Six appendices cover a range of topics including benefit design, EPSDT requirements, use of public- private partnerships to expand access, options in state contracting, and cost effectiveness. This document considers seven key decisions states need to make when deciding whether to contract dental Medicaid to a vendor or managed care company. It provides guidance on choosing to contract out, to carve-out, to assign financial risk, to engage a single or multiple vendors, and to select, finance, and manage a plan. It concludes with “lessons learned” from a variety of states.

This handbook and model contract provides information on how to implement contracting between FQHCs and private dentists. It has been endorsed by the American Dental Association, National Association of Community Health Centers, and thoroughly vetted with HRSA and CMS.
