3. ADDENDA ISSUED: (Addenda No. 1 issued 3/20/2017, Addenda No. 2 to be issued 3/23/17).

No oral interpretation of the meaning of the plans, specifications or other bid documents will be made to any bidder. Every request for such interpretation should be in writing addressed to City of Mesquite Purchasing at above address or [email protected] and WRA Architects Inc. and to be given consideration, must be received by March 30, 2017. Any and all such interpretations and any supplemental instructions will be in the form of written addenda to the contract documents which, if issued, will be via city website not later than one day prior to the date fixed for the opening of bids. Failure of any bidder to receive any such addendum or interpretation shall not relieve such bidder from any obligation under this bid as submitted. All addenda so issued shall become part of the contract documents.

4. RECEIPT AND OPENING OF SEALED BIDS:

The Owner invites Competitive Sealed Proposals (CSP) on the forms prepared by the Owner; a copy of the forms is included in the specifications, all blanks of which must be appropriately filled in. Competitive Sealed Proposals will be received by the Owner at the time and place indicated in the "Advertisement for Bids” and then at said place will be opened, read publicly and then evaluated.

The Owner may consider informal any bid not prepared and submitted in accordance with the provisions hereof and may waive any informalities or reject any and all bids. Any bidder may be withdrawn prior to the above scheduled time for the opening of bids or authorized postponement thereof. Any bid received after the time and date specified shall not be considered. No bidder may withdraw a bid until forty five (45) days after the actual opening thereof.

All Bids must be submitted on forms prepared and furnished in the specifications (in duplicate - one to be submitted with the CSP and one to be retained by the Bidder for his records). Only the original shall be signed.

Each CSP must be submitted in a sealed envelope bearing on the outside the name of the Bidder, address, and the name of the project for which the Bid is submitted. If forwarded by mail, the sealed envelopes containing the CSP must be enclosed in another envelope addressed as specified in the Bid Form.

5. METHOD OF BIDDING:

The Owner will receive Competitive Sealed Proposals on a Lump Sum basis which shall be comprised of all divisions of the work as noted in drawings and specifications. The CSPs shall also include all information requested in the special provisions.

6. TELEGRAPHIC MODIFICATION:

The Owner will not accept telegraphic modification of Bid after receipt by the Owner of any CSP submitted.

7. QUALIFICATIONS OF BIDDER:

The Contractor must provide the Qualification Information requested in the Bid Form Information included in the specifications. This information must be turned in with the Bid Form.

8. SUBCONTRACTS:

The bidder is specifically advised that any person, firm or other party to whom it is proposed to award a subcontract under this contract must be acceptable to the Owner.

Before using the bid of a sub-contractor, the Bidder shall investigate and satisfy himself that the sub- contractor he will propose to the Owner can satisfy all of the requirements above expressed. The Owner reserves the right to reject any bid if the evidence submitted by, or investigation of, such bidder fails to satisfy the Owner that the bidder and/or any sub-contractor he proposes can properly qualify to carry out the work contemplated therein.

9. BID SECURITY:

All bidders must submit with bid either a Bid Bond on the form provided herein, a cashier’s or certified check in the amount of five percent (5%) of the total bid (including all alternates).

10. LIQUIDATED DAMAGES FOR FAILURE TO ENTER INTO CONTRACT:

If a Contractor fails to enter into a contract 10 days after notice of award, the Owner may cash the Contractor’s Bid Security as liquidated damages for failure to enter into contract.

11. TIME OF COMPLETION:

AN UPDATED SCHEDULE WILL BE ISSUED IN ADDENDA NO. 2

12. CONDITIONS OF WORK:

Each bidder must inform himself fully of the conditions relating to the construction of the project and the employment of labor thereon. Failure to do so will not relieve a successful bidder of his obligation to furnish all materials and labor necessary to carry out the provisions of his contract.

Each bidder should visit the project site and be thoroughly familiar with all existing conditions.

13. NOTICE OF SPECIAL CONDITIONS:

Attention is particularly called to those parts of the contract documents and specifications, which deal with the following: a. Insurance Requirements. b. Contract Form. c. General Clauses and Conditions. d. All Bid Forms requested. e. Standards of Conduct f. Inspections and Testing of Materials (Section – 01400 – Quality Requirements) g. Alternate Bids – (Section – 01 23 00) – To be discussed in detail.

14. LAWS AND REGULATIONS:

The bidder's attention is directed to the fact that all applicable Federal Laws, State Laws, Municipal Ordinances, and rules and regulations of all authorities having jurisdiction over construction of the project shall apply to the contract throughout, and they will be deemed to be included in the contract.

