1. BACKGROUND

Afghanistan’s General Directorate of Pharmaceutical Affairs (GDPA) has been making good progress toward advocating and advertising medicine safety and pharmacovigilance at the national level. These activities are technically and financially supported by the Strengthening Pharmaceutical Systems (SPS) project, which is funded by the US Agency for International Development (USAID). The four-day pharmacovigilance training and one-day orientation workshop for staff from select Kabul hospitals, Ministry of Public Health (MoPH) staff, and pharmaceutical stakeholders were the basis for this advocacy and information sharing with health staff on the importance of medicine safety.

In response to the positive feedback from participants in the aforementioned gatherings, the GDPA started to expand the program to the health service delivery level by sharing this information with hospital staff, beginning with three pilot hospitals. The Medicine Safety Committee (MSC) assigned a subcommittee—which included representatives of the MSC, Avicenna Pharmaceutical Institute (API), General Directorate of Curative Medicine (GDCM), and SPS—to facilitate pharmacovigilance orientation workshops at these three hospitals.

The hospital-level orientation workshops began at Istiqlal Hospital (December 3, 2014), and continued at Infectious Disease Hospital (December 17, 2014) and then Mehtarlam Provincial Hospital (January 12–13, 2015). These three hospitals were previously selected as pilot sites for the feasibility of pharmacovigilance interventions. Technical staff from MoPH, GDPA, MSC, and SPS facilitated the pharmacovigilance orientation workshops for hospital clinical staff that included physicians, pharmacists, nurses, and midwives.

Objectives

The aim of conducting of these orientation workshops was to orient the clinical staff of Istiqlal Hospital, Infectious Disease Hospital, and Mehtarlam Provincial Hospital on medicine safety and pharmacovigilance through GDPA efforts to implement this program in these hospitals.

The main topic areas follow—

Workshop Structure

The orientation workshop at Istiqlal Hospital opened with a supportive speech from Hospital Director Dr. Ali Eshan. Dr. Eshan noted that pharmacovigilance is a new concept in Afghanistan and expressed thanks to the GDPA and its technical partner SPS for selecting Istiqlal Hospital as a pilot site for this intervention. He assured any kind of support for running this program in Istiqlal Hospital, and encouraged the health staff to report properly and in a timely manner.

The orientation workshop in Infectious Disease Hospital opened with a supportive speech from Hospital Director Dr. Abdul Rahman Akbari. Dr. Akbari mentioned that the imported medicines in Afghanistan enter in various ways; they may be imported through legal pathways or illegally (smuggling), so it is very important to track the quality of such medicines. One of the ways that will assure patient safety is pharmacovigilance. Noting that pharmacovigilance is a new concept in Afghanistan, Dr. Akbari recommended that all staff carefully listen to the presentations and ask facilitators for clarification when necessary. He also thanked GDPA and SPS for selecting Infectious Disease Hospital as a pilot site for this intervention. He assured any kind of support for running of this program in Infectious Disease Hospital, and encouraged the health staff to report properly and in a timely manner.

Due to the high number of clinical staff and their interest in participating in this workshop, the orientation for Mehtarlam Provincial Hospital was conducted in two batches. This allowed staff (divided into two groups) to participate and also to carry out their routine tasks.

Laghman Public Health Director Dr. Abdul Latif Qayumi opened the Mehtarlam Provincial Hospital orientations with a supportive speech. Dr. Qayumi noted that imported medicines in Afghanistan are often of poor quality, and the one department of quality control at the MoPH level cannot check all the imported medicines. For this reason, he continued, health providers must be vigilant and monitor medical patient treatments. He recommended that all hospital staff carefully listen to the presentations and ask for clarifications from the facilitators when needed, because pharmacovigilance is a new concept in Afghanistan. Dr. Qayumi expressed thanks to GDPA and SPS for selecting Mehtarlam Provincial Hospital as a pilot site for this intervention. He assured any kind of support for running this program in Mehtarlam Provincial Hospital and encouraged the health staff to report properly and in a timely manner.

The orientations continued in all three selected hospitals with technical presentations from facilitators, and question-and-answer sessions.

A grand total of 257 health professionals (190 males and 67 females) participated in workshops at the three hospitals (figure 1): 87 health staff (58 male and 29 female) from Istiqlal Hospital, 89 health staff (72 male and 17 female) from Infectious Disease Hospital, and 81 health staff (60 male and 21 female) from Mehtarlam Provincial Hospital. Participants represented a wide range of hospital functions and units: male and female outpatient department (OPD)/emergency, internal medicine, surgery, plastic surgery and burn, obstetrics and gynecology, newborn and nutrition, anesthesia, pediatrics, male and female infectious wards, acute gastroenteritis (AGE) ward, and pharmacy and nursing departments.

At the end of each orientation, each respective hospital medical director closed the workshop; at Mehtarlam Provincial Hospital, the Laghman public health deputy spoke as well. Each speaker offered a directive to participants and acknowledged GDPA for implementing this new concept with proper guarantees for more support in this regard.

3. STRENGTH POINTS

The orientation workshops went smoothly and were well received by participants; some key strengths of the process include—

 Close coordination among facilitators from GDPA, Faculty of Pharmacy, and SPS  On-time logistical and financial management by SPS administrative team  Facilitators used different methods for interactions among participants  Active participation of attendees with clear guidance from facilitators

6. CONCLUSION

Pharmacovigilance orientation workshops were successfully conducted in all three selected hospitals. Participants shared their experiences and knowledge of key issues and are better prepared to implement and support pharmacovigilance activities in their hospitals.

7. RECOMMENDATIONS

The GDPA thanked Istiqlal, Infectious Disease, and Mehtarlam Provincial hospitals for conducting this intervention in their facilities, and recommended that all staff work to report ADRs in a timely manner, distinguish ADRs from other medicine-related problems, and also offer their support during supervision from GDPA and SPS staff.
