CITY OF BAY AREA (BART) TRANSBAY TUBE EMERGENCY Standard Operating Guideline

SCOPE Establish guidelines for safe and effective operations at an incident inside the BART Transbay Tube. These procedures are a guide. Flexibility in dealing with unforeseen circumstances is vital to the success of the operation.

DEFINITIONS BART Operations Control Center (OCC) The OCC (“BART Central”) is where BART train controllers, power, and support personnel are located. power is controlled remotely at this location. Blast Damper Exhaust dampers spaced 1,000’ apart that remove smoke from trackways into the upper gallery, and vent out of either the Oakland or SF vent structures. Blast damper locations are identified in the lower gallery by a ladder and corresponding Blast Damper (BD) sign indicating the damper number at that location.

Blue Light Station Blue Light Stations in the Transbay Tube Lower Gallery contain 1. Emergency telephone direct-connect line to BART Central, 2. Each phone has an ID number that BART Central will verify to identify the instrument location, 3. A Fire Department Yellow Phone Jack.

Cross Passage Door Doors spaced 330’ apart that allow egress from bores into the lower gallery. There are 57 numbered doors, beginning with #1 in SF to #57 at the Oakland vent structure. EBRCS Regional Communications System

Electrical Safe Clearance BART electricians ensure that the remotely controlled circuit breakers are physically disabled by racking them out and tagging them.

Line Anchor Starting point (0.0 milepost) for a particular . Line Anchors start at 3 points in the Wye and increase in different directions out of the Wye. M – Line Anchor begins at the end of 12th St Station Platform and increases as the trackway extends towards Millbrae. Mine Phone Phone system inside the Transbay Tube lower gallery can be used as a Public Address (PA) system to broadcast messages to people in the lower gallery.

MP Mile Post Marker. The spacing of milepost markers in the Transbay Tube is 1/50 mile or 105’. Distance is measured in miles from the Line Anchor in the to that MP location. M 1 and M 2 Tracks The BART rail lines run between 12th St Station Platform in the Oakland Wye to Millbrae. Trains on the M 1 line are traveling from Oakland towards . Trains on the M 2 line are traveling from San Francisco towards Oakland. MW4 Maintenance Way #4. A BART map identifier identifies the Maintenance Way located East of the Maritime Street overpass on 7th Street. This is also where the secondary Oakland Command post is located for the Transbay Tube. Oakland Vent Structure Located inside Trapac Port Terminal Property. Enter through the gate at 1155 Maritime ST, then follow the signs and red line on the ground. The area around the vent structure is gated. After entering this area, crews should ensure the gate is closed and locked. Self-evacuees from incidents could find their way out through the vent structure, and we do not want them leaving the secured area onto an active Port Property. OWS Power Off Status BART OCC indicates that circuit breakers are open on their display board through remotely controlled and monitored equipment. A sensor attached to the third rail is annunciating a power-off status.

Rescue Train A revenue train that has been cleared of patrons for use by Fire Departments to reach an emergency scene. SFFD San Francisco Fire Department Staging An area to be established at the OWS on the platform for collecting available personnel and equipment. Station Agent BART employee assigned as a point of contact in the station for the public and emergency responders. TBT Transbay Tube - The 3.5-mile underwater tube between the ventilation structures in Oakland and San Francisco. It consists of two trackways, an upper exhaust gallery, and a lower utility gallery between the trackways. Train Controller Personnel in OCC that monitor and direct trains traveling within the BART system. Train Operator Employee that operates the BART train. Unified Command Authority structure in which two or more individuals share the Incident Command (IC) function, each already having authority in a different responding agency.

Yellow Phone Closed-circuit phone system for use by fire personnel to communicate to Incident Command Post (ICP) or from blue light station to blue light station within the lower gallery.

