Intrasurgical posterior capsule tears are the most common and can occur during any stage of cataract surgery. The incidence of posterior capsule complications is related to the type of cataract and conditions of the eye, increases with the grade of difficulty of the case, and furthermore is influenced by the level of experience of the surgeon. Timely recognition and a planned management, depending upon the stage of surgery during which the posterior capsule tear has occurred, is required to ensure an optimal visual outcome. COMMON RISK FACTORS FOR POSTERIOR CAPSULAR RUPTURE (PCR): 1. Intraoperative factors causing variation in anterior chamber depth 2. Type of cataract 3. Extended rhexis Intraoperative factors causing variation in anterior chamber depth Intraoperative shallow anterior chamber could be due to various reasons. It may be a tight lid speculum, tight drapes, or pull from the recollecting bag. In all the above cases, remove the precipitating factor (Remove the speculum pressure, remove the tight drapes and collecting bags). Variation in the amount of space in the anterior and posterior chambers may result from changes in the intraocular pressure (IOP) due to an alteration in the equilibrium between inflow and outflow of fluid. Diminished inflow may be secondary to insufficient bottle height, tube occlusion or compression, bottle emptying, too tight incisions compressing the irrigation sleeve, or the surgeon moving the phaco tip out of the incision, making the irrigation holes come out of the incision.

We would like to bring to the author’s Research Centre, 2Dr Agarwal’s notice that one of us (SJ) described the use of external endoillumination for DMEK, Refractive and Cornea Foundation (DARCF), Chennai, India and the same was published under our names in 2014 as endoilluminator-assisted transcorneal illumination for DMEK,2 with study patients included from January 2013. There are also numerous other references to this online.3–6 We have termed this tech- nique endoilluminator-assisted DMEK or E-DMEK and endoilluminator-assisted pre- Descemet endothelial keratoplasty (E-PDEK) when applied to PDEK.7 E-DMEK makes the surgery simpler by allowing good visualization and better surgeon comprehension of graft morphology and dynamics and enhanced visualization and three-dimensional depth perception through edematous and hazy corneas (Figure 1). The technique also aids the surgeon with better comprehension of graft orientation and positioning (Video S1). We would like to gently bring to the authors’ attention that our work was the initial report on E-DMEK, and that this has been missed in their citation.
