Comfort Dental Junior Starting Lineup – Rules 2017 COMFORT DENTAL JUNIOR STARTING LINEUP – RULES 2017 Official Rules NO PURCHASE NECESSARY 1. HOW TO ENTER: Fill out an entry form at www.comfortdental.com. All entry forms will be collected by the Comfort Dental corporate office, and Winners will be drawn from all eligible entries. No mechanical reproductions will be accepted. All entries become property of Sponsor and will not be returned. Entries that are, in the sole discretion of the Sponsor, incomplete, illegible or misdirected will not be eligible. 2. PRIZES: Winners receive four (4) ticKets to a Colorado RocKies (“RocKies”) 2017 home game, behind the scenes tour of Coors Field, their name announced and listed on the RocKies scoreboard, a baseball autographed by a Rockies player (selected at the sole discretion of Sponsor), and a personalized RocKies jersey. Estimated retail value of each prize: $500.00 (five hundred dollars). All 2017 RocKies home game prize dates will be selected by Sponsor in its sole discretion. In the event of rain out or cancellation, a re-scheduled 2017 Colorado RocKies home game, if available, will be selected by Sponsor as a replacement prize. Travel and all expenses related to use or acceptance of the prize are Winner’s responsibility. If Winner cannot attend the game designated by Sponsor as the prize, the prize will be forfeited and an alternate Winner will be selected by Sponsor. 3. Eligibility: SweepstaKes are open to all Colorado residents, ages 6-12. The immediate family members and employees of Sponsor, the RocKies, and employees of each of their respective directors, officers, shareholders, partners, parents, subsidiaries, divisions, and affiliates (and each of their respective spouses and immediate family) are not eligible. Winner must be accompanied by his/her parent or legal guardian, and both of these individuals will be counted as Winner’s guests. 4. DRAWING: Winners will be selected from all eligible entries approximately three weeks prior to each respective RocKies game date. Odds of winning will depend on the number of entries received. Sponsor reserves the right to select particular Winners through fundraisers and/or due to special needs. Winner will be notified by mail, email or by telephone. Winner may be required to fill out an Affidavit of Eligibility Form and W-9 form (when applicable), signed by Winner’s parent or legal guardian. Noncompliance with the Affidavit of Eligibility requirements may result in disqualification and an alternate winner may be drawn. Prize notification returned as undeliverable will result in award to an alternate Winner. No prize substitution, cash equivalent or transfer of prize by Winner in violation of these rules and regulations is permitted. Sponsor reserves the right, in its sole discretion, to substitute a prize of equal or greater value. Void where prohibited by law. SweepstaKes are subject to all applicable federal, state, and local laws, rules and regulations. All federal/state/local taxes are Winner’s sole responsibility. Prize acceptance constitutes acceptance of prize conditions and permission to Sponsor and Rockies to use Winner’s name, address (city and state of residence), photograph, voice, statements and/or other likeness and prize information in perpetuity for purposes of advertising and trade in any and all media now Known or hereafter devised, throughout the world, without further compensation, or consideration, notification or permission, unless prohibited by law. 5. Limitation of Liability: Sponsor is not responsible for printing or typographical errors in any sweepstaKes- related materials, stolen, lost, late, misdirected, damaged, incomplete, or illegible entries, and/or transactions that are processed late or incorrectly or are lost due to computer or electronic malfunction. Sponsor reserves the right to cancel or modify the sweepstakes if fraud or technical difficulties compromise the integrity of the sweepstakes as determined by Sponsor, in its sole discretion. Winners’ parents or legal guardians agree (1) to release and hold harmless, discharge, and covenant not to sue Sponsor, the Colorado RocKies Baseball Club, Ltd., Denver Metropolitan Major League Baseball Stadium District, and each of their respective agents, directors, officers, shareholders, partners, insurers, servants, parents, employees, subsidiaries, divisions, affiliates, predecessors, successors, and representatives (collectively, the “Released Parties”) from and against any and all claims, demands, losses and liabilities arising out of participation in the sweepstakes or receipt, redemption or use of the prizes awarded, and (2) that the Released Parties have neither made nor are in any manner responsible or