Delaney, DDS, MSD, and Martha Ann Keels, DDS, PhD Parents often are concerned with “lumps and bumps” that appear in the mouths of children. Pediatricians should be able to distinguish the normal clinical appearance of the intraoral tissues in children from gingivitis, periodontal abnormalities, and oral lesions. Recognizing early primary tooth mobility or early primary tooth loss is critical because these dental findings may be indicative of a severe underlying medical illness. Diagnostic criteria and .treatment recommendations are reviewed for many commonly encountered oral conditions. INTRAORAL SOFT-TISSUE ABNORMALITIES Congenital Lesions Ankyloglossia Ankyloglossia, or “tongue-tied,” is a common congenital condition characterized by an abnormally short lingual frenum and the inability to extend the tongue. The frenum may lengthen with growth to produce normal function. If the extent of the ankyloglossia is severe, speech may be affected, mandating speech therapy or surgical correction. If a child is able to extend his or her tongue sufficiently far to moisten the lower lip, then a frenectomy usually is not indicated (Fig. 1). The knowledge of cementum was extended through (1) a more complete elemental composition analysis and (2) the relation of element distribution to the ultrastructure structure throughout the life of a tooth. This study was exploratory and demonstrated that lead, zinc, mercury, and barium follow the same general line of changes, and most likely represent changes in health and exposure to these metals in the general environment. Copper, manganese and vanadium varied very little. Tim Wright, DDS, MS The development of the human dentition involves a highly orches- trated series of events that are strictly genetically controlled. The devel- opmental timing, location, morphology, structure, and composition of teeth are primarily determined by cascades of molecular events that are regulated by hundreds of genes.4oNormally, humans have 20 primary (i.e., deciduous, or shedding) teeth, which are lost in childhood, and 32 permanent teeth. Dentition begins to form at approximately 6 weeks in utero and continues through late adolescence, when the development of the permanent third molars is completed. Because the development of dentition is prolonged, it is susceptible to environmental influences for many years. A basic understanding of normal dentition and its development allows clinicians to accurately identdy normal and abnor- mal dental conditions and make recommendations for appropriate thera- peutic interventions and patient counseling. Delineating normal from abnormal dental development requires careful evaluation of the patient, including a medical, dental, and family history; clinical examination; and radiographic evaluation and may require special laboratory tests. This article reviews normal dentition and fundamental concepts of tooth development and provides a conceptual framework for diagnosing de- velopmental defects of teeth.
