A. Basic Services of Funeral Director and Staff

Prices are effective as of January, 2016 and are subject to change without notice.

The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods you selected.

PROFESSIONAL SERVICES: A. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF ...... $2995.00 Our charge includes consultation with the family or responsible party; staff and facilities to respond to initial requests for service; coordinating service plans with cemetery, and/or other parties involved in the final disposition of the deceased; securing official documents and necessary registrations and sheltering of the deceased. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains.) B. EMBALMING ...... $695.00 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement which does not require you to pay for it, such as direct cremation or immediate burial. C. EMBALMING SPECIAL CASES ...... $1200.00 D. OTHER PREPARATION OF THE BODY ...... $295.00 Includes cosmetology, casketing and dressing of the remains. E. HAIRDRESSING ...... $100.00

FACILITIES: A. USE OF FACILITIES AND STAFF FOR VIEWING/VISITATION ...... $795.00 B. USE OF FACILITIES AND STAFF FOR FUNERAL CEREMONY ...... $595.00 C. REFRIGERATION ...... $195.00 It is policy of our firm to require refrigeration of an unembalmed body after 24 hours. At 4 days of refrigeration $195.00 will be applied to the contract. 5 days or more of refrigeration there will be an additional $50.00 charge per day.

TRANSPORTATION: A. TRANSFER OF REMAINS TO FUNERAL HOME ($1.50 OVER 40 MILES) ...... $695.00 B. USE OF HEARSE FOR TRANSPORTATION ($2.00 OVER 40 MILES) ...... $695.00

FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME ...... $2995.00 This charge includes transfer of remains, minimum services of staff, necessary authorizations, embalming, local transportation (airline or other shipping charges are not included). RECEIVING REMAINS FROM ANOTHER FUNERAL HOME ...... $2995.00 This charge includes minimum services of staff, care of remains, and transportation of remains to cemetery or crematory (this does not include a funeral ceremony or any type of visitation).

DIRECT CREMATION ...... $2995.00 Our charge for a direct cremation (without ceremony) includes local transportation of remains to our crematory, the cremation process at our crematory, a cardboard container for the remains, a container for the ashes, filing necessary authorizations and permits with state and local authorities and all services of our staff (this does not include a visitation or any type of funeral service or necessary permits obtained from local counties). If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are constructed of cardboard. DIRECT CREMATION WITH A MEMORIAL SERVICE/VISITATION ...... $3995.00 This charge includes all items listed under “DIRECT CREMATION”, minimum services of staff and a memorial service at our funeral home without the body present. DIRECT CREMATION WITH CONTAINER PROVIDED BY PURCHASER ...... $3095.00 DIRECT CREMATION WITH MINIMUM CASKET ...... $3695.00 DIRECT CREMATION WITH ANY TYPE OF CASKET IS $2995.00 + CASKET PRICE If you want to arrange a direct cremation, you can use an unfinished wood box or an alternative container. Alternative containers can be made of materials like heavy cardboard or composition materials (with or without outside covering), or pouches or canvas.

IMMEDIATE BURIAL ...... $2995.00 Our charges for immediate burial (without ceremony, visitation, embalming and casket) includes all local transportation and necessary staff and authorizations.
