APPLICATION FOR ROTARY CLUB OF CHULA VISTA SCHOLARSHIP AND/OR JOSEPH RINDONE SERVICE AWARD Rotary Club of Chula Vista Post Office Box 626 Chula Vista, California 91912 School Year 2012-2013 General Information One Rotary Scholarship/Joseph Rindone Service award in the amount of $1,800 will be provided to a senior student from each of the five Sweetwater District high schools served by the Chula Vista Rotary Club. This year the Rotary Scholarship will be combined with the Joseph Rindone Service Award into a single award for each school (rather than two separate awards as in past years). Eligibility The Rotary Club's Scholarship/Service recognition program is made available to seniors at five high schools in the Chula Vista area: Castle Park High School, Chula Vista High School, Hilltop High School, Olympian High School, and Otay Ranch High School. Seniors attending Eastlake High School and Bonita Vista High School should contact the Chula Vista Sunrise Rotary Club (www.chulavistasunriserotary.org) for information about their scholarship programs. Selection Criteria The criteria for selecting recipients for the Rotary Scholarship/Joseph Rindone Service award are, in order of importance: 1. Academic Achievement: Transcript grades, GPA, rank in class, academic rigor (AP, IB, honors classes). Source: School records (transcript); 2. Service: Types of service and hours served in school and community service activities. Source: Application form, applicant essay, school records (transcript record of service hours); 3. Need: Family financial factors, other college financial assistance received, student work record. Source: Application form, applicant essay; 4. Co-curricular Activities: Participation in sports, music, clubs, and other school groups. Source: Application form, applicant essay, school records (transcript). 1 Instructions Deadline for submitting applications: April 26, 2013. You may choose to submit your application in one of two ways: 1. Directly to the Rotary Club of Chula Vista. Print this application and complete it. Scan: (1) the completed application, (2) a copy of your transcript, and (3) any letters of recommendation you may have requested from teachers, counselors, or school administrators. Send all of the scanned documents as attachments to an Email: [email protected] If you do not have access to a scanner, you may mail hard copies of your application, transcript, and letters of recommendation to: Rotary Club of Chula Vista Executive Secretary P.O. Box 626 Chula Vista, California 91912 2. Through your high school’s scholarship counselor. Print this application, complete it, and submit it to your school’s scholarship counselor.
