Provides services for persons over 18 who are physically or mentally impaired and who are in a situation where they are harmed or threatened with harm by the actions of themselves or others. This often involves the frail elderly and adults who are physically or mentally disabled, seriously ill or alcohol/substance abusers. Potential clients have no one able or willing to help them meet their essential needs such as food, clothing, shelter, and for medical care. Any concerned person or agency should make a referral for services using the three numbers provided above. All calls are confidential and may be made anonymously.

The Sheriff’s Office regularly visits senior centers, VFW halls and other community groups to provide emergency medical alert ID cards. The cards contain important information such as their identity, personal doctors, next of kin, medical conditions and medications, and who to contact in case of an emergency. The cards are meant to be carried at all times to assist emergency services personnel in providing the best possible care in the event the individual is unable to communicate their needs and other critical information*.

The Suffolk County Office for the Aging is the designated Area Agency on Aging under the Older Americans Act. For over 40 years, Suffolk County Office for the Aging has administered federal, state, and county programs for persons 60 years of age and older.

Specific goals for the Office are to develop a comprehensive service system designed to assist persons 60 years of age and older in maintaining maximum independence and dignity, to remove barriers to economic and personal independence, and to provide a continuum of care for the vulnerable elderly. Most importantly, Suffolk County Office for the Aging acts as an advocate on behalf of all Suffolk County senior citizens.

Initiated in 1969, Housing Services (LIHS) is a HUD-approved counseling agency and Long Island’s only HUD qualified Fair Housing Enforcement Organization. The Anchor Partner Program of the NYS Attorney General’s Homeownership Protection Program (HOPP) is coordinated by the Empire Justice Center on Long Island. The Program staff provides and coordinates legal services and housing counseling services related to HOPP and Super Storm Sandy out of the Central Islip office at the Touro Law School Public Advocacy Center. HOPP works with homeowners to help them understand their finances, connect them to free legal services, and even speak to the bank on their behalf. All this help is available in many languages, and is offered free of charge.

Catholic Charities' housing department provides seniors who meet income guidelines with the opportunity for safe and affordable housing and access to a supportive network of services. The program currently operates 16 housing facilities in Nassau and Suffolk counties, totaling more than 1,329 units.

Catholic Charities' housing department works to ensure the physical, social, and spiritual well-being of residents. Dedicated staff arranges social and educational events at the sites, identifying seniors’ needs, and help them access services for which they are eligible.

Apartments are available in Floral Park, Freeport, Inwood, Oceanside, Uniondale, Amityville, Brentwood, Central Islip, Coram, Hampton Bays, Medford, and Selden.

Fee: Rents vary by site. For some sites, rent is calculated as a percentage of the resident's income. Other sites have fixed rent.

Eligibility: Seniors 62 years and older who meet income guidelines are eligible with the exception of St. Anne's Gardens, which accepts seniors who are 60 or older. CARECEN has provided high quality legal assistance to immigrant communities in Nassau and Suffolk counties for over thirty years. SEPA Mujer, Services for the Advancement of Women is a community-based not for profit organization dedicated to the empowerment of Latina immigrant women through education and advocacy, legal rights and leadership skills training, and community organizing. The organization was formed in November 1993 by Long Island women, both immigrants and advocates, in response to the need of Latina immigrant women to educate, support and organize in order to improve their condition as one of the most vulnerable groups on Long Island and in the country.

For over 20 years, SEPA Mujer has trained community organizations and public agencies on special immigrant relief under the Violence Against Women Act, facilitated workshops on community organizing and other topics relevant to the community, and worked with service providers and law enforcement to appropriately respond to the needs of Latinas. SEPA Mujer focuses its legal services on relief for immigrant survivors of violence, and provides free legal representation and referrals to support battered immigrant women in leaving abusive relationships.

The Crime Victims Center (CVC) is a community based not-for-profit organization providing trauma informed, comprehensive bilingual support services to child and adult victims of sexual assault, victims of domestic violence, human trafficking, survivors of homicide, all victims of violent crime and elderly, minor and disabled victims of all crime. The CVC is a NYS Rape Crisis Center and the Suffolk County Crime Victims Center, providing a 24-Hour Crisis Hotline, victim support services, community-based prevention and education programming in schools and community organizations and Megan’s Law community and law enforcement support programs.