TRANSBAY TUBE CONSTRUCTION The Transbay Tube comprises fifty-seven steel and reinforced concrete sections, each averaging 330 feet in length. It is approximately 3.5 miles in length with a continuous walkway on one side of each bore and cross- passage doors at nominal 330-foot intervals. The TBT is connected at each end to a ventilation structure. There are two galleries between the trackways. The upper gallery is an exhaust duct, and the lower gallery stores equipment and enables personnel to travel through the without being inside the trackway. Both galleries run throughout the length of the TBT. The lower gallery in the TBT is supplied with air from a pair of fans, one in each of the vent structures. Both the San Francisco and Oakland vent fans operate in the supply mode. These fans maintain positive pressure in the lower gallery. The water supply to the TBT consists of an 8” main supplied from both the San Francisco and Oakland sides, with 3" outlets in both bores at cross passage doors every 330'. The water main is a wet system with three motor- operated valves. One is at either end of the TBT in the Oakland and San Francisco vent structures, and the third is in the center of the TBT (in the upper gallery). Although this is a wet system, these valves remain CLOSED until there is a fire or other event that requires they be opened, such as service and testing. These valves are remotely opened by BART Central when water is needed; however, they may be operated locally by fire personnel by either switching from remote to local control and turning on the motor to open the valve, or by disengaging the motor and operating the valve manually. The valves are standard 0S&Y's. There are two auxiliary pumps, connected in parallel, in each vent structure with 500 GPM capacities. Whether activated remotely by BART Central or locally by fire personnel, they can be turned on individually or together. When both are turned on, they deliver 1000 GPM. At the lowest point of the TBT, there is over 60 psi of head pressure, and there are no pressure reduction valves in the system. Incident Commanders may consider having hose lines gated back to reduce water flow. Fire Department Connections are located at both the San Francisco and Oakland vent structures to augment the pressure, if needed. Due to the extreme head pressure in the Tube, the only reason for augmenting pressure by the Fire Department(s) would be in the event of a total loss of domestic water supply. In that event, the Fire Department Connections on the San Francisco side of the TBT are adjacent to the bay and could be supplied either by SFFD Fire Boat or by drafting.

POLICY Upon notification by BART of a fire or other emergency in the TBT, a TBT assignment will be transmitted by Fire Dispatch Center (FDC): 6 Engines, 1 Truck, 1 Rescue, 2 Battalion Chiefs (BC), and an Air Van. FDC will transmit an automatic 2nd alarm upon confirmation of the incident. The SFFD will also be dispatched according to their protocols. GUIDELINES INITIAL ASSIGNMENTS AND COMMAND STRUCTURE Assignments are given based on how the apparatus arrives on the scene. It is expected that Company Officers from the responding apparatus will fill BC roles until relieved. OAKLAND INCIDENT COMMANDER (1st arriving BC) First, BC is to respond to the Command room at OWS. The Command room is located at the corner of Chester ST and 7th ST. The incident Channels for TBT incidents in the command room are: BART Provided P25 Portable Radio - Zone: System 1 Channel: 13 (Command Channel with SFFD) EBRCSA radio – Zone: ZZ (Bart Primary) Channel: 1 (Fire 3A) or 3 (Fire 5A)

 Before using Fire 5A, the IC must request approval from Berkeley Fire via Oakland Fire Dispatch.

The IC shall: 1. Gather information about the incident from FDC while en route and through OCC after activating Command Post

2. Activate Command Post and assume Command of the incident. (The keys to ICP are in the Knox box on the gate at the 7th ST side of the fenced area containing the Command room)

3. Establish telephone communications: A. With SFFD ICP on the red phone (direct line to SFFD Command post at ) B. With BART OCC on the green phone (direct line) C. With OFD Communications Center (FDC) on OFD Black Main Line Phone (same four-digit number as any other internal city network phone). OFD Black Main Line Phone can dial external numbers by pressing nine then area code and phone number.

4. Confirm a Unified Command with SFFD via (BART Provided P25 Portable Radio - Zone: System 1 Channel: 13) or Red phone.

5. Determine the following (From BART OCC): A. Location of the incident train: 1) Identify the bore. M-1 is to San Francisco, and M-2 is to Oakland. 2) Determine the location of the lead car

B. Location of the incident on the train (car number and what position that car is in the configuration of the train) C. Number of cars on the involved track D. The passenger load (light, seated, heavy, crush) E. Status of ventilation fans: 1) Is the ventilation system operating? 2) What Blast Dampers (BD) are open? 3) Which direction is smoke venting? (Towards Oakland or SF)

F. Is the 3rd Rail in “Power off” status? G. Are there reports of self-evacuations?

6. Monitor BART Command: BART Provided P25 Portable Radio - Zone: System 1 Channel: 13 (Command Channel with SFFD)

Oakland Branch: EBRCSA radio – Zone: ZZ (Bart Primary) Channel: 1 (Fire 3A) or 3 (Fire 5A)

