SCHOLARSHIP ELIGIBILITY & SELECTION OF RECIPIENT: The primary purpose of this Fund shall be to provide a scholarship to a Wabash County High School graduate or current resident who is entering graduate school to study in the area of ministry, missionary work, or social work.

REQUIREMENT 1 - STUDENT AGREEMENT: Please read carefully the statements below, and initial each one, indicating your agreement.
In submitting this application, I certify that the information provided is true and complete to the best of my knowledge, and the essay work is my own.
If I am selected as a scholarship recipient, I agree to permit the Community Foundation of Wabash County to confer with my school to verify my continuing enrollment during the term of my scholarship. I also agree that my name and photograph can be used in announcements made by the Community Foundation of Wabash County regarding this scholarship.

Scholarship Conflict of Interest Statement: In connection with any actual or possible conflict of interest, a Community Foundation of Wabash County employee, Board of Directors member, or committee member will disclose that one or more family members (including spouse, brothers or sisters by whole or half blood, ancestors, children, grandchildren, or great grandchildren) is in the pool of scholarship applicants and will, therefore, recuse him/herself from the entire scholarship nomination process for that pool, including all meetings, discussions, debates, and votes regarding nomination. All scholarship endowments listed are held and administered by the Community Foundation of Wabash County.
