URBANA HIGH SCHOOL ATHLETICS ALL SPORTS REGISTRATION 2011-2012 Students must be in good financial standing to participate in any athletics or clinics. Students are considered to be in good standing if they are free from owing any money to Urbana High School. Debts may include, but are not limited to, textbook fines, registration fees, lock fees, and athletic equipment. Students will not be allowed to register for sports if they owe any fines. Fall sports sign ups: At UMS during all-school registration August 1-3, and August 4-9 at UHS. **freshmen please bring 2 copies of your physical to all-school registration Winter sports sign ups: October 2011 Spring sports sign ups: **Track: Indoor season January 2-13, 2012 (for those not in a winter sport); Outdoor season March 1-16, 2012. All other sports: February 2012 During lunch, passing periods, and before and after school

The following information is being provided for those students who are planning to participate in a sport during the 2011- 2012 school year. Sports by season FALL SPORTS: Boys/Girls Cross Country, Football, Boys & Girls Golf, Boys Soccer, Girls Swimming, Girls Tennis, Volleyball, Co-ed Cheerleading (Football) WINTER SPORTS: Boys Basketball, Girls Basketball, Boys Swimming, Wrestling, Co-ed Cheerleading (Basketball) SPRING SPORTS: Boys Track**, Girls Track**, Baseball, Softball, Girls Soccer, Boys Tennis

ELIGIBILITY All sophomores, juniors, and seniors must have passed a minimum of 5 classes at the end of the second semester last year (including any summer school courses) in order to be eligible to participate the first semester of 2011-2012. All students must pass 5 classes the first semester of 2011-2012 in order to be eligible to participate the second semester of this school year. Please refer to the Student Handbook for further information.

TIGER SPORTS SIGN UPS All athletes must be cleared through the Athletic Department, located in the Main Office. Dates are provided above. Each student must bring: - A completed Athletic Participation Form signed by the student and his/her parent (available in the Main Office). - We require that a student provide information verifying they have personal medical insurance or have purchased the voluntary student insurance. Students may not participate without medical coverage. - A current physical. Physicals are valid for one year from the date they are administered. Please keep a copy of your incoming 9th grade physical to turn in as we cannot access it. - $50 (cash, check, money order, or cashier’s check made payable to Urbana High School). Students with Free or Reduced Lunch status on file pay no fee or $25, respectively. The $50 fee is per student per athletic season. The maximum athletic fee per student for the 2011-2012 school year is $100. If a student does not make the team of his/her choice, this fee will be refunded upon request. If a student quits, does not come to practice, or is released from the team, he/she will not receive a refund.

Important: sign-up forms or money are not to be placed in mailboxes or given to any coaches. The Athletic Department will not be responsible for any forms or money placed in mailboxes or given to coaches.

Winter sports sign up will be in October during lunch and before or after school. Spring sports sign up will be in February 2012. Students must keep in mind they must pass 5 classes the first semester in order to be eligible to participate the second semester.

NOTE: The deadline for all fall sports sign-ups will be Friday, August 19th. Teams holding tryouts will not accept any new athletes once their tryout procedures are complete. Most teams who hold tryouts will complete these procedures within their first 3 to 5 practice days. Some students will be allowed to sign up beyond the given deadline if reasonable circumstances have prevented the student from completing the sign-up process on time. Students who wish to sign up past the August 19th deadline need to get approval first from the team’s head coach, then the athletics director.

Athletes are reminded to confirm practice times with their coaches at practice, as the Athletic Department may not have day-to-day changes. Athletes are also reminded to inform their parents of any changes to practice times.
