NOTICE OF FIRST REVISED SEWERAGE SERVICES TARIFF Notice is hereby given that National Water Infrastructure, LLC (Company) filed First Revised Sewerage Services Tariff with the Louisiana Public Service Commission (LPSC) to add a Noncompliant Effluent Fee for Commercial Customers, which is a non-recurring charge applicable to each instance where a Commercial Customer’s effluent to the facilities of the Company exceeds 250 ppm (or mg/L) for Total Suspended Solids or 220 ppm (or mg/L) for BOD 5-d, 20ºC. The Noncompliant Effluent Fee for Commercial Customers shall be $500 per failed test performed by the Company for up to six failed tests in any consecutive 12-month period. Each failed test exceeding six that occurs during the same consecutive 12-month period shall be subject to a Noncompliant Effluent Fee for Commercial Customers of $1,000 per failed test. First Revised Sewerage Services Tariff is proposed to become effective May 1, 2021.

TMBC, LLC (“TMBC”), publishes this Notice of Intent to Submit a Request for Exemption from Louisiana Administrative Code (LAC) 33:VII.10591.G. Specifically, the regulation requires waste tire generators to identify the waste tire fee as the “LDEQ Waste Tire Fee” on sales invoices. However, due to technological limitations of the cash registers and receipt printing equipment used by TMBC stores, the line item labels on sales receipts are limited to 10 characters, including spaces, whereas the label required under LAC 33:VII.10591.G contains 20 characters, including spaces. As such, TMBC proposes to label the waste tire fee on each receipt with the text “DEQTIREFEE”, so that the existing equipment may be used.

The exemption request will not have any significant adverse impact on the health safety, and welfare of the public and the environment. Further, TMBC believes the requested language is consistent with the applicable provisions of the LDEQ waste tire regulations.

As such, pursuant to the exemption provision found in LAC 33:VII.10507, TMBC will request an exemption for its stores located at the following addresses:

Chairman Dugas called the Regular Board Meeting of the Board of Commissioners to order at 6:00 p.m. The invocation was given by Mr. Blevins and the assembly recited the Pledge of Allegiance.

Chairman Dugas called the roll with the following members present: Mr. Darren Blevins, Mr. Joseph Blanchard, Mr. Robert Dugas, Mr. Jeff Easley and Mr. Leslie Falks. Chief James T. Wascom was absent. Several LPFPD4 personnel were present along with Mr. Trey Sanders and Mr. Yanez Moree.

Chairman Dugas announced that public input would be accepted from any member of the audience wishing to address an agenda item. There being no public input, the Board moved on.

Mr. Blevins made the motion, seconded by Mr. Falks, to adopt the minutes of the March 15, 2021 Regular Board Meeting.

The March 2021 Financial Report was reviewed with Trey Sanders from Hannis T. Bourgeois, LLP along with the Board Members. The Financial Report includes Balance Sheet for assets as well as liabilities and fund balance, Statement of Income, Budget to Actual, and General Fixed Assets.

Chairman Dugas along with Project Manager Yanez Moree, from Quality Engineering & Surveying, LLC, updated the Board on Station 11’s property and utility concerns of current site. A discussion was held on selecting an alternate site for construction of Station 11.

There being no further old business, the Board moved on to new business.

Trey Sanders presented the 2020 and 2021 Engagement Agreement with Hannis T. Bourgeois.

Training/Safety Officer Keesler Fly gave the training and safety report for 2021 first quarter. Fire Prevention Officer Matt Gallman gave the fire prevention report for 2021 first quarter. Deputy Chief Jackson presented the response statistics for LPFPD4’s total of 565 calls in February and 534 calls in March of 2021 as well as an update on manpower/force backs.