15. METHOD OF AWARD – COMPETITIVE SEALED PROPOSALS - “BEST VALUE”:

The contract will be awarded to the responsible bidder submitting the CSP “best value” complying with the conditions of the Invitation for Bids, provided it is in the interest of the Owner to accept it. Each sealed bid will be evaluated per the specified evaluation criteria and ranked. The award of the contract may include full consideration of completion time, and Owner bid alternates. The bidder to whom the award is made will be notified at the earliest practicable date. The Owner, however, reserves the right to reject any and all bids and to waive any informality in the bids received whenever such rejection or waiver is in the interest of the Owner.

The Owner also reserves the right to reject the bid of any bidder who has previously failed to perform properly, or to complete on time, contracts of a similar nature; who is not in a position to perform the contract; or who has habitually and without just cause neglected the payment of bills or otherwise disregarded his obligations to sub-contractors, material men, or employees.

16. SAFETY CONDITIONS:

Contractor will be responsible for all safety programs at the construction site.

17. SUPERVISION:

Full time foreman at all times when work is being carried on. (Must be qualified.)

18. PRE-CONSTRUCTION CONFERENCE:

Prior to issuance of Notice to Proceed.

19. ADDITIONAL INFORMATION:

a. The Owner, City of Mesquite, is exempt from State or Sales Tax. b. City Trade registration fees will be paid by each individual subcontractors. c. Use bid forms provided in the specifications. Fill in all blanks. Provide signature by responsible party. d. Construction Staging and Storage of material is limited and shall be reviewed and approved by the Owner. e. Calendar Days for Bid.