Free short and long-term counseling is available for children and their non-offending family members by an agency NYS Licensed Psychologist or Social Worker. Both our Ronkonkoma and Patchogue offices are equipped with full play therapy rooms designed to work with child victims of sexual abuse, domestic violence, and other violent crimes. Agency Certified Rape Crisis Counselors are available for courtroom and medical appointment accompaniment, criminal justice and family court advocacy, crisis support, and information and referrals for concrete services and medical and legal assistance. Assistance is also provided in applying for compensation for reimbursement for expenses related to crime.

The Long Island CASA Programs are foster care advocacy programs in which trained volunteers assist the Family Court by fact-finding, interviewing, and reporting on children in foster care. Volunteers, known as Court Appointed Special Advocates (CASAs) are interested men and women from all walks of life, trained by EAC Network to work with program staff and the courts. When a case is assigned to a CASA volunteer, (s)he will speak with caseworkers from public or private agencies that are caring for the child. (S)he will interview caseworkers, and will make visits to the child in the foster home. Other parties to the case who may be contacted include school officials, therapists, doctors or family attorneys.

The goal of Project Access is to improve non-custodial parents’ access to and visitation with their children, and improve their ability and willingness to participate fully in financially supporting their children and being part of their lives in meaningful, responsible ways. The Suffolk County Child Advocacy Center at The Pat & Mary Bagnato Place For Kids is a facility for child victims of physical and sexual abuse. The mission of the Child Advocacy Center is to provide a neutral, child-focused environment where a coordinated response to child abuse allegations may be pursued in ways that support and protect children, non-offending parents and other family members. Each staff member is a certified Emergency Room Companion who is on call 24 hours a day to meet child victims of sexual assaults at hospital emergency rooms. In addition the Child Advocacy Center hosts public education forums on issues related to child abuse as well as providing referrals, accompaniment to court, offering much needed emotional support to the child and families and assistance in completing forms for Crime Victims Board reimbursement. Child Advocacy Center staff facilitates a peer support group for child victims and their non-offending parents called the Kids 2 Kids program.

EAC Network’s Suffolk Children’s Center at Cohalan Court offers an alternative to crowded courtrooms and waiting areas and enables parents with young children to attend to their court business. The Children’s Center is a safe, secure and nurturing environment for children ages 6 weeks to 12 years of age. All are cared for by trained, professional childcare providers and volunteers. While at the center the children are engaged in supervised free play, arts and crafts, age appropriate computer games, access to a library of children’s books and also receive healthy snacks. Our staff promotes Literacy by giving a new book to each child that attends the center and encourages the parents to read to their children. Community Referrals are also provided to parents and staff assists them in getting any type of services they might need for their children.

Supervised Visitation provides supervision for visits between parents and children when the court feels that unsupervised access may be dangerous and harmful to the child. Families are directed to the program through a court order. After an intake and orientation process, a visiting schedule is determined. Non- custodial parents and their child(ren) generally visit once per week for an hour at EAC Network facilities. Off-site supervision is also available if directed by the court. Program staff monitors visits and provide reports to the court on the progress of visits. Parents also receive help in improving parenting skills, learning appropriate play behavior and help in developing positive relationships with their children. Visiting hours are scheduled for evenings and weekends, at a convenient location in Ronkonkoma. There is a sliding scale fee for services which supplements grant income.

Supervised Exchange Services: • Exchanges may take place at our center or off-site at a neutral location. • Exchanges may take place at the home of either party, when appropriate.

Parenting Education Services: • “Common Sense Parenting” is a research-based curriculum for parents of children ages 2-16. • Participants receive a parent handbook to keep, along with all course material. • Participants who fulfill the necessary requirements receive a Certificate of Completion. • Workshops are facilitated by certified parent trainers as well as professionally trained staff.

Home Evaluation Services: • Evaluators extensively review the home; evaluations are detailed, accurate and thorough. • Parties may provide a detailed listing of concerns regarding the home to ensure all areas are addressed.