7. Request BART Liaison and BART PD to report to Oakland OWS Command Room

8. Communicate with Oakland Branch Director EBRCS radio – Zone: ZZ (Bart Primary) Channel: 1 (Fire 3A) or 3 (Fire 5A) A. Identify which companies are traveling on the Rescue Train B. Ascertain estimated time of departure and arrival at the scene C. Draw pertinent information on wall diagram of TBT, chart incident, and recheck ventilation with BART Central

9. Be prepared to request or assign other companies and resources, as needed.

10. Request permission from OCC (via Green Phone) for Rescue Train to proceed into TBT. Give clearance to Oakland Branch Director (OFD Operations -2nd BC) for the Rescue Train to enter TBT.

11. Notify SFFD IC (via Red Phone) that the OFD Rescue Train is entering TBT.

OAKLAND BRANCH DIRECTOR (2nd arriving BC) The second arriving BC is to respond to OWS and assumes Oakland Branch Director. The Oakland Branch Director shall: 1. Contact Staging Manager A. Check the status of companies assigned to the Rescue Train B. Make contact with Station Agent C. Retrieve and communicate any pertinent information about the station, personnel, or equipment that should be known D. Confirm OFD Companies are loading the required equipment onto the rescue train

2. Remove all unauthorized personnel from the rescue train.

3. When fire companies and equipment are ready, position personnel in train cars, establish personnel accountability. A. The 1st car will contain the Branch Director, Aide, and RIC Team B. The 2nd car will contain Rescue Group C. The 3rd car will contain the Fire Attack Group D. The 4th car will contain the Medical Group

4. Request the latest information from IC, including A. Location of lead car of the incident train and closest cross-passage door number B. The number of cars on the train C. The location of the incident on the train (front, middle, rear) D. The status of the ventilation system, including the current direction of ventilation flow E. The number of passengers on board (light, seated, heavy, crush) F. Ensure the 3rd rail is in “Power off status.”

5. When cleared by the IC, have the Train Operator proceed in "Road Manual" into the unaffected bore. Ensure all crews switch channels from Zone: ZZ (Bart Primary) Channel: 1 (Fire 3A) or 3 (Fire 5A) to Zone: ZZ (Bart Primary) Channel: 2 (Fire 3) or 4 (Fire 5) Once inside the bore, the branch director will monitor the Command Channel – Zone: XX (Bart P25) Channel: 13 (Fire 13)

*BRANCH DIRECTOR CANNOT MONITOR COMMAND CHANNEL (Zone: XX (Bart P25) Channel: 13 (Fire 13) UNTIL TRAIN HAS ENTERED THE BORE! * *Ensure all personnel on rescue train switch Zone: ZZ (Bart Primary) Channel: 2 (Fire 3) or 4 (Fire 5) when entering the bore* 6. As rescue train approaches incident, Oakland Branch Director shall: A. Request updates on conditions from Oakland IC and provide any pertinent updates via the Command Channel – Zone: XX (Bart P25) Channel: 13 (Fire 13)

7. Stop train at cross passages before the reported location to check the gallery for smoke conditions

*ONLY THE BRANCH DIRECTOR OR DESIGNEE GETS OFF THE TRAIN TO CHECK THE GALLERY*

Upon arrival at the Emergency scene, Oakland Branch Director shall: 1. Establish communication with the Oakland IC: via radio Zone: XX (Bart P25) Channel: 13 (Fire 13), by yellow fire phones or by accessing jacks with single prong yellow handphones.

2. Identify the exact MP location and closest cross-passage door to the incident and notify the Oakland IC.

3. Verify power to Third Rail is powered off on incident track through the Oakland IC.

4. Check bore and gallery for evacuees.

5. Communicate with SFFD Branch Director (Face-to-face preferred). Identify SFFD location and coordinate operational activities.

6. Establish an Operation location. A. If possible, coordinate with SFFD and set up in a common location. B. Rescue and firefighting operations should be coordinated from this location.

7. Identify the location of RIC. Consider co-location of RIC with SFFD.

8. Check the effectiveness of ventilation and relay pertinent information to Oakland IC.

9. There should be two (2) trains at the incident location: One (1) OFD and one (1) SFFD. One (1) BART train must remain at the incident scene for escape or transportation of fire department personnel.

