Railway accident investigation report Railway operator : Tarumi Railway Company Accident type : Level crossing accident Date and time : About 11:09, June 10, 2016 Location : Motosu-Minami level crossing, class three level crossing without automatic barrier machine, with road warning device, located at around 15,885 m from the origin at Ogaki station, between Itonuki station and Motosu station, single track, Tarumi Line, Motosu City, Gifu Prefecture SUMMARY On Friday, June 10, 2016, while the outbound 17 train, started from Ogaki station bound for Tarumi station, Tarumi Line of Tarumi Railway Company, was running between Itonuki station and Motosu station, the train driver noticed the light motor vehicle entering into Motosu-Minami level crossing, class 3 level crossing, and he applied an emergency brake immediately, but the train collided with the light motor vehicle. The driver of the light motor vehicle was dead by the accident. PROBABLE CAUSES It is probable that the accident occurred as the train collided with the light motor vehicle that had entered into Motosu-Minami level crossing, class three level crossing, where the road warning device was in operation according to the approaching train. It is probable that the level crossing was in the situation that the approaching train could be recognized by the operation of road warning device installed in the level crossing. However, it could not be determined the precise reasons why the light motor vehicle had entered into the level crossing where the road warning device sounded alarm according to the approaching train, because the driver of the light motor vehicle was dead. .

Gifu is a Prefecture, a city, AND a region (just to confuse things). All this is in the Tokai region of Japan’s main island, Honshu. GIFU TOKYO NAGOYA KYOTO OSAKA Gifu Prefecture is comprised of five regions: Hida, Tono, Chuno, Gifu, and Seino. The southern half of Gifu was once called Mino, while the northern half has been called Hida since time immemorial, or so they say. The Gifu region is the area surrounding Gifu city in the southern part of Gifu Prefecture. HIDA CHUNO SEINO GIFU TONO The Gifu region is bordered by rivers and mountains. For better or worse, this centrally-located area truly is the ‘belly button’ of Japan, since much of the area is less than 100m (328ft) above sea level. This geography is one reason we southern Gifu-ites have to battle through such extreme heat and humidity every summer. JTSB Mission We contribute to - preventing the occurrence of accidents and - mitigating the damage caused by them, thus improving transport safety while raising public awareness, and thereby protecting the people’s lives by - accomplishing appropriate accident investigations which thoroughly unveil the causes of accidents and damages incidental to them, and - urging the implementation of necessary policies and measures through the issuance of safety recommendations and opinions or provision of safety information. JTSB Principles 1 Conduct of appropriate accident investigations We conduct scientific and objective accident investigations separated from apportioning blame and liability, while deeply exploring into the background of the accidents, including the organizational factors, and produce reports with speed. At the same time, we ensure that the reports are clear and easy to understand and we make efforts to deliver information for better understanding. 2 Timely and appropriate feedback In order to contribute to the prevention of accidents and mitigation of the damage caused by them, we send messages timely and proactively in the forms of recommendations, opinions or factual information notices nationally and internationally. At the same time, we make efforts towards disclosing information in view of ensuring the transparency of accident investigations. 3 Consideration for victims We think of the feelings of victims and their families, or the bereaved appropriately, and provide them with information regarding the accident investigations in a timely and appropriate manner, and respond to their voices sincerely as well. 4 Strengthening the foundation of our organization We take every opportunity to develop the skills of our staff, including their comprehensive understanding of investigation methods, and create an environment where we can exchange opinions freely and work as a team to invigorate our organization as a whole.