Founded in 1986, Brighter Tomorrows, Inc. is a 501(c) (3) human service agency dedicated to empowering survivors of domestic violence through the provision of safe housing, supportive counseling, and legal advocacy services on Long Island. Brighter Tomorrows’ value-driven programs seek to connect women and children with a nurturing community while enhancing their personal security, independence, and self-esteem.

Brighter Tomorrows services clients from Nassau and Suffolk Counties, as well as the Five Boroughs, the Tri-State, and across the East Coast. Our secure domestic violence shelter offers clients the opportunity to begin to recover from their abuse in a safe, structured environment. The residential program includes case management services, clinical support groups, educational workshops, transportation, and childcare. Brighter Tomorrows’ innovative transitional housing program facilitates independent living for previous residents of our domestic violence shelter and for survivors of human trafficking. Clients are provided with ongoing support services and referrals while they attend degree programs or vocational training and seek out permanent employment and housing. In addition to our shelter services, Brighter Tomorrows offers individual and group counseling in a non-residential setting and staffs a 24-hour crisis hotline. The Long Island Coalition Against Domestic Violence offers a holistic program of services. Recognizing that domestic violence affects all aspects of a victim's life, we address as many needs as possible. Bi-lingual (English and Spanish) services are available for those seeking counseling and in the precincts and courthouse. The Retreat’s mission is to provide safety, shelter and support for victims of domestic abuse and to break the cycle of family violence.

The Retreat is a community-based, not-for-profit domestic violence services agency serving the North and South Forks of Long Island since 1987. The Retreat provides a wide array of direct services including a local residential shelter for women and children, a 24-hour domestic violence crisis hotline, individual and group counseling, legal advocacy for all victims of domestic violence, programs to engage troubled fathers and other men who may be at risk of committing family violence, and an in-school violence prevention education program. The Office of Victim Services may be able to help with medical bills and counseling expenses; burial and funeral costs; lost wages; and other types of assistance. Established in 1976, Victims Information Bureau of Suffolk (VIBS) has been a pioneer in the domestic violence and rape crisis movements. Our mission is to assist the survivors of domestic violence, rape and sexual assault; to prevent the incidence of these crimes through education and services; and to raise community awareness of the need for justice and compassion for victims. Children’s Single Point of Access (C-SPOA) The Children’s Single Point of Access system began in 2001 to simplify and coordinate the process of linking children with serious emotional disturbance and their families to the services that can assist in meeting their needs. The goal of SPOA is to identify children at highest risk of placement in out-of-home settings; develop appropriate strategies to manage those children in their home communities; and monitor and track progress through system. The Children’s SPOA is characterized by a multi-disciplinary representation of agencies. Referrals may be made to SPOA for children, age five through 18 years, with a major psychiatric diagnosis who meet the NYS OMH criteria for Serious Emotional Disturbance (SED).

C-SPOA In-Home Programs Supportive Case Management (SCM), Intensive Case Management (ICM) & Non-Medicaid Care Coordination/Intensive Case Management (NMCC/ICM) All develop service plans based on the specific needs and desires of the child and the family. Programs provide care coordination services, including service linkage, coordination and monitoring and crisis intervention to maximize each youth’s potential for growth and emotional stability within their natural support system and maintain the youth in their home and community. The Medicine Program assists patients who cannot afford to pay for prescription medication. This program is used when a consumer is unable to pay for medications, pending Medicaid approval. There are also some pharmaceutical companies that offer free medications to low- income families. They require a doctor’s consent and proof of your financial status. Depending on what your insurance covers, you may be able to apply. A few companies allow family incomes as high as $40,000 annually (offset by expenses). CARECEN has provided high quality legal assistance to immigrant communities in Nassau and Suffolk counties for over thirty years. We currently offer a variety of legal services in both affirmative and defensive cases, including initial and renewal requests for temporary protected status (TPS) and Deferred Action for Childhood Arrivals (DACA), green card renewals, naturalization, adjustment of status, family petitions, unlawful presence waivers, special immigrant juvenile status (SIJS), among others. Pathway to Citizenship Long Island: This program provides free citizenship legal services, credentialing of foreign-earned degrees and free financial counseling for lawful permanent residents.

