QUILT SHOW ENTRY FORM

All entry forms, photos, applicable fees and a self-addressed stamped envelope must be received by June 19, 2015 .

Each item entered requires a separate form and a good quality 4x6 color photo of the entry with your name, address, phone number and the title of the entry printed clearly on the back. If there is not enough space on this form, please attach another sheet, do not write on the back of this sheet.

QGGH Members will be on duty during judging and show hours and the premises will be locked during non-show hours. Every effort will be made to insure the safety of your work while in QGGH care, but no responsibility for loss or damage toentries will be assumed by QGGH, its officers, agents or volunteers. You are urged to insure your quilt.
