JOB TITLE Service Assistant – PAtH (Prevention of Admission to Hospital) BAND/Post No Level 1a REPORTING TO Service Co-ordinator PAtH

Context The Red Cross is the largest voluntary humanitarian organization in the world. It promotes humanitarian values and provides emergency help to people in short term crisis in the UK and over seas. Within the UK, the British Red Cross operates through 32,336 volunteers and 3,328 staff. Our success in making a difference to the communities in which we operate is dependent on the quality, availability and delivery of aid to those in crisis. Teams comprising Senior Services Managers, Service Managers and Service Coordinators deliver services.

Structure The post holders will report directly to a Service Co-ordinator.

Scope The post of PAtH Service Assistant will cover shifts based at the A&E Departments at the Stoke Mandeville Hospital, Slough and the Wexham Park Hospital, Aylesbury. The shifts required cover 11am to 4pm and 4.30pm to 8.45pm. Please note there may be the requirement to work to 9.30pm on occasions for which additional hours will be paid at your standard rate of pay. The exact hours and days to be worked each week depend on the post number of the role. The service is delivered on a daily basis with the post holder working shifts as per the rota and the role requires a visible presence within the fully operational units of the A&E Department with potentially distressing incidents being seen. This service will be delivered every day of the year excluding Christmas Day and Boxing Day You may also be required to cover for sickness and holidays for which you will be paid at your standard rate of pay and an additional hours timesheet should be filled out. This will be through mutual agreement between yourself and your line manager. The service is aimed at mainly older or vulnerable people in Buckinghamshire and will work closely with the Discharge Occupational Therapy team and A&E departments. One of the key responsibilities for the role is to provide excellent support for vulnerable people treating them with respect and dignity at all times. The service supports service users in A&E who require assistance to return home and to settle in. The service aims to prevent inappropriate admission to hospital. The service provides transport in specially adapted vehicles.

PAtH Service Assistant SMH 2016 Main responsibilities and duties To attend A&E Departments at Stoke Mandeville Hospital and/or Wexham Park Hospital and provide transport and support, at the point of discharge from A&E, to older or vulnerable patients. To undertake patient assessments, complete risk assessments for referrals from the A&E team and support their discharge home. To ensure daily procedures are completed to ensure the service vehicle is compliant with CQC regulations 1. To transport people home from hospital, in a wheelchair accessible vehicle, settling them in and assisting with low level support tasks and their well being. 2. To maintain a duty of care to all service users, staff and volunteers by ensuring that full risk assessments are carried out for all procedures. 3. A reasonable level of physical fitness is required to push wheelchairs up ramps, secure and tie down wheelchairs in accordance with manual handling requirements. 4. To complete next day follow up calls to service users to check on their well-being, logging any concerns and signposting on if required. 5. To complete daily procedures to ensure service vehicle is compliant to CQC standards. 6. To support A&E Departments with additional duties ie medication collections, emotional support during times when the crews are not responding to referrals. 7. To be aware of Health and Safety regulations concerning their service and their own responsibility for Health and Safety. 8. To comply with all procedures when driving a Red Cross vehicle. 9. To complete daily vehicle checks and ensure CQC compliancy. 10. To signpost patients to other organisations as required and highlight any concerns as appropriate. 11. Handling and securing of monies in line with Society procedures. 12. To maintain visibility and develop excellent working relationships within the hospital(s) to raise awareness of the British Red Cross role. 13. To ensure all service users are treated with respect and dignity and are provided with excellent support whilst in the care of the BRC. 14. To maintain accurate and comprehensive records of service delivery and maintain service user confidentially at all times. 15. To maintain accurate logs relating to vehicles – recording mileage/vehicle checks/ CQC compliance/ reporting accidents and near misses. 16. PAtH Service Assistant WPH 2016  Ability to input into the assessment D process under guidance.  Able to respond to emergency situations and be part of the emergency call out E* procedures.  Hold a full manual driving licence valid for use in the UK with no more than 3 D penalty points.  Hold category D1 on licence or equivalent licence to enable post holders to drive disabled passenger E* vehicle (minibus with tail lift).  Able to travel appropriate to the needs of the job. D  Ability to work additional hours to support service during planned or E* unplanned absences.  Ability to clean vehicles to CQC standards