Refugee Child Unit: This unit conducts legal outreach and intake for unaccompanied children that have resettled in Nassau and Suffolk counties, assessing avenues of legal relief and representation for children that are currently in deportation proceedings.

Family Petitions: As a permanent resident or U.S. citizen, you are able to petition for eligible relatives to become lawful permanent residents in the United States.

Adjustment of Status: This is a process by which an eligible applicant can apply to become a permanent resident of the United States.

Deferred Action for Childhood Arrivals (DACA): DACA is a program through which individuals who arrived to the U.S. before their 16th birthday and meet other eligibility requirements may apply for work authorization, in addition to not being placed in deportation proceedings or removed from the U.S. for a period of two years.

Temporary Protected Status: Temporary Protected Status is granted to individuals that are a national of a country designated for TPS or individuals without nationality who last habitually resided in the designated country and who meet other eligibility requirements.

Advocacy and Organizing: Advocacy and organizing are coordinated to address laws, policies, and practices that are unfair and inhumane toward immigrants. Many immigrants - including increasing numbers of legal, long-standing, and elderly immigrants - find the system unresponsive when they seek emergency services and benefits to which they are entitled, such as Medicaid or food stamps. Advocacy helps cut through the red tape on a case-by-case basis. Working with the U.S. Bishops, broad-based community organizing efforts unite the voices of immigrants seeking systemic changes to improve immigration laws and access to public benefits. Vigorous voter registration is central to the empowered participation of immigrants and their families.

The Adult Home Project is a bi-county unit representing adult home residents in matters relating to their financial accounts, the conditions of the homes, and the treatment of the residents.

The Consumer Debt Legal Assistance Project provides advice and/or representation to persons with consumer debt issues, especially in matters of medical and credit card debt.

The David Project Assists AIDS/ HIV-infected persons and their families with legal issues affecting public benefits, housing, health care, and planning for the future through wills, guardianships, and advance directives for medical treatment. In addition to legal services, the David Project provides a range of support services to HIV affected families with minor children in Nassau and Suffolk counties.

The Disability Advocacy Project (DAP) assists disabled persons in appealing the denial of Supplemental Security Income (SSI) and Social Security Disability (SSD) applications.

Education and Disability Rights Project (EDRP) assists income eligible families in protecting the education rights of students with disabilities. Representation includes attendance at Committee on Special Education (CSE) meetings, Resolution Session, Impartial Hearing and appeals to the State Review Office. In addition, eligible individuals of all ages are assisted in the negotiation and appeal of adverse eligibility decisions pertaining to disability related services/programs, including programs funded through the Office of People with Developmental Disabilities (OPWDD), and advocacy for effective life transitions for individuals with disabilities.

Foreclosure Prevention Project assists homeowners to prevent foreclosure, through loss mitigation options and/or legal challenges. The Project is part of a state wide coalition that includes HUD-certified housing counseling agencies. Clients must reside in the homes subject to foreclosure and whose income is within the range of 80% of the median income for Long Island set annually by HUD.

Independent Consumer Advocacy Network (ICAN) provides education, information and advocacy regarding

Long-Term Managed Care including:

The Mental Health Law Project assists persons with a history of serious mental illness, primarily in matters of public benefits, family law, health care, and housing.

The Permanency Legal Assistance Needs Project (PLAN) provides legal assistance to individuals and families affected by every type of cancer, so that they can better cope with legal, financial and medical issues, including permanency planning. Permanency planning assists families to develop a legally enforceable long-term plan for the care of children, and to assure that the client's own wishes regarding end of life decisions are followed.

The Veterans’ Rights Project provides representation, consultation, and information to low income veterans. Services include assistance with veterans’ benefits, discharge upgrades, public benefits, landlord/tenant, and child support matter.

Welfare Unit provides legal assistance to persons who experience problems with public benefits programs which are administered by the local Departments of Social Services, including welfare (TANF and Safety Net), Medicaid, Food Stamps, Child Care Assistance, HEAP, emergency shelter for the homeless and other emergency assistance programs. We also assist low income households in establishing Supplemental Needs Trusts to eliminate the Medicaid spenddown and assist homeless families to obtain rent supplements from DSS to enable families to leave the shelters or retain permanent housing.

