1. JOB DESCRIPTION

This document contains information about the above post. It also forms the job description and proposed job plan for this post. As required by Health Circular HC (90)16, the successful candidate will complete and agree a job plan with the lead clinician and general manager. It will be ratified by the Medical Director after taking up the appointment, will be reviewed at 3 months and thereafter appointment and thereafter on a yearly basis.

THE POST

This is a new post to allow the opportunity to combine providing a service in diabetes and endocrinology and general internal medicine in a new, state-of-the- art hospital and purpose built community diabetes centre. You will join our existing teams of Consultant Diabetologists and Endocrinologists and other Consultant Physicians at North West Anglia NHS Foundation Trust. The appointment is for a total of 10 programmed activities with an approximate split of 8.5 direct clinical care programmed activities (CPA) of Diabetes and Endocrinology and General Medicine and 1.5 PA’s for supporting professional activity (SPA).

The appointee will work alongside Dr S Oyibo, Dr S Sagi and Dr Rajkanna who both work full time, and Dr Roland and Dr A Swamy who both work part time in the Department of Diabetes and Endocrinology and General Medicine, at City Hospital site. Following the recent merger with Hinchingbrooke Hospital, we also work alongside Dr Mathews Krishnan and Dr Krishnan Khan who are both based at the Hinchingbrooke site.

The post holder is managerially accountable to the Clinical Lead for Diabetes and Endocrinology, Clinical Director and General Manager, Emergency, Critical Care and Medicine Foundation Clinical Business Unit and clinically accountable to the Medical Director.

Applicants must be members of the Royal College of Physicians or have equivalent experience and training and must be fully registered with a license to practice with the General Medical Council with specialist registration in Diabetes and Endocrinology and General Internal Medicine or within 6 months of CCT for these specialties.

The Peterborough Diabetes Service There are approximately 13,000 patients with diabetes in Peterborough and adjoining PCTs which form part of our traditional catchment population. The Diabetes Care Team (currently comprising 3 Consultant Diabetologists, 1 Specialist Registrar, 7 Community Diabetes Specialist Nurses, 3 Specialist Dietitians, 3 Specialist Podiatrists and 3 Diabetes Care Technicians) is based at a purpose built Diabetes Care Centre the Peterborough Healthy Living Centre. There are also 3 in patient Diabetes Specialist Nurses. The Peterborough Model of Care is of a community based service supported by the Diabetes Specialist Team Professionals. The Diabetes Care Technicians (a Peterborough innovation) provide a comprehensive annual diabetic review service in the community. The Team provide group structured education programmes for both Type 1 and Type 2 diabetic patients. Peterborough is a founder member of the DESMOND project and is part of the Diabetes Education Network. We run a continuous subcutaneous insulin infusion service for both adult and paediatric patients. We provide a number of Consultant led specialty clinics in Pregnancy, Diabetic Foot, Young Persons, Transitional, Insulin Pump and Older People.

Endocrinology Currently there is a weekly general endocrinology clinic in which the appointee will participate. There is also monthly joint thyroid clinic in the ENT department. Thyroid ultrasound and isotope scanning are also available on site. In addition to thyroid surgery there is a service for minimally-invasive parathyroid surgery. The radio-iodine therapy service for non-malignant thyroid conditions is supervised locally by Dr Oyibo. With the exception of inferior petrosal sinus sampling there is a full range of endocrine investigations available including a comprehensive endocrine biochemical service, CT, MRI ultrasound and DEXA scanning. Patients requiring transphenoidal hypophysectomy are referred to the neurosurgical unit (Pituitary MDT) at Addenbrooke’s Hospital.

JOB TITLE Consultant Physician in Diabetes & Endocrinology and General Internal Medicine based at the North West Anglia NHS Trust (with outreach at the Community Diabetes – Healthy Living Centre)

JOB PURPOSE  To lead, maintain and develop Diabetes & Endocrine services for the North West Anglia NHS Foundation Trust.  To contribute to an acute general medical service of optimum quality within the Trust. And shared responsibility for care of our dedicated medical in-patients.  To provide specialist outpatient Diabetes & Endocrine services and referral services in this sector.

Page 3 of 13  To promote and develop multidisciplinary teamwork across all the Trust.  To integrate with community services including GPs with interest in Diabetes and Care Transformation programmes.

