Nadine Grimm RPh, BSc(Pharm) Nadine Grimm, from Dartmouth, NS, graduated in 1997 with a Bachelor of Science in Pharmacy from Dalhousie University. Since her graduation, Nadine has worked as a staff pharmacist at the IWK Health Centre in Halifax, where she has progressively developed her clinical specialty in neonatology. In her early career, Nadine worked in the satellite pharmacy providing medication to the NICU. In 1998, in addition to her staff pharmacist role, she began working one day per week as an antenatal clinical pharmacist. In 1999, Nadine began to pursue her passion of caring for the neonatal population by taking on the role of neonatal clinical pharmacist. Over her years in NICU, she has become a leader, sharing her wealth of knowledge with nurses, physicians, neonatal nurse practitioners; mentoring residents and pharmacy students, while always keeping the wellbeing and outcomes of her patients and their families in the forefront of the care she provides. Nadine has served on many committees at the IWK during her almost 20 year career. Nadine’s involvement in CSHP started very early in her career. She became a member after beginning work at the IWK. Her first formal involvement was joining the planning committee for the 2001 Summer Educational Sessions in Halifax, a committee she again joined when SES came back to Halifax in 2010. Nadine became a member of the CSHP NS Branch Awards Committee in 2002, which she chaired from 2004-1010 and continues to be a member. Nadine has contributed substantially to the NS Branch of CSHP, serving as President Elect, President and Past President, from 2011 to 2014. She has also served as advocacy representative for NS Branch and is currently on the Board of the Pharmacy Association of Nova Scotia, representing CSHP. Nadine has spoken at numerous pharmacy events emphasizing the impact CSHP has had on her career, the amazing people she has met, and encouraging pharmacists and students to get involved. Nadine lives in Dartmouth, with her husband Shawn, also a pharmacist and Guardian owner, and son Evan. She enjoys traveling and is an avid fan.

The Isabel E. Stauffer Meritorious Service Award is given in recognition of prolonged service to and involvement in CSHP, by a Member of the Society, as exemplified by significant, sustained contributions primarily at the Branch or Chapter level. Individuals are nominated by their peers.

Erin Alexandria Cicinelli BScH Erin Cicinelli is a fourth-year PharmD student at the University of Toronto. She grew up in Bolton, a small town north of Toronto and completed her Bachelor of Science degree at the University of Toronto in 2013. She chose to pursue a career in pharmacy after witnessing the important role that pharmacists play in patient care while working in an independent pharmacy in her hometown. Erin feels fortunate to have gained exposure to many areas of the multifaceted profession of pharmacy. She has completed clinical rotations in hemodialysis, general medicine, and intensive care at the University Health Network. In addition, she has worked on pharmacoepidemiology and pharmacy practice research studies and presented her work at the OPA and CSHP Professional Practice Conferences. In the summer of 2016, she completed a rotation at St. Mary’s Hospital Lacor in Uganda where she worked with the Pharmacy Department to develop protocols and collect data for quality improvement projects. As a pharmacy student, Erin became involved in various leadership, advocacy, and charity initiatives. In 2014, she founded a group that organizes volunteer opportunities in the community to enhance students’ awareness of the social determinants of health. She has served terms on Governing Council at both the university and faculty levels, representing the student voice on curriculum and student life matters. She is currently President of the fourth-year class and works with her council to plan activities that enhance the social and academic experience of her classmates. Erin feels honoured to have been selected as this year’s recipient of the Hospital Pharmacy Student Award. She is grateful to the research supervisors, preceptors, and staff members at her faculty who have mentored and supported her throughout her pharmacy education. Outside of pharmacy, Erin’s interests include playing soccer, hiking, and enjoying the vibrant Toronto arts and culture scene. She hopes to one day practice as hospital pharmacist and incorporate patient care, research, and teaching activities throughout her career.