10. Request any additional personnel or equipment as needed.

11. Contact BART Train Operator (TO) of the incident train. Obtain information about the incident and relay information to the Oakland IC.

STAGING (1st Engine) Officer – Establish Oakland Command . Get keys from Knox box . Access Fire Equipment Room . Make contact with Station Agent . Report to Command Room, transfer Command and be an aide to the IC

Engineer – Staging Manager (STAM) . Collect T-cards from apparatus entering station (ensure accuracy of T-Cards) . Assign incident positions/assignments to arriving units as per SOG . Track all resources on pre-designated T-Card rack . Prepare for the potential of successive alarms and victims brought to OWS

FF/PM – Oakland Branch Director Aide . Proceed to the platform with an airway bag, trauma bag, EKG, and MCI fanny packs . Upon the arrival of the rescue train, ensure the train is empty and all civilians have been evacuated . Coordinate with the Rescue Train Operator to get doors open and prepare train to receive crews . Assume Oakland Branch Director Aide (must have all PPE, SCBA [one-hour bottle], radio, flashlight, and writing utensils [clipboard, paper, and pens]) FF – Systems Officer . Take control of elevators and escalators (must have the second set of keys from Knox Box) . Ensure all civilians have been evacuated off the platform . Reverse the escalators and shuttle crews/equipment to the platform . Maintain control of elevators and escalators throughout the event

FIRE ATTACK GROUP (2nd Engine and 3rd Engine) Travels in 3rd Car 1. The senior officer will be known as the Fire Attack Group Supervisor.

2. Fire extinguishment is the primary objective of the Fire Attack Group. A. The Fire Attack Group will work in coordination with the Rescue Group.

3. Access to the incident will be through the gallery, into the incident bore.

4. Fire Attack Group Supervisor must gain the approval of Oakland Branch Director before entering the incident bore.

5. Connect 2 ½” Wye with two (2) attack lines to the wet standpipe.

6. Upon entering the incident bore, passengers' rescue may supersede all other Fire Attack Group responsibilities.

7. Fire Attack Group Supervisor must maintain the integrity of the group at all times.

8. Fire Attack Group Supervisor keeps Oakland Branch informed of any change in conditions or geographical change.

RESCUE GROUP (1st Truck and Rescue 1) Travels in 2nd Car 1. The Officer of Rescue 1 will be the Rescue Group Supervisor.

2. Rescue of passengers and BART Personnel is the primary objective of the Rescue Group. A. The Rescue Group will work in cooperation with the Fire Attack Group.

3. Access to the incident will be through the lower gallery, into the incident bore.

4. Rescue Group Supervisor must gain the Oakland Branch Director's approval before entering the incident bore.

5. Primary search and assessment of the passenger load should be accomplished immediately upon entering the incident bore. This information shall be communicated to the Oakland Branch Director to determine the need for further resources.

6. All passengers and BART employees that are ambulatory shall be directed to the gallery with advisement to the Medical Group Supervisor A. If necessary, move passengers through the bore, away from the smoke, until they can travel through the gallery to the Rescue Train.

7. After the primary search is completed, a secondary search shall be conducted, and results reported to the Oakland Branch Director.

8. Rescue Group Supervisor must maintain the integrity of the group at all times.

9. Rescue Group Supervisor shall keep Oakland Branch informed of any change in conditions or geographical change.

RIC (4th Engine) Travels in 1st car 1. The officer will be the RIC Group Supervisor.

2. Establishing an effective and adequate RIC response effort is the RIC Group Supervisor's primary objective.

3. RIC will monitor the operational activity on the appropriate TAC channel and place the team, with the proper equipment, in a position to respond if needed. (Optimal location is in the gallery at the door closest to the incident. However, gallery configuration and incident may dictate RIC be staged in a different location.) Attempt to co-locate with SFFD RIC, which may prove beneficial.

4. Notify Oakland Branch Director of the RIC Group location.

MEDICAL GROUP (5th Engine) Travels in 4th car 1. The officer will be the Medical Group Supervisor.

2. Prioritization of victim removal will be the primary objective of the Medical Group Supervisor.

3. Medical Group Supervisor will coordinate Triage and Treatment at the incident scene in the rescue train or gallery The group will prepare the fourth car for the potential of treating multiple casualties. (Consider using additional cars to separate patients based on triage severity.)

