loading, using most current source contribution estimates from a TMDL-I-Plan or Watershed Protection Plan project. EPA guidance notes that “sources that will need to be controlled should be identified at the significant subcategory level with estimates of the extent to which they are present in the watershed” – examples given including linear miles of erodible streambank; others might include estimated number of malfunctioning septic systems, grazing livestock, concentrated bird roosting areas, miles of wastewater line needing repair, etc. (c) Identify each management measure proposed and who will implement it (responsible parties), indicating the status of commitment for implementation – such as selected and funded, or as one among two or more options within a committed implementation, or as planned under certain circumstances and time frames such as continuing high concentrations of a pollutant, or as under consideration by a responsible party at the planning or budgeting stage, or even as an option offered for consideration but not actively being considered yet by a potential implementer. Quantify the extent to which it is to be applied in specific critical areas and in total. Critical areas for a measure might include riparian/stream side areas, natural wetlands, areas of concentrated bird roosting, etc., as a class and/or noting specific locations. Quantification of BMP coverage might include acres (runoff contributing areas) to be treated by a structural BMP for the design storm event, or equivalent measure used as the basis for estimating the resulting bacterial load reductions. (b) Give the bacterial load reductions estimated for each management measure proposed, in the same units used for the load contributions by source under (a) above. The basis of these load reductions should be presented, and needs to include a performance rating for each BMP category and a quantification of the extent of coverage as explained under (c) above. (d) Identify funding and technical support expected to be needed by participating entities to implement each measure, as one-time and/or annual amounts. Identify prospective sources of funds if known (e.g. city budget or grant source). (e) Describe the information & education activities proposed to support or implement each measure as well as broader-scope education and stakeholder involvement. (f) Give the dates by which each set of management measures will be implemented, individually and/or in phases. (g) Describe how specific milestones of the management measures implementation schedule given in f above will be documented, such as posting of photos and map references of completed structural BMPs or links to adopted ordinances on a web site. (h) Give target dates by which concentrations of pollutants at particular locations will be reduced by a target amount, and when other criteria if any are to be reached indicating progress toward achieving the water quality goal. (i) Describe the water quality monitoring to be done to document progress toward and achievement of the water quality criteria as outlined in h above, and to help identify where to focus further efforts if the goal is not achieved.

General notes:  There can be blank or incomplete cells/sections in this matrix. For example, critical areas may not have been identified for the application of some of the BMPs in the (c) column, and some measures may not have educational components (column e).  It is acceptable and expected that the available information will not provide a perfect basis for presenting the requested information in this form, as long as your basis of deriving estimates is explained.
