This appointment is further subject to the following conditions. 1. On appointment they are required to report to the Civil Surgeon of the district of thei r choice, subject to the availability of the vacant post in that district. This appointment is further subject to the condition that in case if their certificate/ documents are fo und to be bogus, their candidature will be cancelled automatically without any notice/ intimation. 2. Th ey will be entitled to draw such allowances and oth er benefits as sancti oned by the Gove rnment of Haryana from tim e to time. 3. The post is temporary and their appointment will be terminable with one month noti ce on either side while they are in the temporary cad re. This co ndition however, wi ll not be applicable in case their servi ces are di spensed with during th e proba ti on period. 4. They will be on probation for a period of two yea rs from the date of joining whi ch ca n be extended, if necessary up to three years. Th e services put in against the temporary post might be counted towards the r eri od of probation but the co mpletion of two years temporary services will not in itself entitle them to co nfirmati on unless the post is substantively vacant. Their appointment during probation period can be terminated without assigning any reasons or giving any notice. 5. If so required, they shall be liable to serve in any Defen ce Service or post co nnected with th e Defen ce of India for a period not exceeding fo ur years includ ing the period spent on training if any, except:- (a) After th e expiry of ten yea rs fro m th e date of appointment and (b) After attaining the age of forty five years. 6. They will be governed by the Haryana Civi l Medical Services (Group A) Service Rules, 2014 as amended from time to time. In respect of pay, pension, leaves and all othe r matters not ex pressly provided for in th e Health Department Harya na Rules, they sha ll be governed by such regulations and rules as have been/ would be framed or may hereafter be adopted by the Competent Authority under the Constitution of India. 7. On appointment/joining they will be required to take an oath of allegiance to the Constitution of India. 8. They will be required to submit an undertaking alongwith the joining report • A declaration attested by Magistrate First class to the effect that you have only one living spouse and are not married to a person already having one livi ng husband/wife. • A declaration that you were not dismissed from service on any occasion under any department of Govt. or convicted by a court of law. • In case, you are married, you will have to file a declaration that you did not give/accept dowry during the marriage. If you are unmarried, you will have to furnish a declaration immediately after marriage regarding non acceptance/ giving of dowry by you as per declaration in Annexure A and B in terms of Government instructions issued vide No. 18/1/2004-2GSI dated 21.02.2006. 9. Their seniority will be determined according to their position in the merit list sent by the Departmental High Powered Selection Committee. 10. They are liable to be transferred anywhere within or outside the State of Haryana. 11. In case they have already appeared before the Medical Board in Haryana during the last 06 months and are declared medically fit, they need not to appear for the same. In ca se they are already employed somewhere on a gazette post under the Haryana Government and they have already produced a medical certificate to the department, they may be exempted from producing fresh medical certificate provided there is no break in the service and they have produced the certificate from their employer. 12. They are required to furnish a Medical Certificate of fitness before joi ning from the Medi ca l Board constituted under Rule 9 of Haryana Ci vil Se rvices (Ge neral) Rul es, 2016. They should appear for medical examination in the office of any Civil Surgeon of Haryana State. 13. The appointment is provisional and is subject to the academic qualification certificate/ Schedule Castes/Backward Classes/ ES M/ DESM/ EWS/ DFF certificate and any other document/ declaration being verified through proper channe l. If the verification reveals that the claims belonging to academic qualification/other documents as the case may be, is false, their services will be terminated forthwith without assigning any further notice and without prejudice to such further action as may be taken under the provisions of the Indian Penal code fo r produ ction of false documents. M. This offer of appoi ntment is further subj ect lo co nd iti on of verification of character and antecedents. In case it is found that th e appointee is nol of good character and anything is adverse agai nst him /her, hi s/her servi ces will be dispensed with at once without any notice to him/ her in this regard. 15. If they are willing to accept this offer of appointment on the above mentioned terms I and conditions, they should report to the concerned Civil Surgeon for joini ng at their respective posting stations within 15 days; no ex tension in joining time wou ld he allowed. If they fail to report for duty within the stipulated period, it will be presumed that th ey are not willing to accept this offer of nppoi ntment and their candidature will be trented as ca ncell ed. Unfill ed vacancies wou ld be filled from the waiting list.
