Dear Honourable Members ,

As the secretary of MOSCMM, Puthupadi, I would like to present before you its Fifth Annual Report. We continue to pursue the philanthropic objectives as we used to in the past. Firstly I must thank H.H.Baseliose Paulose 11 (Catholicose of the East and Malankara Metropolitan) for His Holiness’s inspiring support as the president of the society. I am also grateful to H.G. Zacharia Mar Theophilose (Metropolitan of Malabar diocese and Vice President of MOSCMM Puthuppadi) for his guidance and participation in the daily activities of the society . At this juncture I would like to place on record our indebtedness to your unstinging support and association with the society and its activities. I gratefully acknowledge the assistance and cooperation received from all our Donors, Vendors, Doctors, Patients, Auditors, Legal advisors, Banks, Puthuppadi Grama Panchayat and the Government authorities during the year 2016-17. I recall the service of Mr.Manoj Cherian, who resigned as joint secretary of the Society and Rev.Fr.Bobby Peter, who relinguished his office as Director, on personal grounds. MOSCMM will always remember their services to the society and looks forward to their continued support to achieve our Philanthropic objectives in future. With great pleasure, we welcomed H.G. Dr.Philipose Mar Chrysostom of Mar Thoma Church during his visit to the Dialysis Center. The visit by the dignitory has undoubtedtly boosted the morale of both the staff and the officials. I wish to place on record our appreciation for the committed service of the staff under the leadership and guidance of Dr. Abraham Mammen (Medical Director) and Dr. Rojan Kuruvilla (HR). It is a fact that in the absence of such a dedicated team we would have accomplish nothing. I also thankfully recall the service of Mr.Raju Mammen renders voluntary service in the pharmacy at the Dialysis center. Immense is our gratitude to Rev.Philip Ramban for his signal support as founding member of the

MOSCMM 1 ANNUAL REPORT 2016-17 society and its spiritual advisor. However the mention of the innumerable benefactors of the society is beyond the boudary of such a sketchy report. Yet, a few names ought to be mentioned - Sr. Rachel, who left us to join Yacharam School, (Hyderabad) Mr.Vineeth – (Dialysis In charge) who emigrated to Canada, Mr. Joy John and Mr. M.K. Kuriakose ( Facility Management), Mr. Charles Thomas , incharge of website maintenance and Mr. Tony Abraham, supervising IT supports in general. I am happy to inform you that during this year we have completed the construction of Sewage Treatment Plant (STP). The construction of a new well to ensure adequate water supply to the dialysis center has also began. Requesting your continued support and patronage in the years to come

Dear Members ,

It is with great pleasure and honor that I present the annual report of the Medical Director, St George Charity Dialysis Centre , Puthupady for the year 2016-2017. Consolidating and progressing from the position we had established in 2015, I am proud and honored to inform you that we have now a well established Stand alone centre of world class amenities. So at the outset I would like to thank the almighty for His continued support and guidance. Our Vice President H.G. Zachariah Mar Theophilose , Rev K.I Philip Remban , all the office bearers and the members of the society have worked diligently, prayed fervently and thus guided us with great discretion.

On 8th December 2016 we completed five years of service. I am pleased to announce that we have a total of 26 patients on two shifts per day. We have successfully completed over 8000 dialysis without any problems. We have seven dialysis machines which are used daily on two shifts. We continue to have the support of three renowned nephrologists who supervise the patients on a regular basis. Our staff team consisting of seven nurses, two housekeeping staff, two incharge of the office and two for security continue to be our source of pride and joy, as they render invaluable service in the care of the dialysis patients. I firmly believe that the reputation of the centre as a heaven for those with renal problems is attributable solely to the sincere and dedicated service of our staff. We plan to increase the patient load to a maximum of thirty in the coming years. A Sewage treatment plant is started as it is essential for the pollution control board certification. Due to the acute water shortage during summer , it is necessary to find out additional water sources for the center. This is being done urgently. It would be inappropriate if I do not recall the timely and sensible support received from our Secretary , Joint Secretary , Treasurer, and all the members of the various committees. A very big thank you to all and I hope to have their continued support in the years to come. I thank you all for having given me this opportunity to present the report on the activities of the dialysis centre for the year 2016- 2017. I am indebted to each one of you for having given me this opportunity to serve this institution in this capacity. I pray that your prayers and support will always be with the centre in the years to come.

Report on the Financial Statements

We have audited the accompanying financial statements of Malankara Orthodox Syrian Church Medical Mission which comprise the Balance Sheet as at March 31, 2017, the Income and Expenditure Account and Receipt and Payment Account for the year then ended, and a summary of significant accounting policies and other explanatory information.

Management’s Responsibility for the Financial Statements Management is responsible for the preparation of these financial statements that give a true and fair view of the financial position and financial performance of the Church in accordance with the Accounting Standards. This responsibility includes the design, implementation and maintenance of internal control relevant to the preparation and presentation of the financial statements that give a true and fair view and are free from material misstatement, whether due to fraud or error.

Auditor’s Responsibility

Our responsibility is to express an opinion on these financial statements based on our audit. We conducted our audit in accordance with the Standards on Auditing issued by the Institute of Chartered Accountants of India. Those Standards require that we comply with ethical requirements and plan and perform the audit to obtain reasonable assurance about whether the financial statements are free from material misstatement.

An audit involves performing procedures to obtain audit evidence about the amounts and disclosures in the financial statements. The procedures selected depend on the auditor’s judgment, including the assessment of the risks of material misstatement of the financial statements, whether due to fraud or error. In making those risk assessments, the auditor considers internal control relevant to the Diocese’s/Church’s preparation and fair presentation of the financial statements in order to design audit procedures that are appropriate in the circumstances, but not for the purpose of expressing an opinion on the effectiveness of the Diocese/Church’s Internal Control. Opinion In our opinion and to the best of our information and according to the explanations given to us, the financial statements give the information required and give a true and fair view in conformity with the accounting principles generally accepted in India:

(a) in the case of the Balance Sheet, of the state of affairs of the Diocese/Church as at March 31,2017; (b) in the case of the Income and Expenditure Account, of the excess of income over expenditure for the year ended on that date; and (c) in the case of the Receipt and Payment Account, of the receipt and payment for the year ended on that date.