JOB ROLE  To share in the collective responsibility for the provision of acute medical services at the North West Anglia NHS Trust and to provide specialist inpatient and outpatient Diabetes & Endocrine services.  The post holder will be part of a team of Consultant in Diabetes & Endocrinology based at the Trust and the diabetes teams the community Diabetes (Healthy Living) Centre roughly 2 miles from the hospital.  The post-holder will share the responsibility for provision of care for in- patients on the Diabetes/General Medical ward.  To contribute to the development of excellence in clinical practice through research, audit and participation in clinical and social care governance.

JOB LOCATION  The post holder will be based at the North West Anglia NHS Foundation Trust. All on-call and inpatient service delivery will be based at this site. There may be some cross site responsibility from time to time.  The post holder, along with colleagues, will also assist in the delivery of a weekly outpatient service at the Healthy Living Centre.

KEY CLINICAL TASKS  The post holder, along with colleagues, will provide specialist led inpatient and outpatient Diabetes & Endocrine services.  The post holder shall with colleagues maintain and further develop the Diabetes & Endocrine services at the North West Anglia NHS Foundation Trust.  The post holder’s prime clinical responsibility will be accountability for the continuous care of his/her in-patients and outpatients, and as part of this responsibility, a share of the out-of-hours general medical on -call rota arrangements at the North West Anglia NHS Foundation Trust.  The post holder will work as an integral part of the Multidisciplinary Trust wide Diabetes network, and attend Clinical Governance and Multidisciplinary Team meetings at the Healthy Living Centre.  The post holder must ensure appropriate standards of care for patients and appropriate standards of education and training for medical staff in training.

PROPOSED JOB PLAN 1. Two clinics in general endocrinology at Peterborough City Hospital per week 2. One specialty diabetes or endocrine clinic (to be arranged) 3. Three whiteboard rounds and ward rounds for our dedicated in-patient ward. Page 4 of 13 4. The appointee will provide leave cover for the Consultant in Diabetes and Endocrinology who normally supervises the team’s inpatients. This cover will include providing daily senior input for inpatients on the diabetes and endocrinology half ward and any outliers under the team, conducting full ward rounds twice a week, supervising the trainee doctors and providing the service for diabetes and endocrinology advice for referrals from ED, Emergency Short Stay, Woman’s Health and other areas. 5. It is expected that there will be the need from time to time to make changes to the job plan. It is expected that the successful candidate will be prepared to negotiate such changes for the benefit of clinical service delivery and the department as a whole. In particular it is anticipated that with new consultant appointments and retirements the proportion of diabetes and endocrinology will increase and the post will eventually become full time diabetes and endocrinology.

OTHER DUTIES The successful candidate will be required to undertake managerial duties associated with the care of their patients and the running of their Clinical Department

RESEARCH We have an established record not only in participation in major multi-centre clinical trials but also regular publications and local educational projects (1 to 3 peer reviewed papers or abstracts each year over the last 10 years).

TEACHING AND AUDIT

The department has a long standing commitment to undergraduate medical education, taking students regularly from Leicester and Cambridge Universities, and sometimes from other areas. The post holder will be expected to take part in this teaching. The department also has an active postgraduate programme

Page 5 of 13 particularly aimed at the FY2s and the MRCP, and the post holder will be expected to take part in this teaching. The post holder would be expected to share in the training of the Specialist Registrars with the existing consultants.

Continuing medical education and professional development is expected and encouraged by provision of appropriate facilities and study leave.

Peterborough Hospital is one of eight hospitals within the Norfolk, Suffolk and Health Authority designated as a Postgraduate Medical Education Centre. An active programme of postgraduate education is provided on site.

The physicians meet on Thursday morning and review clinical cases, the delivery of care, medical literature and audit. A rolling half day programme of audit and education takes place once a month in which routine clinical commitments are cancelled. A well established clinical audit department provides assistance for these sessions.

MENTORSHIP The post holder will be given the support of an experienced consultant to provide mentorship, with regular meetings. This can be another consultant in the same department or a consultant in a different department depending on the post-holder’s preference.

GENERAL INFORMATION ON PETERBOROUGH AND SURROUNDING AREAS

Pleasant countryside surrounds Peterborough, punctuated by picturesque villages and bustling market towns. At the north of Cambridgeshire and close to the counties of , Rutland, and Norfolk, the surrounding area is varied and rich in character.

Stamford is generally acknowledged as one of the most historic and beautiful stone-built towns in . The town’s intricate street patterns retain their medieval design with narrow passageways and cobbled streets housing a wealth of architecturally important buildings and interesting shops.

