ILIOPSOAS TENDONITIS / BURSITIS What is the Iliopsoas and Bursa? The iliopsoas is a muscle that runs from your lower back through the pelvis to attach to a small bump (the lesser trochanter) on the top portion of the thighbone near your groin. This muscle has the important job of helping to bend the hip—it helps you to lift your leg when going up and down stairs or to start getting out of a car. A fluid-filled sac (bursa) helps to protect and allow the tendon to glide during these movements. The iliopsoas tendon can become inflamed or overworked during repetitive activities. The tendon can also become irritated after hip replacement surgery. Signs and Symptoms Iliopsoas issues may feel like “a pulled groin muscle”. The main symptom is usually a catch during certain movements such as when trying to put on socks or rising from a seated position. You may find yourself leading with your other leg when going up the stairs to avoid lifting the painful leg. The pain may extend from the groin to the inside of the thigh area. Snapping or clicking within the front of the hip can also be experienced. Do not worry this is not your hip trying to pop out of socket but it is usually the iliopsoas tendon rubbing over the hip joint or pelvis. Treatment Conservative treatment in the form of stretching and strengthening usually helps with the majority of patients with iliopsoas bursitis. This issue is the result of soft tissue inflammation, therefore rest, ice, anti- inflammatory medications, physical therapy exercises, and/or injections are effective treatment options. -

The Importance of Load Vector in Physique Enhancement and Sport Training An Excerpt from Advanced Techniques in Glutei Maximi Strengthening Available at www.TheGluteGuy.Com By Bret Contreras, MA CSCS Most experts in the fitness industry fail to recognize the role that the load vector plays in determining hip extensor recruitment during hip extension exercises. As a case in point, most articles written on glutei maximi training recommend axial loaded exercises such as squats, deadlifts, and lunges. But axial exercises are greatly outperformed in glutei maximi mean and peak activation by anteroposterior exercises. I don’t expect the experts to know this, as to my knowledge, I am the first author who is pointing this out scientifically. Although it’s been hinted about by strength coach Mike Boyle in the past and recently inferred (during the time I was writing this book) by vertical jump specialist Kelly Baggett, and although there have been many small-scale EMG studies performed in the scientific literature, to my knowledge a large-scale EMG study involving dozens of advanced and complex hip extension exercises has never been disclosed to the public – until now.1,2 I will discuss specific exercises and EMG data later in this book, but first let’s continue talking about load vectors. I just performed a Google search for “best glute exercises” and the first article that came up was a web document written by Ray Burton, an ISSA Certified Trainer out of Canada, entitled “Glute Exercises – My Top Three Choices.” His top three choices are reverse lunges, sumo squats, and a special step up (a hybrid step-down/assisted pistol movement), which are all axial loaded exercises.3 I’m not trying to pick on Burton, as articles like these are littered throughout the Internet and forums. -

A altered Sihler’s staining was conducted on sartorius muscles (15 specimens). The nerve entry points and intramuscular arborization areas were measured as a percentage of the total distance from the most prominent point of the anterior superior iliac spine (0%) to the medial femoral epicondyle (100%). Intramuscular neural distribution were densely detected at 20–40% and 60–80% for the sartorius muscles. The result suggests that the treatment of sartorius muscle spasticity requires botulinum neurotoxin injections in particular locations. These locations, corresponding to the locations of maximum arborization, are suggested as the most safest and effective points for botulinum neurotoxin injection. Introduction Spasticity is a main contributor to functional loss in patients with impaired central nervous system, such as in stroke, cerebral palsy, multiple sclerosis, traumatic brain injury, spinal cord injury, and others 1. Sartorius muscle, as a hip and knee exor, is one of the commonly involved muscles, and long-lasting spasticity of the muscle results in abnormalities secondary to muscle hyperactivity, affecting lower levels of functions, such as impairment of gait. -

Pes Anserinus Syndrome

Safran, MD Professor, Orthopaedic Surgery Chief, Division of Sports Medicine PES ANSERINUS SYNDROME DESCRIPTION The pes anserinus is the tendon insertion of 3 muscles of the thigh into the upper leg (tibia), just below the knee to the inner side of the front of the leg. Where the tendon attaches to bone, there is a bursa sac between the bone and the tendon. The bursa functions like a water balloon to reduce friction and wear of the tendon against the bone. With this syndrome there is inflammation and pain of the bursa (bursitis), tendon (tendinitis), or both. FREQUENT SIGNS AND SYMPTOMS Pain, tenderness, swelling, warmth and/or redness over the pes anserinus bursa and tendon on the front inner leg just 2-3 inches below the knee. The pain is usually slight when beginning to exercise and is worse as the activity continues. Pain with running or bending the knee against resistance Crepitation (a crackling sound) when the tendon or bursa is moved or touched CAUSES Strain from sudden increase in amount or intensity of activity or overuse of the lower extremity usually in the endurance athlete or the athlete just beginning to run. May also be due to direct trauma to the upper leg. RISK INCREASES WITH Endurance sports (distance runs, triathlons) Beginning a training program Sports that require pivoting, cutting (sudden change of direction while running), jumping and deceleration. Incorrect training techniques that include excessive hill running, recent large increases in mileage, inadequate time for rest between workouts.. Poor physical conditioning (strength/flexibility) Inadequate warm-up prior to practice or play Knock knees Arthritis of the knee. -

Roberts, MD; Pamela Landsteiner, MD A 19-year-old man presented to the dermatology clinic with intermittent pruritic lesions that began on the bilateral buttocks when he was living in Reserve Officers’ Training Corps dormitories sev- eral months prior. The eruption then spread to involve the penis, suprapubic area, periumbilical area, and flanks.copy The patient attempted to treat the lesions with topical antifungals prior to evalua- tion in the emergency department where he was treated with permethrin 5% on 2 separate occa- sionsnot without any improvement. A medical history was normal, and he denied recent travel, animal contacts, or new medications. Physical examina- Dotion revealed several 2- to 4-mm erythematous papules and superficial erosions with an ill-defined erythematous background most notable on the penis, suprapubic area, periumbilical area, flanks, and buttocks. We report this case to emphasize the importance of the patient, we performed trigger point injections under ultrasonography physical examination in patients presenting with symptoms guidance on the myofascial TrPs. Dramatic improvement of the suggestive of lumbar radiculitis.
