For each child/young person for whom the Local Authority maintains a statement of Special Educational Needs (SEN) or Education, Health and Care (EHC) plan the education provider should gather advice, hold a meeting and complete this review form on at least an annual basis, recommended six monthly for a child under 5. Reviews mustfocus on the child or young person’s progress towards achieving the objectives/outcomes specified in their statement of SENor EHC plan.

This form is also used when:

  • The transfer of a statement of SEN or Learning Needs (Difficulties) Assessment (LNA/LDA) into an EHC plan is considered
  • The decision has been made to hold either an additional or an early SEN review meeting

Before the review meeting

The education provider must complete Part 1: sections 1 to 8(boxes inpurplemarked ) of this SEN review form in advance of the review meeting.

Ideally three and at leasttwo weeks before the SEN review meeting date. The educational setting must collate and send to all those invited to attend the SEN review meeting copies of:

  • The completed Part 1of the SEN review form
  • The child or young person’s contribution to their SEN review
  • The parental contribution to the SEN review
  • The reports provided by all the professionals invited to the SEN review

Following the review meeting

Once the SEN review meeting has been held, the educational setting mustensure the whole document is fully and clearly completed (boxes in blue).Acopy of the completed SEN review form mustbe sent to everyone invited to the review meeting within two weeks of the meeting. A copy of the completed SEN review form and a copy of all of the collated reports, contributions and supporting evidence mustbe sent to the SEN Assessment and Provision Team within two weeks of the meeting.

2:11Transfer between phasesof education

Only to be completed during the summer one year prior to starting school, summer term of Year 1 for pupils in infant schools, summer term of Year 5 and summer term of Year 10.For Year 5 and Year 10 this needs to be completed even if the preferred placement is the same as the current one. Post-16 placements will be agreed for a fixed period of time. Planning for the end of this fixed period must begin at least one year before the end of the placement is due.

The local authority provides subsidised transport for all young people to access post 16 education. All young people, including those with a statement of SEN or an EHC plan must apply for Post 16 transport.Applications are considered on a case-by-case basis by the Pupil Transport Team.
