1. Step 1: Requestor Information - Complete for all SAP vendor requests. 2. Step 2: Vendor Update Options - Select one of the five options, including:  Add a new vendor: complete this form and request the vendor submit a substitute W – 9 (or vendor’s W – 9). NEW VENDORS WILL NOT BE ADDED UNTIL A W-9 HAS BEEN PROVIDED.  Change a vendor name: complete this form and request the vendor submit a substitute W – 9 (or vendor’s W – 9). VENDOR NAME CHANGES WILL NOT BE PROCESSED UNTIL A W-9 HAS BEEN PROVIDED.  Change a vendor’s address, phone or FAX number: complete this form.  Partner with purchase order: complete this form.  Add additional “Order To” or “Remit / Payment To” information: complete this form. 3. Step 3: Vendor Order and Remit / Payment Information – Update information if Change Vendor Address / Phone / FAX was selected in Step 2. 4. Step 4: EMAIL [email protected] the SAP Internal Vendor Request. Vendors should email W-9 forms to the same address.
