Travel Quarantine for Maui County • The form is available the County’s website at www.MauiCounty.gov. • We are discouraging any non-essential travel at this time. • We continue to ask for everyone’s help and cooperation as we carry out this travel process. Police and Park Rangers Enforcing Emergency Rules • Park Rangers will be enforcing the County of Maui Public Health Emergency Rules and issuing citations for violations. • This includes violations for gatherings of more than 10 people, disregard of physical distancing and use of large structures in beaches and parks. Police and Park Rangers Enforcing Emergency Rules • Maui Police Department also continues to enforce the mandatory 14-day Quarantine Order for transpacific and interisland travelers. • To report a suspected violation of the 14-day quarantine, email [email protected]. • Mahalo to all our residents who have kept our families and communities safe, and avoided large gatherings. Mandatory Travel Declaration Form • The Mandatory Travel Declaration Form is for all interisland arrivals within Maui County. • For Maui County residents departing to another Neighbor Island County – you must visit their county’s website to fill out their travel forms. Mandatory Travel Declaration Form • For example, if you are traveling to Kauai then you need to visit Kauai County’s website to complete their travel forms • There are no travel forms or quarantine for travel to Oahu. • HOWEVER, residents will need to complete the County’s Mandatory Travel Declaration Form for their return trip back home to Maui County. Mandatory Travel Declaration Form • There are two processes for the Mandatory Travel Declaration Form on the County’s website, depending on how you are traveling. Mandatory Travel Declaration Form • An online form is available for arrivals at: • Kahului Airport Main Terminal • Lanai Airport • Molokai Airport Mandatory Travel Declaration Form • A paper form that must be printed out is for arrivals at: • Kahului Airport Commuter Terminal • Kahului Airport Private Jet • Lahaina and Manele Small Boat Harbors • Kapalua Airport NOTE: Paper forms will also be available at these locations. Mandatory Travel Declaration Form Mandatory Travel Declaration Form • Personnel are located at the airports and harbors to assist travelers with the forms and process. • For any questions, call the Office of the Mayor at (808) 270-7855 or email [email protected]. Request for Limited Quarantine Form • Interisland travelers may request a limited travel quarantine due to essential work or medical reasons. • Requests should be completed through the County’s website at least five (5) days prior to departure. • Please do not send requests to the old email that was initially provided. Request for Limited Quarantine Form Modified Quarantine Exemptions • Interisland travelers should be prepared to provide: • Government issued ID for all travelers • Medical Letter from treating physician • Essential Work letter from employer Modified Quarantine Exemptions • Examples of requests that do not qualify: • Providing a negative COVID-19 test • Trips to see family and/or friends • Travel identified as personal errands • Vacation Modified Quarantine Exemptions • IMPORTANT NOTE: Requests for Limited Quarantine only allow travelers to conduct their essential function as well as travel to and from their essential functions. • For example, an essential worker would only be allowed to leave their place of lodging for essential work purposes. • Also, the purpose of your trip needs to be for essential work or medical reasons. If you are an essential worker and visiting family on the Big Island, then you will NOT be granted limited quarantine when you return home to Maui County. Transpacific Travelers Transpacific Travelers • All transpacific travelers must complete the State of Hawaii Traveler Health Form prior to boarding at https://health.hawaii.gov/travel/. • Transpacific travelers seeking a modified travel quarantine must email requests to the State at [email protected]. Lito Vila Administrator, Department of Motor Vehicles & Licensing Road Test Appointments in Maui County • The Department of Motor Vehicle and Licensing will begin scheduling modified road test appointments, starting tomorrow, Aug. 13. • The modified road tests will be administered in phases, with the First Phase only open to individuals with permits expiring prior to Aug. 1, 2020. • NOTE: The State has extended expired permits until Sept. 30, 2020.
