While cardiac arrhythmias vary widely in their clinical presentation, they possess shared complex electrophysiologic properties at cellular level that have not been fully studied. Over the last decade, our current understanding of the functional roles of non-coding RNAs have progressively increased. microRNAs represent the most studied type of small ncRNAs and it has been demonstrated that miRNAs play essential roles in multiple biological contexts, including normal development and diseases. In this review, we provide a comprehensive analysis of the functional contribution of non-coding RNAs, primarily microRNAs, to the normal configuration of the cardiac action potential, as well as their association to distinct types of arrhythmogenic cardiac diseases. REQUIRED COMPONENTS OF THE MEDICAL RECORD The medical record shall include information to support the patient's diagnosis and condition, justify the patient's care, treatment and services, and document the course and result of the patient's care, and services to promote continuity of care among providers. The components may consist of the following: identification data, history and physical examination, consultations, clinical laboratory findings, radiology reports, procedure and anesthesia consents, medical or surgical treatment, operative report, pathological findings, progress notes, final diagnoses, condition on discharge, autopsy report when performed, other pertinent information and discharge summary. 2. ADMISSION HISTORY AND PHYSICAL EXAMINATION FOR HOSPITAL CARE Please refer to CaroMont Regional Medical Center Medical Staff Bylaws, Section 12.E. A. The history and physical examination (H&P), when required, shall be performed and recorded by a physician, dentist, podiatrist, or privileged practitioner who has an active NC license and has been granted privileges by the hospital. Memorandum Summary • The attached interpretive guidelines correspond to the regulatory changes published November 27, 2006 amending Hospital Conditions of Participation (CoPs) pertaining to requirements for history and physical examinations; authentication of verbal orders; securing medications; and post-anesthesia evaluations. • The Tag numbers have been revised to correspond to the Tag numbers reflected in the December, 2007 ASPEN release. State survey agencies should not use the temporary Tag numbers identified in the S&C letter of February 23, 2007 (S&C-07-13). • The interpretive guidelines also include newly-adopted additional changes that were incorporated into the Calendar Year 2008 Outpatient Prospective Payment System (OPPS) regulation. They are effective January 1, 2008. The interpretive guidelines attached to this memorandum reflect the regulatory changes that were published on November 27, 2006 amending the Hospital Conditions of Participation (CoPs) pertaining to requirements for history and physical examinations; authentication of verbal orders; securing medications; and post-anesthesia evaluations (CMS-3122F, 72 FR 68672). These changes were discussed in an S&C letter published on January 26, 2007 (S&C-07-13; revised 2/23/07). -
EHR Go Guide: the Notes Tab
EHR Go Guide: The Notes Tab Introduction The Notes tab in the EHR contains narrative information about a patient’s current and past medical history. It is where all members of the health care team communicate about the patient during a hospital visit or while receiving outpatient care. Notes are documented on regular intervals about clinical events related to the diagnoses and treatment of the patient. There are many types of notes in the EHR including: admission notes, assessments, SOAP notes, exams, reports, screening tools, progress notes, evaluations, and more. Notes are sometimes referred to as “Flow Sheets.” Most note options are templated, meaning specific fields or questions are built in to the note for the clinician to answer, while other notes allow for free-text narrative entry. Case patients in EHR Go have existing notes in their chart. You may review these notes to gather detailed information about the patient’s condition. It is often helpful to review the patient’s notes first, to learn about the patient, before reviewing the information in the other EHR tabs. In some activities, you’ll need to write a new note. This guide explains how to access and review existing notes, edit notes, and create new notes.