The Hospital Pharmacy Student Award is given to a pharmacy student who shows promise as a future hospital pharmacy practitioner through student activities or experiential training in direct patient care, research, or education. The selected candidate exhibits eagerness, dedication, and a positive attitude toward academic learning and toward the profession and practice of hospital pharmacy.

In recognition of outstanding career contributions to hospital pharmacy or some related field of healthcare, CSHP Members may nominate their colleagues for an honorary CSHP membership. Honorary Members have been elected by unanimous vote of the Board in recognition of outstanding contributions to the advancement of hospital pharmacy or a related healthcare field, and exceptional service directly or indirectly to the mission of CSHP. The status of Honorary Member shall be granted for life, subject to the provisions of Bylaw Article 3.1.1, and subject to the Policy on Honorary Membership. Honorary Members shall be exempt from the payment of membership fees.

Rosemary A Bacovsky BScPharm, MPharm, MHSA, FASHP, FCSHP, Honorary Membership CSHP Rosemary Bacovsky graduated from the University of Alberta with a BScPharm (1977), MPharm (1985) and MHSA (1997). She completed a residency at the Royal Alexandra Hospital. While at the Cross Cancer Institute (1978-88), she implemented clinical oncology programs and a chemotherapy IV admixture program, one of the first in Canada. She developed the CSHP standards for handling and disposal of hazardous pharmaceuticals which were endorsed by the Canadian Standards Association. She was instrumental in the establishment of the International Society of Oncology Pharmacy Practitioners and in the organization of its third international Symposium (Toronto 1993). After leaving the Cross Cancer Institute, she pursued a master’s in Health Services Administration. She started consulting while doing graduate studies. Her recommendations while reviewing Alberta Health drug plans, resulted in the drug benefit list, least cost alternative policy and revised pharmacy reimbursement. In 1990-96, she was employed by Alberta Health working on policy issues, including developing home parenteral drug therapy programs and managing the government drug programs during her last year. During this time, the Government of Alberta was revising health profession legislation. She saw this as an opportunity for pharmacists to expand their scope of practice, especially prescribing. She believed that pharmacists should proactively use their knowledge in managing medication therapy rather than being reactive by correcting prescribing being done by others. She championed politicians, government officials and fellow pharmacists for expanded scope. In the early 2000s, she prepared discussion papers for the Alberta Pharmaceutical Association that documented that pharmacists were already essentially prescribing through delegated activities and that their education had the most training surrounding medications of all the health professions. She also helped develop the prescribing framework, wording for legislation and relevant standards. On April 1, 2007, pharmacists in Alberta were granted the broadest scope of practice in North America, including independent prescribing. Rosemary held numerous elected and appointed executive and other positions in provincial, national and international pharmacy and health organizations. She received many awards including the Alberta Pharmacy Centennial Award of Distinction, CPhA Centennial Pharmacist Award, CSHP Distinguished Service Award, and the University of Alberta Faculty of Pharmacy and Pharmaceutical Sciences Outstanding Alumni Award. She is a Fellow of CSHP and ASHP. Since leaving Alberta Health in 1996, she consults full time. Currently she prepares reports on pharmaceutical policy and serves as an expert witness in federal court proceedings on drug plans and practices. When not consulting, she and her husband travel the world to enjoy great food experiences.

CSHP Fellow status is conferred by the Fellows (FCSHP) Recognition Committee upon CSHP Members who have demonstrated noteworthy, sustained service and excellence in the practice of pharmacy in an organized healthcare setting. Congratulations are extended to CSHP Members who were awarded the status of CSHP Fellow in 2016: Donald Kuntz, Janice Munroe, and Marc Perreault

