Upon your enrollment in the MGM Resorts International Self‐Limit Program, the following guidelines will apply when you visit any MGM Resorts International property:
You are permitted to: • Patronize non‐gaming amenities at the property, including but not limited to staying in the hotels, dining at restaurants, etc. • Play cash in the casino
You are restricted from: Marker privileges Front Money deposits Receiving complimentaries or any other type of incentive Earning points, rewards, tier credits and other benefits Check cashing privileges Credit card cash advances Use of safety deposit boxes Receiving gaming‐related direct mail marketing, telemarketing or promotional mail Receiving gaming‐related customer recognition programs or offers
Your self‐limit status will remain in effect for a minimum of ONE YEAR from the date of enrollment.

SELF-LIMIT PROGRAM ENROLLMENT FORM
MGM Resorts International (“MGM Resorts”) is committed to promoting public awareness and education and to providing information on available resources to those patrons who believe they may have a gambling problem. MGM Resorts maintains a program in compliance with Nevada Gaming Regulation 5.170 that allows our patrons to voluntarily self-limit themselves from certain activities and privileges, which are provided as a service and convenience to our patrons by completing this form. This self-limitation will be effective within 14 days of this request being received by MGM Resorts.
PATRON’S REQUEST TO SELF-LIMIT 1. I, the undersigned patron, hereby request to self-limit my access to casino and casino related privileges, including receiving complimentaries. I agree that as a result of being self-limited I no longer will be permitted to have check cashing privileges, marker privileges, front money deposit privileges, cash advances processed at the casino and/or poker cages, use of safety deposit boxes at the casino and/or poker cages, as well as not being permitted to receive (i) gaming related direct mail marketing; (ii) gaming related telemarketing; (iii) gaming related promotional mail; and (iv) gaming and gaming related customer recognition programs at any of the following MGM Resorts properties: • Nevada: ARIA, Bellagio, Excalibur, • Mississippi: Beau Rivage (Biloxi), Gold Luxor, Mandalay Bay, MGM Grand, Strike (Tunica) The Mirage, Park MGM, and New York • New Jersey: Borgata (Atlantic City) New York. • New York: Empire City Casino (Yonkers) • Maryland: MGM National Harbor • Ohio: MGM Northfield Park • Massachusetts: MGM Springfield • and any future casinos operated by MGM • Michigan: MGM Grand Detroit Resorts in the United States. I understand that the states of Maryland, Massachusetts Michigan, Mississippi, New Jersey, New York, and Ohio have (and future states in which MGM Resorts may operate a casino in the future may have) their own laws governing self- limit, self-exclusion or disassociated patrons. Those states’ laws require that persons wishing to avail themselves of the protection afforded under those laws to complete their state’s forms. However, by agreeing to be self-limited in Nevada, you agree that MGM Resorts will limit the casino and gaming related privileges available to you at its properties in Maryland, Massachusetts, Michigan, Mississippi, New Jersey, New York, Ohio, and any property in the United States which MGM Resorts may operate in the future.
2. I understand that I will not be permitted to participate in online or mobile gaming using any of the following applications or websites: BETMGM.COM, BORGATACASINO.COM, BORGATAPOKER.COM, and any other online gaming application or website that BetMGM1 may operate in the future (collectively referred to as “Online Gambling Applications”).
3. I understand and agree that no further points, rewards, tier credits or any other benefits may be earned from any customer recognition programs in which I have participated. I further understand and agree that by requesting to be self-limited any points, rewards, tier credits or any other benefits I previously have earned from any MGM Resorts customer recognition programs or through an Online Gambling Application will be forfeited, included but not limited to any Express Comps or tier credits. I also understand and agree that MGM Resorts shall continue to honor my request to self-limit my access to the gaming and gaming related privileges described above unless and until I request to rescind the voluntary self-limitation. I understand and agree that my request to rescind must be in a notarized writing; in a form approved by MGM Resorts and that I shall not be allowed to make such a reinstatement request earlier than one year after the date I became self-limited. Notwithstanding any recission, the states of Maryland, Massachusetts, Michigan, Mississippi, New Jersey, New York and Ohio, as well as jurisdictions in which the Online Gambling Applications operate, may have different rules regarding reinstatement which MGM Resorts will honor. MGM Resorts maintains the right to deny any reinstatement request, for any reason.
4. I understand that upon being self-limited, I will be ineligible to enroll in the M life Rewards Program or the Credit Cards Rewards Program (M life Mastercard). If I am already enrolled in the M life Rewards Program or the Credit Card Rewards Program, my benefits cease, all accrued benefits are forfeited as specified in Paragraph 2, above and I am not longer able to participate in such programs.

5. I have been advised and agree that if I receive check cashing/marker privileges or gaming related (i) direct mail marketing, or (ii) telemarketing information, or (iii) promotional materials after the effective date of self-limitation, I will contact the MGM Resorts Corporate Compliance Department at [email protected] within ten (10) days of receipt of such information or materials. If I have questions concerning this Policy, I will contact the MGM Resorts Corporate Compliance Department at [email protected]. I acknowledge that for my request of self- limitation to be truly effective, I must exercise self-restraint and I should not ask any MGM Resorts employee to provide me with any of the services or privileges, which are the subject of this request. I agree this self-limitation request does not release me from any debts incurred prior to, or after, the effective date of this self-limitation request. I hereby release and hold MGM Resorts, its subsidiaries and affiliates, harmless from any claim by me or any third party arising from my presence, acts or omissions upon the premises of any MGM Resorts properties which occur after the effective date of self-limitation. I further agree that this self-limitation request does not obligate MGM Resorts properties to return any funds wagered by me prior to, or after, the effective date of this self-limitation request.
