A 6-year-old boy, who was diagnosed as having neph!otic syndrome, was treated with oral administration of prednisolone and cyclophosphamide for 4 years, receiving a total dose of 49.5 g cyclophosphamide. At age 27, a gross hematuria with bloody clots appeared and he presented with postrenal renal failure. He underwent a radical cystourethrectomy and ileal conduit for stage a pT3a pNO MO transitional cell carcinoma of the bladder. He was not given any adjuvant treatments because of his renal insufficiency, and he died from the disease 14 months after rad­ ical surgery. INTRODUCTION Urothelial cancers arise in the urothelium of the renal collecting systems, the ureters, bladder, prostate and urethra. The bladder is the most frequent site of disease accounting in Canada for 5.8% of new cancers and 3.3% of cancer deaths in males, and 2.1% of new cancers and 1.5% of cancer deaths in females (Canadian Cancer Statistics 2008). This difference in incidence is attributed in part to the smoking patterns seen between Canadian men and women. The most common histology is transitional cell carcinoma (TCC). An important feature of transitional cell malignancy is the propensity to behave as a field defect with multifocal disease in primary and recurrent presentations.

Seven chidren (five bladder base/prostate, one vagna and one pelvic side wall tumours) had receivd irradiation to the pehlis with extenal beam alone, brachytherapy or both. Al seven of these patients had markedly reduced functional badder capacity (11-48% of mean expected value for age) and abnormal voiding patters, though bladder complance was not reduced and bladder emptying was almost complete in five cases. Four of these chidren also had upper tract dilatation and two required reconstructive bladder surgery because of severe bilateral hydronephrosis. By contrast, each of four childr treated without radiotherapy had a normal functional bladder capacity and a normal voiding pattern. All survivors of pelvic rhabdomyosarcoma, especially those who have received radiotherapy, should be carefully monitored for dysfunction of both lower and upper urinary tracts.

Summary In the early fifties, total ischiectomy was in vogue as a procedure in the surgical treatment of ischial pressure sores. The immediate results by various authors were impressive. One of us, however (A.E.C.), reported the later development of urethral fistulae and the high incidence of perineal urethral diverticulae. The procedure has been abandoned and few surgeons recommend less radical ischiectomy supplemented by muscle transplantation. Introduction The authors present 10 cases of perineal urethral fistulae, consequent to total ischiectomies. The clinical data are presented in Table l. This was supplemented by suprapubic diversion in most of them and in one patient with bladder neck occlusion. One patient healed without diversion and one healed spontaneously. Another required an ileal conduit with cysto-prostato-urethrectomy. Two developed carcinomas, one in the urethra and one in the bladder and both ended fatally. It is the opinion of the authors to deplore total ischiectomy in the surgery of ischial pressure sores, because of complete anatomical distortion of the perineum and exposure of the urethra to trauma, diverticulation and fistulisation.

What is a Cystectomy? Cystectomy is sometimes referred to as a radical cystectomy, a cystoprostatectomy or a cystourethrectomy. Cystectomy is the medical term for the removal of the bladder. Males In males, a cystectomy and a radical cystectomy involve the removal of the entire bladder and the prostate gland (see diagram below). A cystourethrectomy involves the removal of the bladder, prostate and urethra (water pipe). Male Pelvis Females In females, a cystectomy involves removal of the entire bladder and urethra (water pipe), the ovaries, uterus (womb) and the upper part of the vagina. Female Pelvis What is a Neo (new) Bladder? This booklet is for people who have been advised by their urologist that they need to have a cystectomy (removal of the bladder) and who may be suitable to have a new bladder reconstructed out of bowel tissue. The advantage of this operation is that, in the long term, you will not need to wear a bag on your abdomen to collect urine. Instead the neo (new) bladder is connected onto the urethra (the water pipe) and urine is passed naturally. Some people may need to use a small disposable catheter once or twice a day to empty the neo bladder. The operation has several names. This type of bladder reconstruction is sometimes called an Orthoptic Reconstruction or Neo bladder. This operation is not possible for everyone who has a cystectomy, especially if you have previously had radiotherapy to the pelvis or if you have a history of bowel abnormalities.