Donald Kuntz BSP Don Kuntz is Clinical Coordinator and Special Projects lead pharmacist for the Regina Qu’Appelle Health Region at the Wascana Rehabilitation Centre, which has 250 complex needs long term care beds, 50 active rehabilitation beds and 5 pediatric respite beds. Don is currently involved with two projects at Wascana, the first being pharmacy technician computer order entry with pharmacist verification. The second project is the establishment of a five bed dementia assessment and treatment unit for patients from southern Saskatchewan whose behaviour is not safely managed. This project also includes the establishment of a multi-disciplinary resource team to assist with assessment and treatment of these patients. In 1981, Don graduated from the University of Saskatchewan with a Bachelor of Science in Pharmacy. Shortly thereafter he joined the research team studying and measuring hospital pharmacy workload, travelling Canada to observe hospital pharmacists and technicians in action. Following the conclusion of the study, Don began a career in hospital pharmacy in Regina where he was a staff pharmacist for 12 years. This was followed by 10 years as a Clinical Coordinator with cardiology as his clinical practice area. In 2005 Don moved to the Wascana Rehabilitation Centre plus responsibilities for the outpatient pharmacist directed anticoagulation clinic, kidney transplant clinic and hemodialysis units. Don has provided rehabilitation teaching for COPD patients for the past 17 years. Don was seconded for regional initiatives including 2008-2009 as a manager with the Quality Improvement Department for the successful implementation of medication reconciliation on admission to acute care. Don continues to be involved and consulted with medication reconciliation challenges within the health region and province. He has precepted and mentored pharmacy residents with rotations in cardiology, anticoagulation management and rehabilitation/long term care/geriatrics for the past 25 years in addition to pharmacy students. Don has volunteered with CSHP for many years, serving on a variety of provincial and CSHP Official committees, task forces and steering committees including Branch Delegate from 2004 – 2014 and the 2015 Steering Committee from inception to completion of the initiative. Don has received numerous recognitions during his career including the Saskatchewan Branch Pharmacist of the Year in 2005, the JL Summers Achievement Award in 2010 and Pharmaceutical Care Awards in 2000 and 2001. Don was a finalist for a Saskatchewan Healthcare Excellence Awards in 2005 for which he was nominated by the patients at the Wascana Rehabilitation Centre. Don received a Saskatchewan College of Pharmacists Presidential Citation in April 2015 for dedicated commitment and excellence in patient care. In January 2016, Don was honored to receive the CSHP Distinguished Service Award. Don has a keen interest in patient safety initiatives and standards of care. He is a strong advocate for his patients and is passionate about direct and reliable care.

Janice Munroe BSc.Pharm. Janice Munroe’s pharmacy career has focused on advancing hospital pharmacy practice and improving medication safety for over 36 years. During her career, she has held a variety of hospital pharmacy positions in addition to working in community pharmacy concurrently for a 10 year period. Throughout her diverse career Janice has held a variety of clinical, distribution, supervisory, mentorship, coordinator and leadership roles. Most recently, Janice became the Pharmacy Site Coordinator at Ridge Meadows Hospital for both Clinical and Distribution Services. From 2005 through to October of 2016, Janice was the Regional Medication Safety Coordinator for the 13 sites in the Fraser Health Authority. It was while she was in this role that she established the CSHP Medication Safety Pharmacy Specialty Network (PSN). From the PSN’s infancy when there were only 10 members it has grown into the well established and effective PSN that it is today with over 300 members and a very high level of activity/sharing. Janice has been a mentor to pharmacists and pharmacy students alike. She established a 4 week medication safety rotation to support the education and the spread of knowledge of important medication safety principles. A key component of the rotation is the completion of medication safety projects that have ultimately shaped the medication use system in both FH and across LMPS. As a result of sharing her knowledge and experience, Janice has become an established and recognized speaker on a variety of topics. Throughout her career, Janice has been an active supporter and contributor to CSHP. She has held a number of executive level positions at the branch level in both Manitoba and in British Columbia. Janice has also held several executive officer positions at the CSHP National level including the Presidential Officer position which was responsible for the Internal portfolio from 2010-2013. Janice has also been a long standing supporter of the importance of education for hospital pharmacy leadership. For over 7 years, she has organized and facilitated the annual CSHP Hospital Pharmacy Management Seminar held in Harrison Hot Springs, BC. This Seminar has focused on advancing the knowledge and education of hospital pharmacy leaders across Canada. Janice supported practice excellence in her role as an active Accreditation Canada surveyor for over 8 years from 2008-2016 conducting both international and Canadian surveys. She has been an active participating member of the Accreditation Canada committee responsible for the Medication Management Standards. She was responsible for revising the standards and ROPs along with establishing the first expert Faculty and the associated FAQs. Janice is an avid world traveller with her husband. When at home, she enjoys spending time with her two sons and three grandchildren.

