TREETREE OFOF LIFELIFE

The Tree of Life is a spectacular 6.5- foot bronze tree with gold leaves. The tree is displayed on the Residential Wing of Kitty Askins Hospice Center, in the family room.

3HC is pleased to offer an opportunity for you to honor your loved one through the dedication of a leaf. Each leaf will become a permanent memorial on the Tree of Life and will be engraved. We are asking for a minimum donation of $100 for each leaf ordered, a donation that will help meet the needs of hospice patients at the Kitty Askins Hospice Center and throughout eastern NC.

While we try to accommodate request for special placement of the leaves, we are not always in a position to honor the request.

Leaves are engraved throughout the year with a Dedication Ceremony to follow. You will receive an invitation to the ceremony taking place closest to the completion of your leaf*. During 2012, this form must be received by December 19th for the ceremony on January 19th; March 19th for the ceremony on April 19th; June 19th for the ceremony on July 19th; or September 18th for the ceremony on October 18th.

*Dates of the ceremonies are subject to change.