20. GENERAL PROJECT SCOPE, LOCATION & PHASING

1. Existing Adjacent Elementary School Project Building and Site a. The adjacent elementary school project construction will continue during the summer. The adjacent Park site will be redeveloped by the Elementary School Contractor and is scheduled to be complete by October 1, 2017. b. The Elementary School Contractor will be demolishing the adjacent Elementary School and link to this Recreation Center. The School project Contractor will be responsible to infill the wall where the link is demolished. This work is scheduled for this summer 2017. c. The existing Fire Alarm Panel for the existing school and recreation center is currently located in the Elementary School. The existing Fire Alarm Panel is to be relocated into the existing Recreation Center main mechanical room and reprogramed by the School Fire Alarm Subcontractor. 2. Asbestos Abatement a. Owner to discuss schedule b. Owner to discuss scope of abatement 3. Existing Building Renovation a. Mainly interior renovation and room finishes b. Revision of two restrooms to provide ADA group restrooms c. Revision of one restroom to include shower facilities. d. Expansion of Weight Room facility and supplemental AC. e. Miscellaneous casework revisions and updates f. New Gym flooring g. New basketball goals h. Existing building crawlspace i. Pre-stressed floor and roof structure j. C. Layout across wall or floor so that there is no tile less than 1/2 tile or 3” whichever is smaller. Notify Architect for specific directions if this affects a design or pattern. D. Where floor and wall tile are the same size, align floor tile joints with wall tile vertical joints. Align joints of accent banding tile with adjacent wall or floor tile. Notify Architect of questions concerning alignment. E. Cut and fit tile to penetrations through tile, leaving sealant joint space. Form corners and bases neatly. Align floor joints. F. Place tile joints uniform in width, subject to variance in tolerance allowed in tile size. Make grout joints without voids, cracks, excess mortar or excess grout, or too little grout. G. Form internal angles square and external angles bullnosed. H. Install non-ceramic trim in accordance with manufacturer's instructions. I. Install thresholds where indicated. J. Sound tile after setting. Replace hollow sounding units. K. Keep control and expansion joints free of mortar, grout, and adhesive. L. Install grout sealer to all cementitious grout. M. Keep control joints free of adhesive or grout. Apply sealant to joints. N. At expansion joints in wall tile, provide bullnose tiles to finish each side of the expansion joint opening unless a surface applied expansion joint cover is specified. At control joints in walls, do not run tile continuous; however regular tile may be installed on each side of control joint and apply sealant at joint to cover edges (do not grout). Otherwise, finish control joints as noted above for expansion joint openings. O. Install control joints at perimeter of tile floors and at other locations shown on Drawings, in accordance with ANSI A108.1 and A108.5 P. Prior to grouting, allow installation to completely cure; minimum of 48 hours. Q. Grout tile joints unless otherwise indicated. Use standard grout unless otherwise indicated. R. At changes in plane and tile-to-tile control joints, use tile sealant instead of grout, with either bond breaker tape or backer rod as appropriate to prevent three-sided bonding. S. 2.03 ACCESSORIES (FOR ATHLETIC SURFACES) A. Ventilating Base: Molded rubber, 4 inch high with a 3 inch toe, ventilating type, pre-molded outside corners, with adhesives and accessories, color as selected. B. Resilient Pads: 3/4" thick, hemispherical or manufacturer's standard equivalent design, two stage, polyurethane, Red-70D durometer. C. Thresholds: Mill finish aluminum; National Guard Products #427. 1/2" x 7" with beveled edges. 2.04 SOURCE QUALITY CONTROL A. Inspect and stamp species and grade on underside of each piece of wood flooring at factory. PART 3 EXECUTION 3.01 EXAMINATION A. Verify existing conditions before starting work. B. Verify that concrete subfloor surface is smooth and flat to plus or minus 1/4 inch in 10 feet. C. Verify wood subfloor is properly secured, smooth and flat to plus or minus 1/8 inch in 10 feet. D. Verify that required floor-mounted utilities are in correct location. E. Report any discrepancies which will affect the flooring installation. F. Verify all penetrations in concrete subfloor have been sealed. G. Prior to beginning installation, Wood Flooring Installer is to submit the following: 1. Verify moisture test readings on the concrete subfloor at 50' o.c.e.w. meet requirements of this section and the manufacturer. 2. Verify moisture test readings on maple, plywood and sleeper materials after stabilizing. (Use meter with teflon coated probes.) Moisture should be 10-12% range. 3. Relative humidity of the room air is maintained at levels required by this section and the manufacturer. 4. Calculations for dimensioning the ‘washer row’ spacing within the wood floor, based on the submitted product data, the installation moisture content, and the expected high end moisture content of at least 12%. 5. Dated certification from the manufacturer that the test data complies with their recommendations and that the calculations for internal expansion have been reviewed and are calculated correctly. 3.02 PREPARATION A. Subfloor: Place two layers of plywood subfloor over resilient pads. 1. Cover concrete with vapor retarder, sealing and lapping joints a minimum of 6". 2. Install lower layer of subfloor perpendicular to finish maple flooring spacing all edges 1/4” and stagger joints 4'. Provide 1-1/2” expansion voids at perimeter and at all vertical obstructions. The underside of first layer shall have resilient pads attached 12” on center (32 per sheet) and 6” from edges on all sides. Install solid blocking at doorways, under bleachers in the stacked position, and below portable goals. a. NOTE: Provide red 70D durometer pads under bleachers in the extended position. 3. The second layer of subfloor shall be laid at a 45-degree angle over the first layer, spacing all edges 1/4” stagger joints 4'. Provide 1-1/2” expansion voids at perimeter and at all vertical obstructions. Attach second layer of subfloor with fasteners 12” on center. B. Prepare substrate to receive wood flooring in accordance with manufacturer's and MFMA instructions. C. Broom clean substrate. 3.03 INSTALLATION A. Sheathing Paper: Place over wood subfloor; lap edges and ends 2 inches, staple in place. B. Wood Flooring:

1. Install in accordance with manufacturer's and MFMA instructions; predrill and blind nail to sleepers. 2. Lay flooring parallel to length of room areas. Verify alignment as work progresses. 3. Arrange flooring with end matched grain set flush and tight. 4. Terminate flooring at centerline of door openings where adjacent floor finish is dissimilar; provide divider strips and transition strips in accordance with flooring manufacturer's recommendations and as indicated. 5. Install edge strips at unprotected or exposed edges, and where flooring terminates. 6. Secure edge strips after installation of flooring with stainless steel screws. 7. Install flooring tight to floor access covers. 8. Provide 1-1/2 inch expansion space at fixed walls and other interruptions. C. Install hard rubber vented cove base at the wood floor perimeter where adjacent to walls. Use manufacturers premolded corners trim at all outside corners. Properly mitered corners allowed at inside corners only. Use adequate adhesive to secure base to wall only (allow movement between base and wood floor). Adhesive is not to block ventilation channels in base. D. Finishing: 1. Mask off adjacent surfaces before beginning sanding. 2. Sand flooring to smooth even finish with no evidence of sander marks. Take precautions to contain dust. Remove dust by vacuum. 3. After sanding, buff entire floor using 100 grit screenback or equal grit sandpaper. 4. Remove all dust by vacuum or tack. The entire floor is to be perfectly clean. 5. Apply finish in accordance with floor finish manufacturer's and MFMA instructions. 6. Apply filler and stain and four finish coats. 7. Apply first coat, allow to dry, then buff lightly with steel wool to remove irregularities. Vacuum clean and wipe with damp cloth before applying succeeding coat. 8. Lightly buff between coats with steel wool and vacuum clean before applying succeeding coat. 9. Apply last coat of finish. 3.04 CLEANING A. Clean and polish floor surfaces in accordance with floor finish manufacturer's instructions. 3.05 PROTECTION A. Prohibit traffic on floor finish for 48 hours after installation. B. Place protective coverings over finished floors; do not remove coverings until Substantial Completion. END OF SECTION