liable for any warranty, representation or guarantee, expressed or implied, in fact or in law relative to any prize. As a condition of participating in this SweepstaKes, each entrant agrees that any and all disputes, claims and causes of action arising out of or connected with this Sweepstakes or the prizes awarded shall be governed by the laws of the State of Colorado without giving effect to any choice of law or conflict of law rules, and that any and all disputes that cannot be resolved between the parties and causes of action arising out of or connected with this Sweepstakes shall be resolved individually, without resort to any form of class action, exclusively before a court located in the City & County of Denver, CO. The Cardinals went 14-1 in 2019, en route to their first national title, as they defeated UW-Whitewater, 41-14. The Cardinals did not play in 2020 due to the COVID-19 pandemic. Oddly enough, all four North Central selections were part of the College Football America Yearbook’s 2020 Starting Lineup — running back Ethan Greenfield, wide receiver Andrew Kamienski, offensive lineman Sharmore Clarke and defensive back Jake Beesley. Many of the members of this year’s Starting Lineup are tapping into the extra year of eligibility provided to all players due to COVID-19. Other players that made the Starting Lineup include Muhlenberg quarterback Michael Hnatkowsky, Saint John’s wide receiver Ravi Alston, Hardin-Simmons offensive lineman Boomer Warren, Delaware Valley defensive lineman Michael Nobile, Rose-Hulman linebacker Michael Stevens and Mary Hardin- Baylor defensive back Jefferson Fritz. Saint Francis (IN) has two selections, running back Justin Green and linebacker Piercen Harnish. Both were a part of CFA’s preseason Starting Lineup in 2017. Green rushed for 2,031 yards and 29 touchdowns in 2017 and is already the Cougars’ leading career rusher. Harnish had 125 tackles, with 17.5 for loss and 3.5 sacks. Morningside, which reached the NAIA semifinals and lost to Saint Francis (IN), 43-36, has three players in the Starting Lineup — quarterback Trent Solsma, wide receiver Connor Niles and punter Spencer Wyant. Solsma threw for 4,406 yards and 55 touchdowns in 2017 as the Mustangs led the nation in total offense per game (575.6 ypg). Niles returns after catching 96 passes for 1,449 yards and 17 touchdowns last year. Wyant averaged 42.8 yards per punt in 2017, with a long of 66 yards. Joe Cannavino had a relatively brief football career, and was never exactly a household name. Still, he deserves plenty of credit for one quality: he knew how to exit the stage. Cannavino had his best moments in his final game as both a college and pro player. Joseph Patrick Cannavino was born on January 20, 1935 in Cleveland, Ohio. He stayed in that city throughout his childhood, attending Collinwood High School on Cleveland’s East Side. Joe is one of eight people from Collinwood as of 2020 who reached the NFL. The latest of them was Cecil Shorts, who played for three teams in the 2010s. Cannavino was one of those three-sport standouts that were relatively common in those days. He piled up eight letters in high school, and also played for the basketball and baseball teams. However, Joe did his best work as a Railroader for the football team. Cannavino played both ways (defensive back and running back), and was an all- conference pick. Exception: First names may be omitted from the written lineup card as long as they are printed on the card’s roster. First, last names and uniform numbers of all eligible substitutes. First and last name of the head coach, or co-head coaches. Each team must submit a complete lineup card to the plate umpire at the pregame meeting at home plate. Confirm that the lineup card has players first, last names and positions listed along with all substitutes. Make sure there are no duplicate numbers. Make sure there are at least 9 or10 players listed in the starting position. If the DP/FLEX is used, verify with the coach the player listed as the DP is the one who will hit for the FLEX in the 10th spot. DP : Plays Offense FLEX : Plays Defense Bats for the FLEX Can play defense for any player. Run for the DP. Hit for the DP. She can only run for the FLEX. She can only bat for the FLEX. She may play defense for any player. Players not listed on the lineup card are not eligible to play. All players listed in the starting lineup must be in uniform, in the dugout area and are available to play at the start of the game. The umpire may ask for the lineup card to be corrected before accepting it as official. As a preventative measure to avoid possible re-entry eligibility infractions, prior to the game, circle all starters that are listed on the bottom of the line up card.
