This often involves the frail elderly and adults who are physically or mentally disabled, seriously ill or alcohol/substance abusers. Potential clients have no one able or willing to help them meet their essential needs such as food, clothing, shelter, and for medical care. Any concerned person or agency should make a referral for services using the three numbers provided above. All calls are confidential and may be made anonymously. The cards contain important information such as their identity, personal doctors, next of kin, medical conditions and medications, and who to contact in case of an emergency. The cards are meant to be carried at all times to assist emergency services personnel in providing the best possible care in the event the individual is unable to communicate their needs and other critical information*. * While the cards may help in an emergency, they cannot be used for official identification purposes since they are only as accurate as the information provided by the applicant. The Program staff provides and coordinates legal services and housing counseling services related to HOPP and Super Storm Sandy out of the Central Islip office at the Touro Law School Public Advocacy Center. HOPP works with homeowners to help them understand their finances, connect them to free legal services, and even speak to the bank on their behalf. All this help is available in many languages, and is offered free of charge. 4 | P a g e Catholic Charities - Housing Department Contact: 516-733-7076 www.catholiccharities.cc Catholic Charities' housing department provides seniors who meet income guidelines with the opportunity for safe and affordable housing and access to a supportive network of services. The program currently operates 16 housing facilities in Nassau and Suffolk counties, totaling more than 1,329 units. Catholic Charities' housing department works to ensure the physical, social, and spiritual well-being of residents. Dedicated staff arranges social and educational events at the sites, identifying seniors’ needs, and help them access services for which they are eligible. Apartments are available in Floral Park, Freeport, Inwood, Oceanside, Uniondale, Amityville, Brentwood, Central Islip, Coram, Hampton Bays, Medford, and Selden. Fee: Rents vary by site. For some sites, rent is calculated as a percentage of the resident's income. Other sites have fixed rent. Eligibility: Seniors 62 years and older who meet income guidelines are eligible with the exception of St. Anne's Gardens, which accepts seniors who are 60 or older.
