QUESTIONNAIRE FOR CONSULTANT SERVICE WIDE SAFETY MANAGEMENT SYSTEM (SMS)

Class I : (LIH), (HNL), Airport (MKK), (LNY), (OGG), (JHM), Hilo International Airport (ITO) and Kona International Airport at Keahole (KOA). General Aviation Airports: (PAK), (HDH), (JFR), (LUP), (HNM), Upolu Airport (UPP), and Waimea-Kohala Airport (MUE). IMPLEMENTATION PROGRAM Experience and professional qualifications relevant for the project For the following qualifications, please provide a response for each of the items below detailing the number of airports, project scope, staff involvement, duration, and key deliverables: 1. Extensive recent experience in developing and developing and implementing SMS programs at United States Part 139 certified airports. 2. Demonstrated knowledge with Part 139 requirements and experience in integrating an SMS program to an airport’s Part 139 program. 3. Experience integrating SMS and Wildlife Hazard Management, 4. Extensive and recent experience in developing and authoring SMS manuals for Part 139 certificated airports. 5. Experience developing and deploying SMS inspections and audits including development of relevant, practical, and useful SMS program metrics. 6. Expertise and Experience in U.S. CFR Part 139 airport management and operations. 7. Expertise and Experience developing operational, transition, and integration plans for existing and new SMS management and organizational staff positions. 8. Experience developing airport system wide SMS strategies for various size airports and operations. 9. Knowledge and familiarity with Federal Aviation Administration (FAA) Advisory Circular 150/5200-37A (Introduction to Safety Management Systems (SMS) for Airport Operators 2012). 10. Knowledge and familiarity with FAA Order 5200.11 for system-wide Safety Risk Management (SRM) integration for operations, planning, engineering, and construction. 11. Extensive experience in developing (SRM) programs and facilitating safety assessments. 12. Knowledge and familiarity with FAA Advisory Circular 150/5370-2F – Operational Safety on Airports during Construction for program integration. 13. Extensive knowledge of available SMS Software Systems including requirements definition, selection, integration, implementation, testing and staff training. 14. Proven ability in designing, developing, and delivering training materials/techniques for SMS programs including collaboration with local academic institutions and colleges. Past performance on projects of similar scope at US Airports including development, implementation, training, and documentation of SMS systems. For US airports only where your firm has provided SMS services, please list the following: a) Project title, location, year completed and scope b) Client’s name and phone number c) Client’s primary project manager or contract administrator d) Successful project elements e) Project challenges and how they were overcome f) How successes or lessons learned can apply to this contract Capacity to accomplish the work in the required time 15. Please provide your firm’s organizational chart showing how this project will be staffed by the following classifications, and the name of the employee(s) per classification. Please submit a resume of your key employee(s) for those classifications denoted by an “*”. Resumes shall be limited to one page maximum per person. a) SMS Program Manager/Principle Investigator b) Part 139 Compliance and Integration Expert(s) c) SMS Development and Implementation Expert(s) d) SMS Trainer(s) 16. For each key employee(s) identified in the organizational chart, please list all of their current projects(s), and the expected project completion date(s). 17. Does your firm close its office for any extended period of time during the year? If yes, please identify the dates. How will your firm respond to any request for services during this time period? 18. The following are Airports policies with regard to consultant contracts and will apply to your firm and subconsultant(s). Please provide an explanation for any of the conditions that your firm cannot meet: a) Fee proposal shall be formatted by the above classifications, whose efforts will be compared to those efforts of the equivalent Airports Division’s classifications. The clerical classification will not be considered a project chargeable classification, and shall be considered as an overhead expense. b) The direct labor rates shall be the actual rates, supported by the latest payroll register for each employee. Hourly rates will be based on 2,080 hours per year. If there are multiple employees per classification, then an average rate shall be used, If payroll register are not available, then the latest DAGS maximum hourly rates will be used. There will be no increase to the direct labor rates during the contract time period. c) The maximum allowable multiplier factor (overhead rate and profit) shall be 2.88. However, the overhead rate shall be the actual overhead rate, subject to an Airports Division or any other Department of Transportation Division audit. Federal audits will not be accepted. The profit rate shall be limited to 10% maximum. d) If your firm is selected, your firm will be obligated to staff the project with those key employee(s) listed in the organizational chart submitted in question 10. Key employee(s) shall not be replaced, unless they are no longer employed by your firm, and their replacement shall be subject to the Airports Division’s approval. e) The maximum allowable multiplier factor (overhead rate and profit) shall be 2.88.