EXHAUST AND SUPPLY AIR FANS

PART 1 GENERAL

1.01 RELATED REQUIREMENTS

A. Comply with Division 1 - General Requirements and referenced documents. B. Comply with all other Division 23 Sections, as applicable. Refer to other Divisions for coordination of work with other portions of work. 1.02 SYSTEM DESCRIPTION

A. Provide exhaust fans of the type, rotational speed, and arrangement indicated. B. Each fan shall be rated to deliver the capacity indicated in the tabulation on the Schedule against the external resistance of the system in which it operates. 1.03 QUALITY ASSURANCE

A. All equipment and materials shall be new and of the best quality. B. All equipment and materials shall be installed by experienced mechanics and as recommended by the fan manufacturer. C. All fans shall bare the AMCA and U.L. Labels. Capacity ratings shall be based on tests performed in accordance with the latest version of AMCA Standard 210 and Publication 211. 1.04 SUBMITTALS

A. Product Data: Submit manufacturer's descriptive literature and installation instructions together with fan curves. B. Shop Drawings: Submit in accordance with Section 23 05 00. C. When equipment, other than specified, is proposed, the Contractor shall be completely responsible for electrical revisions necessitated. Submit listing of electrical feeder and conduit sizes, breaker sizes, and motor starter sizes for each item of equipment where motor sizes are required to be larger than specified to meet scheduled capacities. 1.05 PRODUCT HANDLING

A. Cover and protect fans in transit and at site. Fans not properly protected and stored and which are damaged or defaced during construction shall be rejected. Cover all openings to prevent entrance of dirt and debris until final connections are made. B. Storage and protection of materials shall be in accordance with Section 23 00 00. PART 2 PRODUCTS

2.01 CEILING CABINET EXHAUST FAN

A. Provide in-line type ceiling cabinet exhaust fans with the capacities and characteristics scheduled. B. Fans shall be AMCA certified and bear the label thereof.

C. Casing shall be made of galvanized steel and acoustically insulated for quiet operation. Housing shall be installed to provide for accessibility and removal of motor and blower without removing housing from the system. D. Motors shall be permanently lubricated and have accessible internal wiring. Provide permanent split capacitor (PSC) motors. Provide external toggle disconnect switch with each fan. E. Provide noiseless backdraft damper integral with unit. F. Provide SCR fan speed controller with minimum stop for motor protection to be factory mounted on unit to be used for final air balance purposes. G. Provide flat roof caps of the sizes indicated for each fan. Each cap shall be a minimum of eight inches (8") in diameter and shall be the curb mounted type to ensure proper flashing. Curbs shall be minimum eight inches (8") high, made of galvanized steel and be insulated with minimum 1-1/2", 1-1/2 PCF density insulation, have continuous perimeter treated wood nailer and be furnished with a neoprene isolation strip to be placed on the top of the nailer. Provide sloped bottom of curbs to match roof pitch to allow for roof caps to be installed level. H. Acceptable manufacturers: 1. Loren Cook Gemini. 2. ACME Masterette. 3. Greenheck SP/CSP. 4. Penn Zephyr. 5. FloAire. 6. Twin City Fans and Blowers, T or TL series. PART 3 EXECUTION

3.01 DELIVERY AND PROTECTION

A. Deliver all equipment to the site as indicated in Division 1. B. Contractor to perform installation and start-up to include installation of all accessories as required to make a complete and operating system. C. All equipment shall be handled carefully to avoid damage and be protected from exposure to the weather and dirt. All equipment shall be examined upon delivery to the site and evidence of abuse, damage, or exposure to weather and dirt shall be grounds for refusal to accept individual pieces of equipment. Rejected items shall be replaced promptly at no cost to the Owner. 3.02 EXHAUST FANS - INSTALLATION

A. Install fans, suspended from structure, or as indicated, and provide vibration isolation internally or externally as required, as specified herein, or as specified in other sections of these specifications. B. Suspended fans shall be set level with all thread rod from structure above. C. Field install motor and other accessories not factory installed. D. Verify operation of automatic motorized and backdraft dampers. E. Adjust fan drives and replace sheaves as required to obtain scheduled capacities as directed by the Test and Balance firm.
