REGION V (Bicol) ‹ • A storm surge occurred in the coastal of Bagatabao in Bagamanoc, Catanduanes on 25 December 2011. Twenty (20) houses were partially damaged affecting 23 families /46 persons and damaging 2 hectares of rice field. • MSWDO in Catanduanes distributed relief goods to each of the affected families • A fishing boat, FB Jasmin, capsized when it encountered engine trouble and battered by strong winds while at the vicinity of Magallanes, enroute to Bulan, Sorsogon. Onboard the FB Jasmin was three fishermen, namely: Carmelito Cha, 71 yrs. old; Ronnie Toca, 44 yrs. old; and Danilo Rosanna, 27 yrs. old, all from Brgy. Dancalan, Donsol, Sorsogon. On 26 December 2011, the three fishermen were rescued by the MV Our Lady of Miraculous Medal while enroute to Pioduran Port

REGION VI (Western ) ‹ • Flooding occurred in the Provinces of Capiz and affecting 1,099 families/3,686 persons. • A landslide also occurred in Banga, destroying five (5) houses and affecting 15 families/74 persons.

REGION VII (Central Visayas) • Due to flooding and landslides, evacuation/pre-emptive evacuation were conducted but most families have already returned to their homes in the following 4 cities and 9 municipalities: o Danao City : Brgys Looc, , Maslog, Guinacot, Taytay, Tuburan, Taboc and Suba o : Brgys Guadalupe and Mabolo ( Magtalisay) o City : Brgys Umapad, Banilad, Paknaan and Maguikay o Talisay City : Brgys Dumlog and Poblacion o Asturias : Brgys Sta. Lucia (Sitio Labangon and Daan Lungsod) and Poblacion o Balamban : Brgys Tunga and Buswang o Carmen : Brgy Luyang o Consolacion: Brgys Pitogo (Sitio Laray), Cansaga, Jugan, Tayud, Nangka, Danglag, Gareng, Panas, Lanipga, Panoypoy, Tina-ong o Lilo-an : Brgys Cotcot and Ubay o Minglanilla : Brgy Tungkil o Sogod : Brgy Bagatayan o Tabogon : Brgy Maslog o Alicia , Bohol: 4 Barangays

2 REGION VIII (Eastern Visayas) • Flooding occurred in the following areas: Brgys Tacuranga, Cogon and Cangumbang Palo, due to heavy rains; Likewise in Brgys Tacuranga, Cogon and Cangumbang; • A minor flooding was also reported at the low-lying areas of St. Bernard, due to the intermittent rains brought by the Low Pressure Area and tail end of cold front affecting 8 barangays: Hindag-an, Balodbod, Mahayahay, Sitio Ma.Socorro, Mahayag, Lipanto, Catmon and Tabontabon; As of 8:00 AM, 29 December 2011, floodwater already subsided and most evacuees returned to their homes except those from Brgy Tabontabon • A landslide occurred in Mahaplag-Sogod Road - clearing operations was conducted by DPWH Southern Leyte and it is halfway passable • State of Calamity was declared in the Abuyog, Leyte due to flooding in 14 barangays, namely: Poblacion 1-9, Pagsangan, Can Apurong, Udyungan, Burubudan and Catmarating ; closely monitored the swollen riverbeds located within the municipality and likewise alerted concerned barangays; except for evacuees from Brgy Tabontabon, all affected families have returned to their homes • Flood waters submerged the boundary of Brgy Bonifacio and District I in the municipality of Julia, Leyte. • Flooding affected eight (8) Brgys in City.

REGION X ‹ • Flashflood also occurred in Valencia City, Bukidnon due to overflowing of Polangi River, affecting 12 Brgys, namely; Batangan, Bagontaas, Catumbalon, Kahaponan, Lumbayao, Lumbo, Maapag, Pinatilan, Poblacion, San Isidro, Sinabuagan and Sugod; two (2) barangays in , Bukidnon (Busco Butong and San Jose) and Brgy Tugop in San Fernando were affected by flooding because of heavy rains and overflowing of flood water in Pulangi river – floodwater has subsided as of 1:30PM, 27 December 2011 • Houses and properties of residents were washed out by flood including agricultural crops. Residents evacuated to their respective Brgy Halls and nearby schools for shelter.

REGION XI ‹ • Flooding/flashflood occurred in and Asuncion, Davao del Norte due to the overflowing of Saug/Libuganon River and Ilog Creek that traversed the said municipalities. Flood water already subsided in Kapalong, Asuncion and . Flooding destroyed agricultural land planted with bananas and rice and the Delta Banana Packing Plant in Kapalong. ‹ Compostela Valley • Flooding were experienced in low-lying areas in the municipalities of Compostela, Montevista, New , Monkayo, Maco and , all of Compostela Valley Province • Eighty six (86) families from Brgys New Visayas, Banagbanag and San Jose of Montevista were evacuated to Brgy Halls of New Visayas and San Jose. • Landslide occurred at Brgy. Camanlangan and Mt. Diwata, Monkayo.

