Overview This manual, prepared by the Vital Statistics Unit in collaboration with the Health Data NY program from the Division of Information and Statistics within the Office of Quality and Patient Safety, is intended to identify individual registration districts in New York State and their corresponding Gazetteer Code as of November 2015. Registration districts are places in New York State such as , , villages, and facilities that have registrars that record vital events.

These registration districts are used to identify residences and places of occurrence of events reported as part of the vital records registration system of the New York State Department of Health.

The Bureau of Vital Records, within the Division Administration of the New York State Department of Health maintains the list of all vital records registrars within the state and is solely authorized to recognize such forms submitted by any jurisdiction as discussed in Public Health Law 4120.

New York State Department of Health - Bureau of Vital Records All new vital records registration districts must notify and be approved by the Bureau of Vital Records, within the Division Administration of the New York State Department of Health. The creation of a new village does not necessitate the creation of a new registration district. Contact the Bureau of Vital Records for a list of additions.

The Bureau of Vital Records maintains the list of non-municipal registration districts. Contact the Bureau for a list of non-municipal subtractions.

New York State Department of State – Division of Local Government The New York State Department of State keeps track of the status of municipalities in the state. Dissolved villages are removed from updates of the Gazetteer, and noted in a change log. A list of dissolved villages is maintained in the Local Government Handbook. Update Vital Records Systems

New York State Office of Information Technology Systems Once a list of codes that are no longer valid or new codes are established, the revised codes are processed by the NYS Office of Information Technology Systems (OITS) to update Vital Records data systems.

Updates to the Gazetteer The Gazetteer is updated annually by the Vital Statistics Unit within the Bureau of Health Informatics.

How these codes are used The codes in this manual, are used by coders in the Bureau of Vital Records to identify residences and places of occurrence of events reported as part of the vital records registration system of the New York State Department of Health. No other known use of the full four digit codes currently exist in the Department of Health although in previous years other programs did use these codes on their records. The two digit county coding scheme is still used regularly by many programs. How to obtain an electronic copy

This version of the Gazetteer is the first update since November 2007. The updates in this document reflect the removal of registration districts that are no longer recognized by the Bureau of Vital Records.
