This used a telehealth device similar to a smartphone to help patient’s record daily medical readings to help manage their Chronic Obstructive Pulmonary Disease (COPD) or heart failure at home. BCH evaluated this scheme, however the organisation no longer exists, and therefore this evaluation cannot be requested.