The Re-entry Project provides legal assistance to persons previously incarcerated on civil legal issues arising from their incarceration. The focus of this project is to remove barriers that are preventing their successful reintegration into society.

Social Workers - Social workers have been an integral part of Law Services throughout its history. In general, our social workers help our most vulnerable clients complete tasks necessary for a successful outcome of their legal cases and address social issues that may be impeding the resolution of their legal issues. Our David Project social workers provide family stabilization support services to HIV affected families with minor children.

The Pro Bono Project (Suffolk) The Projects strive to supplement the funded civil legal services provided by Law Services’ staff with the generous volunteer assistance of the private bar. The Project staff refers a variety of legal matters to private volunteer attorneys, usually in the areas of matrimonial, bankruptcy and landlord-tenant law. Legal assistance is also provided in guardianships and with social security disability applications. The Suffolk Pro Bono Project will provide advice-only in child support matters.

Senior Citizen Law Project (Nassau County, only) provides legal representation, advice, and referrals to Nassau County seniors relating to health care, housing, wills, and future planning concerns. The project also handles a wide variety of additional problems, including but not limited to, public benefits, health coverage, advance directives and some housing concerns. Clients must meet financial eligibility guidelines.

The Nassau Civil Unit (Nassau County, only) assists low-income tenants facing eviction, primarily from federally subsidized or rent stabilized housing. It also assists low-income households challenging denials or termination from federal housing programs, including “Section 8”, in administrative hearings and through affirmative litigation in State Supreme Court or in Federal District Court. The Unit works in conjunction with our Volunteer Lawyer Project’s in-court program.

Advanced Bankruptcy Clinic The Advanced Bankruptcy Clinic represents debtors in Chapter 13 Proceedings, in which debtors arrange a multi-year payment plan to pay outstanding debts to avoid liquidation. Students in this clinic also represent defendants in Adversary Proceedings in bankruptcy. These proceedings include matters in which a trustee attempts to recover assets transferred by a debtor and similar matters involving contested issues. Students appear in the U.S. Bankruptcy Court under attorney supervision on behalf of their clients.

Bankruptcy Clinic The Bankruptcy Clinic provides advice and represents clients who are in financial distress. Students gather information from and on behalf of the client and then prepare a Chapter 7 Bankruptcy Petition. Students also participate in U.S. Bankruptcy Court at Section 341 hearings as well as prepare, file and argue numerous bankruptcy motions.

Disaster Relief Clinic The Disaster Relief Clinic provides students an opportunity to learn basic lawyering skills essential to the practice of law while assisting clients who have been impacted by Hurricane Sandy. Under faculty supervision students represent clients in real estate and land use matters, public benefits including FEMA claims, public utilities issues, insurance matters, employment issues, contractor and consumer issues as well as issues that have a broad impact on disaster recovery problems affecting a larger number of clients or public policies for disaster recovery or assistance.

Elder Law Clinic The Elder Law Clinic introduces students to the unique challenges of practicing law on behalf of the elderly, while also providing training in basic law practice skills. As the elderly population has increased, the field of elder law has become a growing specialty, demanding more sophisticated expertise to effectively advise and assist clients. Under the supervision of experienced faculty, students spend 12 to 15 hours per week advising and representing senior clients in a wide range of legal matters in courts, administrative agencies, and negotiation settings. Through their work, students experience law practice in the larger context of social, supportive, and advocacy services for the elderly. Students also develop an appreciation for the role of lawyers and legal institutions in protecting rights and enhancing the quality of life of the aging population.

Family Law Clinic The Family Law Clinic offers students the opportunity to learn family law practice from a variety of perspectives, including litigation, negotiation, and court proceedings. Under the supervision of experienced faculty and practitioners, students represent clients in divorce litigation, child support enforcement, and family offense proceedings.

Through an intensive training covering the dynamics of family violence, the social and psychological dimensions of domestic abuse, the system of shelters and social services available to victims, and the police and criminal court procedures involved in the response to domestic violence. Student also learn special skills needed to interview and counsel clients who have endured these traumas.