RECON GROUP (6th Engine) Checks in at OWS then reports to Oakland Vent Structure Officer / FFPM / FF – Get keys from Knox Box at Vent Structure gate.  Access the vent structure on the “delta” side door  Report ventilation conditions to Oakland IC (Damper control board)  Access the gray cabinet and retrieve:

- Yellow phones, Fire Cart instructions

 Proceed into the lower gallery and board fire carts  Proceed to the incident via lower gallery reporting to Oakland Branch Director on

Zone: ZZ (BART Primary) Channel: 2 (Fire 3) or Channel 4 (Fire 5)

While en route, update Oakland Branch Director regarding:

- Smoke conditions in the gallery - If any victims have self-evacuated into the gallery - Any additional resources needed to respond to vent structure to assist with victims self- evacuating Engineer – Secure water supply from a hydrant  Connect two (2) leads to the FDC (“Charlie” side of vent structure) *DO NOT CHARGE* Wait for order to charge system from Oakland IC.  Ensure the gate surrounding the vent structure is secured.  Monitor radio traffic on Zone: ZZ (BART Primary) Channel: 1 (Fire 3A) or Channel 3 (Fire 5A) for updates and direction.

AIR VAN – Respond to West Oakland Station 1. Officer – Assume Division OWS

2. Assist crews moving equipment to rescue train

3. Prepare station to receive patients

4. Coordinate with ambulance company and BART Police

5. Position Air Van to refill bottles from the incident

6. Pre-position bottles on the platform for the exchange

If the number of cars in the rescue train is less than four (4) cars, IC shall ensure a secondary rescue train is sent to OWS with enough cars to accommodate the incident train's passenger load. Getting crews to the incident is the main priority.

COMMUNICATIONS  OFD units will enter the TBT and report directly to the Oakland Branch Director. All units will be on Zone: ZZ (BART Primary) Channel: 2 (Fire 3). A second TAC channel may be utilized if necessary: Zone ZZ (BART Primary) Channel: 4 (Fire 5).

 SFFD units will enter the TBT and report directly to the San Francisco Branch Director. San Francisco Operations will be on Zone: XX (BART P-25) Channel: 1 (Fire 1). A second TAC channel may be utilized if necessary: Zone XX (BART P-25) Channel: 2 (Fire 2).

 The Command Channel for the incident will be Zone XX (BART P-25) Channel: 13 (Fire 13) (Underground ONLY)

 Oakland IC and Oakland Branch Director shall communicate on Zone: XX (BART P-25) Channel: 13 (Fire 13) or the Yellow phone. The Oakland IC will monitor BART P-25 portable – Zone: System 1 Channel: 13 and EBRCS portable Zone: ZZ Channel: 1 (Fire 3A) and Channel: 3 (Fire 5A)

 Before using Fire 5A, the IC must request approval from Berkeley Fire via Oakland Fire Dispatch (Though Oakland Fire’s secondary Tac Channel usually is channel 4, when EBRCS zone ZZ was established, it was meant for each agency in EBRCS to have one above ground EBRCS channel and one BART P25 below-grade Tactical channel patched. The goal is to keep personnel from having to transition zones in the incident.)

 Incident Commanders should consider using “8CAFIRE1” as a backup if needed for long-range communications in the underground due to an EBRCSA failure

 Incident Commanders should consider the use of Simplex Tactical channels at the incident site if established Tactical Channels become overloaded

CONSIDERATIONS The SOG outlines a fire incident. The structure and assignments should be modified to fit the needs of the incident. Situations that may warrant additional resources or a change in tactics include:  Location of incident (Incident closer to SF/OAK, or incident in a section of the tunnel between the portal entrance and Oakland Vent Structure HazMat incident (Mutual Aid HazMat should be considered early since E3 and T3 are first due companies for a TBT incident)

 Train derailment

 Construction / maintenance accident

 Self-evacuation of passengers (If OCC gets reports of people in the trackway, third rail power will be cut and could result in rescue trains stopping well before the incident)  Self-evacuated passengers exiting at Oakland vent structure or tunnel portal at MW4

 Potential for many victims requiring removal from rescue train at OWS platform to the street level and transport from OWS

 Use of XAL Mutual Aid Plan

 For large scale incidents, Incident Commanders should consider assigning a Liaison reporting to BART Central
