VENDOR NAME CHANGES WILL NOT BE PROCESSED UNTIL A W-9 HAS BEEN PROVIDED.  Change a vendor’s address, phone or FAX number: complete this form.  Partner with purchase order: complete this form.  Add additional “Order To” or “Remit / Payment To” information: complete this form. 3.