Marc Perreault B.Pharm, M.Sc., PharmD Marc achieved his BSc in Pharmacy at Université de Montréal and a hospital residency in Hôpital St-Luc in 1989. This was followed by a post-BS PharmD degree from the State University of New York at Buffalo in 1991. He moved back to Montreal to complete a 2 year fellowship in cardiovascular pharmacology. Since then, his area of clinical expertise has been critical care pharmacotherapy. Depuis 2001, il détient un poste hospitalo-universitaire partageant ainsi son temps entre les soins critiques et sa charge de travail professorale. Il a débuté comme professeur adjoint, puis agrégé et maintenant titulaire de clinique. Il a participé à la mise en place du programme de PharmD de premier cycle et a mené la refonte du programme de MSc en pharmacothérapie avancée. Il a été directeur du programme de résidence de cette Université de 2007 à 2016 où environ 40 candidats sont promus chaque année. Marc has always been advocating for our profession through a variety of activities either provincially through APES or nationally through CSHP. He has been involved with the CSHP research committee, and he currently serves as an assistant editor for the Canadian Journal of Hospital Pharmacy since 2012. He sat on the PEBC panel of examiners and has been involved with several task forces dealing with specialization/ professionnal development within Quebec. In 2015, he received from APES the Roger Leblanc Award for excellence in his career. Recently, with great enthousiam, he joined the Canadian Pharmacy Residency Board as the AFPC representative. His current clinical practice remains critical care. He has worked at the Hospital, General Campus as a clinical pharmacy specialist in the intensive care unit where his preceptorship role was recognized by the Mentorship Award from the Canadian Society of Hospital Pharmacists - Branch in 2001. Il travaille à demi-temps à l’unité des soins intensifs de l’hôpital Général de Montréal où lui et ses collègues forment des étudiants, des résidents et des résidents spécialistes québecois, canadiens et étrangers à leur rôle actuel et futur. Awards Committee will soon invite applications and nominations for the 2017-2018 National Awards CSHP Program. The goal of the awards program is to improve patient outcomes by promoting excellence in the practice of pharmacy throughout hospitals and other collaborative healthcare settings. The awards are presented to CSHP Members (pharmacists) in recognition of their professional dedication. The Hospital Pharmacy Student Award is presented to a student pharmacist.

The program includes general and special awards.

Consider applying for the New Technology Award. This award is our latest offering, and it recognizes CSHP Members who have contributed significantly to the development, implementation, and/or evaluation of new technology in an organized healthcare setting and meeting one or more of the following criteria: promotes a safe and effective adoption of this new technology at other health facilities in Canada; provides a comprehensive review of the technology within their organization. This review may include a systematic approach to implementation, monitoring resource requirements and/or other outcomes attributable to uptake of the technology among staff and

patients; may include a comprehensive review of the potential risks and benefits that the new technology introduces to the practice setting.

For the GENERAL AWARDS program, you may submit two projects for consideration (each project is eligible for two awards, for a maximum total of four award submissions) using the online application for general awards. For each SPECIAL AWARD, applicants or nominators must complete the specific online nomination for the intended award.

Once the program has opened, a call for awards will be issued by email and in the eBulletin. Criteria, application and nomination forms will be available at the CSHP website: cshp.ca/programs/awards/ national_e.asp

Application and nomination forms, as well as the relevant signature forms, are due by September 5, 2017, except for applications and signature forms for the Hospital Pharmacy Student Award, which are due by September 30, 2017.
