Medical History, 1995, 39: 288-316 Pancreatic Organotherapy for Diabetes, 1889-1921 ROBERT TATTERSALL* Introduction The isolation of insulin by Banting and Best in 1921 and its rapid commercial exploitation revolutionized the treatment of diabetes. The story, frequently told as a romance of medicine, is that a young orthopaedic surgeon, Frederick Banting, went to the local Professor of Physiology, J J R Macleod, asking for facilities to try to isolate the internal secretion of the pancreas. Macleod was sceptical but provided a lab, dogs and a student assistant to measure blood sugar. After only three months, Banting and Charles Best isolated a potent anti-diabetic substance which was first used on a human diabetic in January 1922. Controversy followed when Banting and Macleod, not Banting and Best, were awarded the 1923 Nobel Prize. Much has been written about the events in Toronto in 1921-1922l as well as about the priority disputes which followed.2 Two editorials in the Lancet in 1923 questioned why this discovery had taken so long.3 The seductively simple critique ran as follows: in 1869, Paul Langerhans described his eponymous islands, in 1889 Oskar Minkowski produced severe diabetes in dogs by pancreatectomy, in 1891 George Murray cured myxoedema by injections of thyroid extract and in 1893 Gustave-Edouard Laguesse suggested that the islands of Langerhans produced "an endocrine secretion" which controlled carbohydrate metabolism. How could it have taken thirty years for therapeutics to exploit the physiological suggestion? J. H. HOVELL The extra-oral approach to the ascending ramus is difficult owing to the proximity of the facial nerve. The intra-oral approach is, however, quite simple and fixation by sigmoid notch wiring straightforward. It was not until I was well advanced with my assessment of these cases that my views on open ascending ramus slides became modified. I have not yet done a sufficient number of these cases to form other than tentative conclusions, but I feel that this lecture would not be complete without at least this mention of my most recent work. In conclusion I must again thank Mr. C. Bowdler Henry for allowing me access to his cases and Mr. Richard Battle for his helpful advice and encouragement. Also I owe much to personal communications from many of my colleagues, especially Mr. Fickling, Mr. Fordyce and Mr. Rowe. The series was founded by the late Professor John Pickstone, and its ambitions refect his commitment to the integrated study of medicine, science and tech- nology in their contexts. He repeatedly commented that it was a pity that the foundation discipline of the feld, for which he popularized the acronym ‘HSTM’ (History of Science, Technology and Medicine) had been the history of science rather than the history of medicine. His point was that historians of science had too often focused just on scientifc ideas and institutions, while historians of medicine always had to consider the understanding, management and meanings of diseases in their socio-economic, cultural, technological and political contexts. In the event, most of the books in the series dealt with medicine and the biomedical sciences, and the changed series title refects this. However, as the new editors we share Professor Pickstone’s enthusiasm for the inte- grated study of medicine, science and technology, encouraging studies on biomedical science, translational medicine, clinical practice, disease histories, medical technologies, medical specialisms and health policies. The books in this series will present medicine and biomedical science as crucial features of modern culture, analysing their economic, social and political aspects, while not neglecting their expert content and con- text. -

Clinical trials in British medicine 1858 - 1948, with special reference to the development of the randomised controlled trial Benjamin Toth A dissertation submitted to the University of Bristol in accordance with the requirements of the degree of Doctor of Philosophy in the Faculty of Medicine. University of Bristol Department of Social Medicine November 1998 Word count 79,542 1 Abstract This thesis arose from a desire to explore the reasons for two related phenomena: why randomisation wasn’t introduced into clinical trials before the 1940s, and why in 1947 it was. In seeking to answer these questions, this study focuses on the development of clinical trials in Britain from the mid-nineteenth century to the MRC streptomycin trial published in 1948. Control groups and quasi-random allocation and were known to British clinicians in the nineteenth century, well before they became formalised in statistical theory. Chapter two argues that the collective therapeutic enquiries of the 1860s were an attempt to reform the medical profession in the light of the deficiencies of the 1858 Medical Act. Reform in this context was an attempt to create an ‘ideal practitioner’, defined here as one who used a statistical style of knowledge to guide both medical practice and medical etiquette. However, methodological elements such as control groups were largely irrelevant to this enterprise. Collective enquiries were overshadowed by the possibility of an exact knowledge of therapeutic action. Drugs such as diphtheria anti-toxin and Salvarsan, both products of German laboratories, mark the beginning of the modern era of therapeutics. Clinical trials played a secondary role in the development and testing of such drugs. -

We have seen a case of "diabetic non-ketotic hyperosmolar coma" with ketosis. An 84-year-old man was brought into the hospital in a deeply comatous and dehydrated state. The initial blood glucose level was 1252mg/dl with plasma osmolarity of 435 mOsm/l, but no ketonuria was detected by the nitroprusside method (Ketostix). However, the plasma 3-hydroxybutyrate (3-OHBA) level was 5 mM in a newly developed bedside film test. The serum ketone bodies were later found to be 5.56 and 0.82 mmol/l for 3-OHBA and acetoacetate (AcAc), respectively. A marked increase in glucagon, cortisol and ADH with renal dysfunction (creatinine 5.0 mg/dl) were noted. An abnormal electrocardiogram, occular convergence and chorea like movement disappeared after correction of metabolic disturbances. The moderate level of IRI (14 p J/ml) on admission and a good response to glucagon 2 months after admission also indicate that the present case is a typical hyperosmolar non-ketotic coma. Because of a preferential increase in 3-OHBA, ketonuria seemed to be absent in the regular nitroprusside test. Marked dehydration is thought to cause renal dysfunction, and the increase in ADH may have helped to prevent further aggravation of ketoacidosis. -

The spinal cord cannot be repaired. Treatment consists of preventing complications until the spine has stabilised and the patient can be rehabilitated to an independent life. Surgeons have concentrated upon carrying out an operation on the spine. There has been no improvement in treatment until the beginning of the 20th century. The development of treatment in the Ancient World and the Middle Ages until Paré is explored. After Paré medical traditions separated. In the 19*'^ century the controversies over surgery in the United Kingdom between Cooper and Bell are described. The First World War led to the setting up of the first spinal unit in the United Kingdom with outstanding work by Head, Riddoch and Holmes.
