The Council on Aging of Martin County’s 2016 Sage Awards Saturday, February 27th, 2016 Charles & Rae Kane Center 900 SE Salerno Road | Stuart, FL | 34997

The Council on Aging commits itself to helping seniors live well at any age, by remaining independent, healthy and living in their own homes. In the spirit of this mission, the Council presents the 2016 Sage Awards to honor standout seniors in Martin County and St. Lucie County who continue living purposeful lives by offering their experiences, professional skills and compassionate hearts to their communities. Despite the end of a career, not all seniors retire their knowledge, ambition, or leadership, but instead share their wisdom to better their communities. The Sage Awards will to recognize these service-driven seniors and honor them for making significant impacts in the world around them, while displaying a positive image of aging to younger generations. The awards will be presented during a ceremony dinner held at the Kane Center in Stuart, Florida, on Saturday, February 27, 2016. The proceeds from this event will benefit the charitable work of the Council on Aging of Martin County, a private non-profit organization which provides senior services. We invite members of the community to nominate a senior you know who has exemplified exceptional commitment and achievement in one of the seven categories: Arts Faith-Based Community Healthcare/Medicine Military/Veterans Support Professional/Business Community Social or Political Advocacy Volunteerism Please refer to the nomination form for further explanation of each of the award categories. On behalf of the Council on Aging of Martin County, we hope that you help us to recognize the invaluable seniors in our community who are actively creating a better world to leave as their legacy.

Eligibility for Nomination:  Nominee must be 62 years or older at the time of the nomination.  Nominee must currently reside in Martin County or St. Lucie County.  Both the nominee and the nominator must agree to attend the award ceremony. Categories: Please read the following descriptions carefully before selecting a category.

The 2016 Sage Awards Presented by the Council on Aging of Martin County Arts: The nominee presents a positive image of aging and provides outstanding commitment and contributions to the arts as an artist or an arts advocate. He or she maintains an active role within literary, visual or performing arts as either a participant or supporter.

Faith-Based Community: The nominee presents a positive image of aging, and provides outstanding commitment and contributions to faith-based organizations. He or she maintains active involvement and/or leadership roles in religious institutions or organizations.

Healthcare/Medicine: The nominee presents a positive image of aging, and provides outstanding commitment and contributions to the fields of healthcare and/or medicine. He or she maintains active involvement and/or leadership roles in areas including but not limited to direct patient care, administration, policy, funding or research.

Military/Veterans Support: The nominee presents a positive image of aging and, as either a member of one of the branches of U.S. armed services or as a civilian, provides outstanding commitment and contributions to military and/or veteran affairs. He or she maintains active involvement or leadership roles in community organizations committed to supporting a specific branch of armed services or to armed services as a whole.

Professional/Business Community: The nominee presents a positive image of aging and has achieved outstanding commitment and contributions related to his/her professional field during the retirement years. The nominee maintains active involvement related to his/her professional field after the onset of retirement in either a paid or volunteer position.

Social or Political Advocacy: The nominee presents a positive image of aging and provides outstanding commitment and contributions to social and/or political issues. He or she maintains active involvement and leadership roles in community organizations committed to supporting specific social and/or political causes.

Volunteerism: The nominee presents a positive image of aging and has achieved outstanding commitment and contributions to the community as a volunteer to multiple community and/or governmental organizations.

Selection Process

Council on Aging staff will send an e-mail confirmation to both the nominator and the nominee when a nomination form is received. Staff will then review all nominations submitted to ensure eligibility. Staff will also exercise the option of moving nominees to other categories if appropriate.

After this review, all eligible nominees will be asked to submit a photo and may be contacted for additional information. If the nominee is ineligible, the nominator will be contacted. The Sage Awards Selection Committee, consisting of five individuals who are active and involved in our community, will review and score eligible nominations. Invitations to the Sage Awards will be mailed January 16th, 2016, six weeks before the event, which will be held Saturday, February 27th, 2016, at the Charles and Rae Kane Center. It is the Council on Aging’s intent to acknowledge and celebrate every nominee. All nominees will be recognized in pre-publicity, in the event program, at the dinner and during the award ceremony. Nominees who have scored the highest in each category will be presented with an award and given the opportunity to briefly address the guests.
