Cleveland Charter High School Homecoming Dance Guest Contract Please note: All non-Cleveland Charter High School students must complete this form. This form should accompany the guest’s completed behavioral contract. This form is due when ticket is purchased.

Dance & Type: Cleveland Homecoming Dance 2017 Day & Date: Saturday, October 21, 2017 Time: 7:00PM- to 11:00PM

To ensure a safe and contained environment for this event, all guests are subject to advance verification. Each student must submit 1) a clear photo copied current school ID to verify age and 2) obtainment of high school authorization to verify attendance compliance and good standing of the student. Because school signatures must be verified, this form may require 24 hours for approval. Guests who attend another high school are highly encouraged to submit this form EARLY. Former Cleveland students who were expelled or sent to another school for any reason are not eligible to attend this event. All guests must be approved by the Cleveland Charter High School Administration. AGE REQUIREMENTS & ID VERIFICATION

SCHOOL VERIFICATION & AUTHORIZATION Guests must be verified by their school as being in good standing. Students in good standing do not have detention, truancy, or disciplinary problems. School clearance should come from a school Administrator and must include the embossed school seal, school stamp, or attached signee’s business card. This requirement applies to students in traditional high school, home school, distance learning school, or any other recognized high school alternative.

The signees below understand that the Cleveland Charter High School student will be held responsible for his/her guest’s actions and adherence to Cleveland school rules. All attendees must bring their current school IDs in order to be admitted to the event. Evidence of misrepresentation on this form, or failure to adhere to the items outlined in the Behavioral Contract may result in removal from the event. Guests not approved by the Cleveland administration and/or guests removed after approval will not receive a refund or other compensation.

SAFETY & PROHIBITED ITEMS I agree and understand that  Entrance to this event may require security screening. My person and belongings may be thoroughly searched. Any prohibited items found will be confiscated.  The event staff and Cleveland Charter High School administrators have the right to confiscate any item which might be deemed a safety or defacement hazard. Confiscated items will not be returned. Such items include but are not limited to: Perfumes Any illegal substances Canes Alcohol/Flasks Any other liquid items Any tobacco products Chains (wallet, zoot suit, other) Mouthwash Any prescription drugs Lighters/matches Weapons, sharp Pens, pencils, markers Water bottles not pre-approved by objects Gum Eye drops Cleveland administration Containers with screw on cap or lid Glow in the dark items Ear Phones including perfume or cologne Cleveland CharterElectronic High Chargers School will not be held responsible for any lost, stolen, or confiscated items. The defacement or destruction of any property at the event is prohibited. Trespassing beyond the established boundaries of the event is prohibited. To help ensure the safety of all attendees at our dance, breathalyzer testing will be implemented randomly throughout the night and to protect the facility from any vandalism, there will be security guards at the dance checking purses and doing light pat downs prior to any persons entering the facility.

BEHAVIOR EXPECTATIONS

DRESS CODE This is a SEMI-FORMAL event. The following dress code will be applied at this event. If you have questions about your attire, please contact the Cleveland Administration BEFORE the event. In addition to the Cleveland Charter High School Dress Code, the following must be adhered to for Homecoming Dance: For students wearing tuxedos, suits or slacks For students wearing dresses  No casual clothing such as: jeans,  No low cut dresses, no plunging necklines, V-neck dresses shall be no lower than the top denim, sweats, shorts, t-shirts, tank tops, beach of your sternum wear, flip flops, etc.  The midriff area must be covered by fabric.  No baggy pants  Backless dresses may not fall below the waistline  No transparent garments as  No side exposure or high cut slits (this means no cut out holes or slits that replacements for tuxedos, suits or slacks. expose your torso or leg above the thigh)  Chains and/or canes are not permitted.  No mini dresses, mini-skirts or high cut hemlines (if you extend your arms straight down  Baseball hats or gang affiliated headwear is not your sides, make sure that the length is lower than where the bottom of your middle permitted. fingers touches your side)  No body painting of any sort as a replacement for  No transparent garments as replacement for dresses clothes, even if a jacket is used to cover up the body  Clothing may not have any cutouts that reveal any area required to be covered. painting  Dresses must be acceptable without any accompanying cover-ups.  Proper shoes must be worn at all times  No casual clothing such as: jeans, denim, sweats, shorts, t-shirts, tank tops, beach wear, flip flops, etc.  Chains and/or canes are not permitted.  Baseball hats or gang affiliated headwear is not permitted.  No body painting of any sort as a replacement for clothes, even if a jacket is used to cover up the body painting  Proper shoes must be worn at all time

*If you fail to adhere to the dress code policy, you will have the opportunity to change into something more appropriate, as decided by the administration, or you will be escorted from the Homecoming Dance facility. There are no exceptions and no refunds. * MEDICAL AUTHORIZATION
