I, the undersigned, official veterinarian, certify that the animal designated above and examined on this day:

1. originates from a rabies-free country, in line with the recommendations of the World Organisation for Animal Health (OiE);

2. was held in a rabies-free country during the six months preceding its loading, or from birth;

3. has been examined in the last 24 hours, found to be in a good state of health and presented no clinical signs of any species-specific contagious disease on the day of loading;

4. was identified by a permanent mark, prior to its vaccination against rabies;

5. has been properly vaccinated against rabies with an inactivated vaccine: a. received a primary vaccination at least 30 days prior to loading, the vaccination having been administered on1 ………………, or2 b. received a booster vaccination less than one year prior to loading, the booster vaccination having been administered on1 ………………
