<p> TREETREE OFOF LIFELIFE</p><p>The Tree of Life is a spectacular 6.5- foot bronze tree with gold leaves. The tree is displayed on the Residential Wing of Kitty Askins Hospice Center, in the family room.</p><p>3HC is pleased to offer an opportunity for you to honor your loved one through the dedication of a leaf. Each leaf will become a permanent memorial on the Tree of Life and will be engraved. We are asking for a minimum donation of $100 for each leaf ordered, a donation that will help meet the needs of hospice patients at the Kitty Askins Hospice Center and throughout eastern NC.</p><p>While we try to accommodate request for special placement of the leaves, we are not always in a position to honor the request. </p><p>Leaves are engraved throughout the year with a Dedication Ceremony to follow. You will receive an invitation to the ceremony taking place closest to the completion of your leaf*. During 2012, this form must be received by December 19th for the ceremony on January 19th; March 19th for the ceremony on April 19th; June 19th for the ceremony on July 19th; or September 18th for the ceremony on October 18th. </p><p>*Dates of the ceremonies are subject to change. </p><p>------Please PRINT or TYPE clearly.
