About the service we inspected

This service registered with the Care Inspectorate on 12 October 2017.

Fullarton Care Home is registered to provide a care home service (with nursing) to a maximum of 90 older people. The provider is HC - One Oval Limited.

The service is located in a residential area of Irvine, North close to local amenities, shops, and transport links. The care home is purpose-built with accommodation across four units in two buildings, Acorn and Cedars which have 25 beds each and Iona and Skye which have 20. Each building has two floors connected by a passenger lift. All bedrooms are single with en-suite facilities and assisted bathing and showering facilities are provided in each unit.

There are a number of smaller lounges in addition to the main lounge/diners. Residents also have access to an enclosed garden area.

The stated aim of the service is: "To ensure that residents receive the kindest possible care. Our extensively trained team will deliver all the nursing, specific orf every resident to continue their daily lives with privacy, dignity within a safe caring environment."

How we inspected the service

We compiled this report following an unannounced visit over one day, by two inspectors. We gave feedback to the area quality director on 6 July 2020.

The focus of the inspection was to assess progress the service had made following the last inspection in May 2020.

During the inspection, we spoke to people living in the home to find about their experience of living in Fullarton Care Home.

We spent time observing how staff supported and interacted with the people living in the home. Directly observing care is an important way to help us judge whether a service complies with the regulations and meets the outcomes for people.

We spoke with the area quality director for the home, nursing staff, senior care staff, care staff, housekeeping staff, activity staff and catering staff.

We looked at the service's action plan, training records and a range of key documents.

Taking the views of people using the service into account

People we spoke with commented that staff were friendly.

People told us that they enjoyed getting out into the garden and helping with planting flowers and vegetables. One person said "it does me good to get out into the fresh air".

We were told that the food was good.

Taking carers' views into account

We were unable to speak with visiting family members as visiting to the home was restricted in line with current infection prevention and control guidance.

What the service has done to meet any requirements we made at or since the last inspection

Previous requirements

Requirement 1

By 6 July 2020 the provider must ensure that all staff (including domestic and laundry) are trained in infection control procedures appropriate to managing an outbreak of COVID-19 and are following all appropriate Public Health guidance COVID-19 Information and Guidance for Care Home Settings.

This is to comply with Regulation 15(b) - ensure that persons employed in the provision of a care service receive (i) training appropriate to the work they are to perform. Social Care and Social Work Improvement (Requirements for Care Services) Regulations 2011 (SSI2011/210).

This requirement was made on 27 May 2020.

Action taken on previous requirement Staff working in the home had completed training in line with Health Protection Scotland COVID-19 Information and Guidance for Care Home Settings. We saw that staff practice reflected an understanding of infection control and protection procedures. This included the consistent use of Personal Protective Equipment (PPE), use of social distancing and regular cleaning of equipment and 'touch-points' in the home.

We found that the general cleanliness of the home had improved, and a satisfactory standard was being maintained.

People living in the home were being safeguarded as they were being supported by a staff team who adhered to current infection control and protection procedures.

Requirement 2

By 6 July 2020 the provider must take account of the increased dependency levels of people who are isolated in their bedrooms and ensure that there are sufficient numbers of staff available to meet the health, welfare and safety needs of everyone living in the home.

This is to comply with Regulation 15(a) - ensure that at all times suitably qualified and competent persons are working in the care service in such numbers as are appropriate for the health, welfare, and safety of service users. Social Care and Social Work Improvement Scotland (Requirements for Care Services) Regulations 2011 (SSI2011/210).

This requirement was made on 27 May 2020.

Action taken on previous requirement The provider had re-established methods of assessing individual's dependency levels and care needs. The assessments were carried out on a monthly basis or more often if an individual's needs changed. The outcomes of dependency assessments informed the amount of care hours and the skill mix of staff needed to meet the care and support needs of people living in the home.

We saw that recent assessments had resulted in a review of the care hours allotted to the individual units in the home. This ensured that there were sufficient staff available to meet the care and support needs of people.

We saw that staff were well deployed and were aware of their roles and responsibilities when on duty. During our inspection visit we saw there were sufficient numbers of staff available to meet the health, welfare and safety needs of everyone living in the home.

Requirement 3

By 6 July 2020 the provider must ensure that the nutritional and hydration needs of all people living in the home are fully met to maintain their healthcare needs.

In order to do this the provider must:

- review the timing of mealtimes to optimise the opportunities to ensuring adequate intake of calories and nutrients; - review the management of mealtimes to ensure they are well led and staff are deployed effectively to support people with eating and drinking; and - ensure provision of food appropriate to people's needs. This is with reference, but not limited to, people living with dementia who are restless and have difficulty concentrating at mealtimes.

This is to comply with Regulation 4(1) (a)- a provider must make proper provision for the health, welfare, and safety of service users. Social Care and Social Work Improvement Scotland (Requirements for Care Services) Regulations 2011 (SSI2011/210).

This requirement was made on 27 May 2020.

Action taken on previous requirement The timing of mealtimes had been reviewed. This allowed more time between meals and supported the staff team to ensure that people had an adequate intake of calories and nutrients throughout the day.

We saw that mealtimes were calm, and staff had allocated duties to support people with eating and drinking. The mealtime experience could be further improved by ensuring that one person coordinated staff and there was an overview of the management of the meal.

There were choices on the menu which were appropriate for people who were restless or had difficulty concentrating at mealtimes. We saw that staff were aware of the most suitable food choices to offer people to support their individual needs. This helped to support the health and wellbeing of the people living in the home.
