This is an exciting time to enter the field of medicine, with our nation’s healthcare landscape changing every day. The excitement is palpable at the Department of Medicine, where we ar e playing an active role in the transformation of medicine: how we teach it and study it; how we develop new insights and ideas about it; and how we practice it as we heal , families, and communities.

The Department of Medicine is thriving in this time of transformation. Residents are exposed to a remarkable variety of clinical environments and conditions, with populations in every socioeconomic situation. Thomas G. We are building an innovative research enterprise that focuses on healthcare delivery and outcomes, and on moving scientific discoveries and insights from the laboratory through the research “pipeline” into real-world interventions. Many of our faculty conduct cutting-edge research at The Feinstein Institutes for Medical Research, including the new Center for Health Innovations & Outcomes Research (CHIOR). CHIOR puts to work technology, big data, and creative new ways of thinking about healthcare outcomes to measure and improve healthcare delivery today. We have forged an innovative partnership with the Cold Spring Harbor Laboratory in order to expedite investigation into cancer therapeutics. We also benefit from a close partnership with the Donald and Barbara Zucker School of Medicine at Hofstra/Northwell, which is crafting a new paradigm in medical education that blends medicine’s finest traditions and values with state-of-the-art technology in biomedical science.

Thank you for your interest in the Residency Program in Internal Medicine at the Donald & Barbara Zucker School of Medicine at Hofstra/Northwell. Our 130 residents are the Department’s heart and soul, along with our dedicated full-time faculty of more than 300, all collaborating to develop the next generation of healers, thinkers, leaders, and educators.

Northwell’s program is among the largest in the nation, graduating about 60 trainees a year and matching another 60, the second largest Match in State. Both of the main training sites are set in a beautiful suburban community just 30 minutes from , serving a unique patient population drawn from a service area of almost 8 Karen A. Opportunities for research are plentiful, with many faculty welcoming and supporting trainees at the world-class Feinstein Institutes for Medical Research right on campus.

The residency program is deeply committed to evidence-based strategies for physician education, whether classic or cutting-edge. Most didactics are case-based and set in small groups. Team-based learning is the norm in the ambulatory setting. We have also embraced the track, which allows residents to customize their learning. Our tracks include research, the clinician-educator, global health, healthcare reform and advocacy, a community health track, and IMPACcT (IMproving Patient Access, Care, & cost through Training), an interprofessional primary care training program.

Our program consistently and creatively prioritizes resident well-being and work-life balance, starting with its innovative Continuity Model of house staff education. In a variation on “protected time”, each four-week block of inpatient duty is followed by a week dedicated to ambulatory education -- which starts with a weekend off. Residents are thus guaranteed a predictable schedule over time and the comfort of sleeping in their own beds for at least one out of five weeks. This strategy eliminates pressure on residents to virtually be in two places at once; resident life assumes a consistent rhythm and “sleep debit” is minimized. The system enhances not only resident well-being but also clinical care, as it reduces the fragmentation and waste that accompany the traditional system with its constant interruptions.

It’s impossible to capture the Northwell experience in this limited space. The quality of the teaching, the beautiful setting, the warmth of bright, supportive peers, the commitment of staff and administration, including the support that frees up our trainees to focus their time and energy on learning…these don’t come through in the facts and figures but they’re major factors in our success. Call Schedules There are two types of inpatient floor teams: Low teams (1-4) and Care Model teams (A&B). The Low teams are on call at both sites every four days and take admissions until 7pm. On call and post-call days, Low teams may receive “holdovers,” referring to patients who are admitted overnight and signed out to the housestaff team in the morning. Each intern caps at 8 patients. The Care Model teams receive new patients daily via a drip system as they are transferred to the Care Model floors (5 Monti at NS and 9S at LIJ). Some of these patients will be admitted by the Day-Admitting Resident (PGY-2) prior to arrival to the Care Model floor. Each intern caps 8-9 patients. Elective, Ambulatory, Geriatrics, Neurology, Palliative Care and Private Practice Rotations have no call; residents work Mondays through Fridays on those rotations.

Conference Courses A combination of didactic conferences and interactive small group teaching sessions are offered throughout the academic year. These include daily noon conferences, morning reports (3 times per week), intern reports (2 times per week), Grand Rounds, Professor Rounds with the department chairman, outpatient team based learning, physical diagnosis courses and simulation sessions. FAQ

Do I have to go into primary care if I participate?

No, you do not have to enter primary care if you participate. This will be an experience that we hope increases your interest in primary care and ambulatory medicine (particularly by working with a very dedicated and enthusiastic faculty). How will my role be different if I participate in IMPACcT?

In addition to continuing to see ambulatory patients for acute, chronic care and well visits, you will be trained in leading an interprofessional team (pharmacy, physician assistant, behavioral health team). Care delivery and other clinical responsibilities (i.e. task management) will be shared amongst this team.

Are there opportunities for additional clinical time if I participate in IMPACcT?

We are hoping to include additional clinic time in the 3rd year and are working with Residency Leadership to accomplish this.

How is the educational experience going to be different than my current continuity experience?