Burghley House – famous for its horse trials – lies on the outskirts of the town and both the house and gardens are open to the public throughout the summer. Bourne and possess some fine period buildings. Bourne is a small pleasant market town and the birthplace of William Cecil, Lord Burghley and creator of .

The Deepings is an historic group of villages comprising , and Deeping Gate. Market Deeping, with its wide streets, old stone-walled houses and inns, reflects its importance as a major stopping place on the London to Lincoln stage route. Oundle in Northamptonshire, famous for its school and historic associations, is another stone built town with a wealth of local interest.

Rutland is England’s smallest mainland county and is populated by numerous delightful small villages. There is also Rutland water, a man-made reservoir, which supports numerous activities both water based and non-water based with a sailing club, water sport facility, cycling routes and is famous nationally for its bird watching.

Peterborough is proud of its excellent education facilities – over 30 new schools and major extensions have opened since 1970. The city’s secondary schools have an average pupil-to-teacher ratio of less than 17 to 1. In the independent sector there are five boys’ schools (including Uppingham School), three girls’ schools and four mixed (including Oakham, Stamford and Oundle Schools) – all within daily travel of Peterborough and all taking boarders also.

There are excellent transport links, with the A1 providing a fast route to London and the North and direct train services to London with journey time of 50 minutes.

TRUST MANAGEMENT STRUCTURE

North West Anglia NHS Foundation Trust operates a Clinical Directorates model.

Within Clinical Directorates, multi-disciplinary senior management teams have been established. The role of these teams is to actively contribute to the strategic and policy decision making process within the Trust. The membership of these teams varies between Clinical Directorates, but each is chaired by the Clinical Director or relevant lead nurse and overseen by the General Manager. In addition, members of the finance and business and human resources directorates contribute to the work of these teams.

FOUNDATION TRUST STATUS

North West Anglia NHS Foundation Trust was awarded a three-star status in 2003, 2002 and in 2001. The trust became one of the first trusts in the country to gain Foundation Trust status on the 1st April 2004.

3. MAIN TERMS AND CONDITIONS OF SERVICE

The post is covered by the Terms and Conditions of Service under the new consultant contract 2003. The National Health Service Pension scheme is available.

There is also a Local Clinical Excellence Award scheme, North West NHS Foundation Trust arrangements are consistent with current NHS practice (which are details in AL (MD) 6/95 and EL (95)109). The Peterborough Hospitals NHS Trust will honour the Clinical Excellence Awards, Discretionary points or Distinction awards already held by an appointee.

The successful candidate will be required to complete a health statement and the Trust may require an officer to pass a medical examination as a condition of appointment.

The post-holder will be required to comply with the Immunisation Policy as a condition of appointment. A copy of this policy is available on request.

Because of the nature of the work of this post it is exempt from the provisions of the Rehabilitation of Offenders Act, 1974. Applicants are, therefore, not entitled to withhold information about convictions and, in the event of employment, any failure to disclose such convictions could result in dismissal or disciplinary action by North West Anglia NHS Foundation Trust. The successful candidate will be expected to devote substantially the whole of his/her professional time to this appointment and to give it priority on all occasions (subject to the consultant’s ethical obligations to all his patients when emergencies arise)

The successful candidate will be required to live within reasonable distance (normally within half an hour’s travelling time) from the main unit acceptable to North West Anglia NHS Foundation Trust.

The private residence must be maintained in contact with the public telephone service. Assistance will be given with the cost of installation and rental charges.

Newly appointed Consultants may be eligible for reimbursement with the whole or part of the cost of their removal expenses, provided that the removal is necessary to comply with the Trust’s requirements concerning the place of residence.

The person appointed will be indemnified by the Trust for all duties undertaken as part of his/her contracts of employment. He/she is encouraged to take out adequate defence cover as appropriate to cover him/her for any work which does not fall within the scope of the Trust's indemnity scheme.

Annual Leave

The post-holder will be entitled to 32 days (34 days if 7 years or more seniority as a Consultant) annual leave per year.

Study Leave

A maximum of 30 days study leave with pay and expenses can be taken over a three year period, or ten days each year.

Notice Period

The employment is subject to three months notice on either side, subject to schedule 19, terms and conditions of service consultant contract 2003

Salary

The starting salary of the appointment (exclusive of any distinction and meritorious service award payable to you) will be the appropriate point on the Consultant pay scale ranging from £ 76,001 - £102,465 (basic rates of pay 2016)
