1 DY7-8 RHP3 DSRIP Contacts and Resources – 8/30/18 Introduction
The shift of the DSRIP program from project-based outcomes to system-wide quality improvement has highlighted the importance of collaboration, especially since providers must select from a standardized menu of quality outcomes and can account for activity that occurs outside of their system. In order to encourage collaboration within the Region and throughout Texas, the Region 3 Anchor Team and System Design, Implementation, and Performance Committee developed this document containing main DSRIP contacts and outcome selection information. The purpose of this resource is to supply Region 3 Providers with information that can connect them to other providers with shared Category C outcomes for technical purposes, as well as to identify potential partners for collaboration.
Information available in this document:
Tables depicting Category C selections by Measure Bundle topic for all provider types. Measures on the Community Mental Health Center (CMHC) and Local Health Department (LHD) menus that align with the Hospital and Physician Practice Measure Bundles were identified by looking at the 3-digit number in the measure ID. For example, measure A1-207 in the Diabetes Measure Bundle aligns with measure L1-207 on the LHD menu.
A list of RHP3 DSRIP main contacts at each Performing Provider organization. This information was collected from Provider presentations given at the RHP3 DSRIP DY7-8 Kickoff in June 2018. Name, position, and email are listed for all RHP3 Providers, while phone numbers are given for most. Additionally, selected bundles or measures are listed for each Provider, as well as the specific measures for CMHC and LHD providers that align with Measure Bundles.

RHP3 Kickoff Provider Presentations – contains the presentations that were given by Region 3 Performing Providers at the RHP3 DSRIP DY7-8 Kickoff event on June 25, 2018. Providers shared details on their system definitions, Category C selections, and interventions/Core Activities used to address selected outcomes.
State Resources
Category C Summary Workbook – lists Category C measure selections in all regions. This file can be used to identify the specific measures a provider has selected to report on within their bundles or on the CMHC or LHD menus and can be sorted by multiple columns including RHP, provider name, and measure ID.