You will continue to participate in Evidence-based Medicine, TBL, and the Thursday morning ambulatory education sessions. In addition, there will be short case-based didactic sessions with a clinical huddle 2-3 times a week while you’re in clinic, and IMPACcT Workshop Sessions (Thursday afternoons, 5 times a year) which bring together all of the IMPACcT learners and faculty to focus on specific topics (Interprofessional Skills, Health Disparities, Medication Management, Behavioral Health, and PCMH Principles).

What about my current continuity patient panel? Will I lose those patients if I enter IMPACcT?

As your IMPACcT experience will take the place of your usual continuity clinic sessions, it is our goal that your established patients will follow right along with you. This will be a bigger switch for the patients if you currently practice at the ACU, but we will try to facilitate this transition.

How many residents will there be?

There will be 5 R2 and 5 R3 residents in the program. Each firm will have one R2 and one R3.

The unit was high acuity, and we received a transfer from Forest Hills Hospital. Initially, we thought this patient was having an acute MI. There were EKG changes suggestive of ischemia, and he had chest pain 10/10. The interventional cardiologist was concerned and came from home to further evaluate the patient. “This morning on Interdisciplinary Rounds, my team witnessed an interaction between a Resident and patient that we feel needs to be celebrated and acknowledged. Dr. Eugene Lucas was seeing his patient on 6 Monti. This 78 year old woman is receiving comfort feeds, is a DNR/DNI, and is frail and afraid. The team had some questions about the overall plan, and noticed the Resident enter the room. When I walked towards the door, I saw him feeding the patient some applesauce, and smiling while he spoke with her. When he came out, he was happy to discuss the patient with our team. He circled back to ask me if we could arrange for her to borrow a phone so that she could listen to music. He said that she liked classical music, gave me the name of some of her favorite composers, and stated that she would probably like headphones, as she is very courteous of her roommate.”

Global Health Elective: Uganda

Hope the craziness of July is winding down and things are starting to get a little easier back at home. Wanted to update you guys and everyone else on the Uganda trip. It is just as amazing as Sean, Rahmat, Joe, Krystal and Safiya made it sound last year. I've seen some incredible cases, especially ID and cardiac, that you would never see back in Long Island. We had to do lots of reading on Brucella, typhoid, and rheumatic heart disease (been a long time since I had heard of Sydenham's chorea) the past few weeks. The most surprising thing is how much chronic disease is here. There are a sizable chunk of patients admitted for heart failure and COPD exacerbations that I didn't expect. But for every one of those patients, there is an HIV case with a weird presentation of TB. It's a lot of work, about 12 hours a day, 6 days a week, but there isn't any pressure for following up consults or the stress of trying to save H+Ps five times unsuccessfully.

The town is incredible, with amazing scenery and pretty good food. I was surprised that my first dinner here was actually a cheeseburger with fries. We've climbed up most of the hills around the area, been to a couple of lakes, and done a few motorcycle rides.

I'll bring back lots of pictures and more stories. Hopefully we can do a noon conference after Mo gets back in September. All the best to everyone back home.

My time here has been remarkably rewarding and is exceeding all of my expectations. The country and the culture are beautiful. The smiles are innumerable. The benevolence is ubiquitous. The patients immediately inform you of their gratitude and appreciation for the slightest minutia of medical attention.

The pathology seen here is unparalleled to anything in the developed world. Naming only a small sample of diseases encountered typhoid fever, pemphigous vulgaris, I drained an abscess on a 2week old infant, molar pregnancy, a 12 year old with heart failure and a right ventricular heave, and of course malaria.

It is an injustice to the elective to limit its summation to a few brief paragraphs, and especially since I have much more time in Kisoro. If you are thinking of doing this elective, don’t think, do it and accept the challenge. Isn’t your appetite for challenges, why you chose internal medicine in the first place?

Fun updates: Yesterday I climbed a volcano. The day before, my co-resident and I saved a man who got machete through the skull because he cut the water line near the border of Congo. We stuck about 4 IVs in him and fluid resuscitated him about 6L in about 1 hr. From a half alive guy keeled over on the floor, soaked in blood, he bounced back to a guy w tight head bandage bathing himself.

Public Health Elective – New York ACP

My time at the NY Chapter of the American College of Physicians was enlightening and extremely rewarding. I started this elective rotation expecting to learn more about healthcare legislation, but by the end the month, I had gained a much greater understanding of the true intricacies of creating a substantial and lasting piece of law. As I reviewed the many bills being proposed in Albany, I realized that the legislature was considering measures every day that could drastically change the way medicine is practiced. If we as physicians don’t take it upon ourselves to learn about current legislation and to make our voices heard, the entire landscape of the healthcare system could change without the advice of the people who actually work in it every day.

This experience changed the way that I view healthcare. During residency, we are so focused on the task at hand or the single patient in front of us that we often fail to see the bigger picture and the medical system as a whole. During my month in Albany, I was able to step away from my individual patients and consider healthcare from a population standpoint; I was able to see how making changes in legislation can actually affect the health and wellbeing of millions of patients at a time. I was also able to see firsthand how my medical training and knowledge could be useful to legislators, most of whom cannot name medicine as their primary area of expertise. This experience showed me how a single person can inspire change in a society, and I feel extremely fortunate to have been allowed this opportunity during my medical training. -Claire Popplewell, MD