Because protecting victims of family violence is a major focus of the clinic’s mission, students spend one day each week in Family Court, where they interview and represent clients seeking court protection. Whether they secure a negotiated agreement or a court order after trial, students experience the satisfaction of knowing that by obtaining an order of protection, excluding the batterer from the home or confiscating a weapon, their efforts have gained the client a measure of safety.

Immigration Law Clinic The Immigration Law Clinic provides students with a variety of practical lawyering skills such as case planning, research and writing, interviewing and counseling, in addition to helping individual clients. While advocating for immigrants, students experience how cultural, economic and political forces influence the treatment of immigrations in our legal system. The clinic is committed to law reform and to advance the cause of social justice.

Mortgage Foreclosure Clinic Students represent clients whose homes are either in mortgage foreclosure proceedings or are about to go into foreclosure. Representation includes assisting clients to renegotiate the mortgage terms to save their homes, representation in mandatory court conferences and in preparing and presenting an defenses available in the foreclosure case.

Veterans’ and Servicemembers’ Rights Clinic This Clinic assists veterans and active duty service members in matters in which military service has created special protections or create unique burdens. Clinic students work closely with the Suffolk County Veterans Court, a specialized part of the Suffolk County court system that helps veterans charged with criminal offenses by diverting them from the traditional criminal justice system and providing them with the tools they need to live productive lives, through treatment and judicial monitoring. Students may also represent veterans in benefits claims, regaining employment after deployment, employment discrimination claims, obtaining accommodations for disabilities and discharge review and update proceedings.

Direct Services for Homeless and At Risk Long Islanders: The Amityville Community Resource Center (CRC) The Long Island Coalition for the Homeless is the lead entity in a collaboration of organizations that will provide housing and services for homeless veterans at the former Air Force Recruitment Center on Albany Avenue in Amityville. Concern for Independent Living, a non-profit housing developer, will create 60 units of permanent affordable housing for homeless veterans at the site. The Coalition will coordinate the provision of case management, housing placement, job training and placement, crisis intervention and other support services.

Community Assistance Program/Centralized Assessment The Long Island Coalition for the Homeless operates the Community Assistance Program. Staff provide information and referrals for housing and services to callers seeking help, and assist eligible participants in applying for housing and services, gathering needed documentation, acting as advocate/liaison between the housing/service provider and client. In 2011, LICH provided information and referral services to over 1,200 callers. In addition, LICH staff assisted approximately 50 homeless individuals and families in completing housing referrals, gathering documentation and submitting applications. In early 2014, LICH hired a Centralized Assessment Social Worker to provide housing referral assistance to homeless Long Islanders with disabilities. The Social Workers coordinated housing placements with DSS and non-profit housing providers that offer affordable, stable, permanent housing with comprehensive support services.

Began by the efforts of a group of philanthropic Vietnam Veterans, one residence was opened in 1994. UVBH is proud to say we now operate and support 24 locations throughout Nassau and Suffolk Counties (23 for housing and 1 for our headquarters) and provide services to over 185 men, women and children on any given day. Primarily, UVBH was formed to work with the homeless veteran population of the Tri State area and continues to do so; we work with veterans who come to us with physical disabilities, Post Traumatic Stress Disorder, Traumatic Brain Injury, mental health issues and/or alcohol addiction. Many years ago we expanded our efforts to include assisting the non-veteran population and the issues they face, including homelessness, drug and/or alcohol addiction and job skill/readiness development.

UVBH places each person or family that comes to us in the home and program that best supports their needs. Along with a safe and structured living environment we provide our residents with a host of supportive services, including Case Management, transportation for medical visits and appointments, education and job placement programs and services as well as coordination of all mental and emotional counseling. We also connect our residents with entitlements.

Pronto of Long Island is a Community Outreach Center serving our most vulnerable children and families living in Brentwood, Bay Shore, Central Islip and our neighboring communities. We have a Food Pantry, Thrift Center and Warehouse with gently used household and furniture items. We also offer programs and services to help our families improve the quality of their lives. Our support is only temporary as our main focus is to help our neediest families rise beyond their challenges to lead fulfilling lives.