3 REGION XII ‹ North • Landslide occurred at Luhan River 6 km (by aerial distance) downstream from Lake Holon, Sitio Basag, Brgy Salacafe, TIboli, due to continues rains and natural damming of 10 feet in height and 180 feet in width. Water level increased by 5 feet in height in the damning which created a lagoon of 15X25 meters. Water disposal in the damming area is minimal through seepage. Slope leaking is noticeable in the area that will jeopardize the situation. • Flashflood occurred in Brgy , Kabacan of North Cotabato affecting Pedtad, Salapungan and Kayaga because of heavy volume of water coming from Pulangi River in Bukidnon. A total of 855 families/40,000 persons and one (1) mosque were affected; as of 29 December 2011, floodwater has partially subsided but affected/ displaced families still remain at the ECs

CARAGA ‹ • A total of 42 Barangays were flooded in the cities/municipalities of , San Francisco, Rosario, Veruela, Loreto, San Luis and Esperanza affecting 8,474 families or 42,540 persons • Flooding occurred in Brgys Novele, Cabawan, Libuac, and part of Poblacion in Rosario, Agusan del Sur. A total of 42 families/226 persons were rescued and evacuated to Bayugan East Central Elementary School

‹ Dinagat Province • Two (2) barangays were flooded: 1 barangay of Libjo and another in Basilisa affecting 28 families or 140 persons

• A total of seven (7) barangays were flooded in the municipalities/city of Surigao, Claver, Sison, Tubod and Placer affecting 260 families/ 1,144 persons • Twenty two (22) families who evacuated to Brgy. Mabuhay Hall, Sison, Surigao del Norte, opted to return to their respective homes during daytime and back to the evacuation center at night due to fear of another surge of flashflood – 20 families decided to stay put until the weather situation calms down

ARMM • Seven (7) barangays were affected in : 2 in Pagalungan and five (7) in Datu Montawal. Another five (5) barangays are likely to be affected because of water level. Crops along riverside were submerged in water. • As of 29 December 2011, floodwater in the municipality of Montawal is gradually subsiding and no displacement or casualty has been reported

Region X • DPWH reported the following impassable roads: Bukidnon ‹ RCPC Cross Drainage along Jct. SH Manolo fortich-Libona –Indahag Road Km.1465+042, Libona, Bukidnon ‹ Warning and directional sign was installed. Provided detour road.

4 de Oro ‹ Lumbia-Uguiaban bypass Road (Mangalay Section) ‹ Warning and directional signs installed. Vehicles can pass to an access road or detour road.

Lanao del Norte ‹ Mandulog Bridge 1 Highway Secondary Diversion Km.1522+484. ‹ Barricades were installed before both approaches. Clearing or scraping of the road /bridge from mud, wood/logs and plant debris is still ongoing.

CARAGA • DPWH reported the following impassable roads/bridges: Agusan del Sur : ‹ -Talacogon-Loreto-Veruela-Sta.Josefa Road (detour of Mambaos Balley Bridge II K1391+747) ‹ Daang Maharlika (Rosario Section) K1323+925+K1324+230 (impassable to light vehicles) ‹ Brgy Poblacion along the national highway is not passable to all kinds of vehicles. ‹ Los Arcos Section, Magsaysay, Laminga Section, Nuevo Trabajo Section, NRJ Bayugan-Calaitan- Road (Sto. Region XI • Four (4) houses in Barangay Florida were totally destroyed • A rice and banana plantation in 2 Semong, the Delta Banana Packing Plant in Kapalong and 328 has of banana Cavendish were also flooded • Eleven (11) houses in Brgy Gupitan and agricultural areas in Brgy Sampao were damaged

Region VII • Three hundred (300) houses were partially damaged in Brgy Mabolo, Cebu City while 400 houses were totally washed out in Brgy Magukay, Mandaue City

CARAGA • Ten (10) houses were damaged in (2 totally/ eight (8) partially)

6 II. ACTIONS TAKEN

Region IV-A • Coordinated with MPS Naujan, Oriental Mindoro and Public Safety Department of City, Oriental Mindoro regarding the flooding incident and with the Coast Guard District Palawan re maritime incident • Disseminated SWB and Gale Warning to PDRRMCs, LGUs, Local Radio stations and Line Agencies through SMS and facsimiles • CG Districts Palawan rescued passengers of a motorized Banca in Cuyo, Palawan • Coast Guard cancelled all trips of small vessels due to strong winds and big waves

Region V • MSWDO in Catanduanes distributed relief goods to each affected family

Region VII • OCD VII called up PCG District Office, 505 th SRS 2AD and CENTRAL COMMAND for deployment to affected areas • Activated respective local DRRM Councils for further monitoring of the situation in their AOR • Responding Units: Emergency Response Unit Foundation; PNP; BFP; Barangay SAR Volunteers; and Philippine Red Cross • CENTCOM – AFP notified OPCON units to monitor hazard prone areas and coordinated with local DRRMCs for pre-emptive evacuation and staging of equipment /responders • BDRRMC teams issued warning notices to hazard prone areas and activated Brgy SAR teams • PCG-DCEV monitored the development of the situation and coordinated with local PNP not to allow fishing crafts to venture to sea • Evacuees stayed at covered court and were provided with food by local MSWDO • Activated the following responding units: Emergency Response Unit Foundation. PNP, BFP, Brgy SAR volunteers, PRC and Local Social Welfare Office • UNDER STATE OF CALAMITY: Municipality of Consolacion and City of Danao

Region VIII • RDRRMC VIII continuously disseminated weather advisories to local DRRMCs media and member agencies ; continuously monitored the possible effects of LPA and advised the LDRRMCs and concerned agencies to take appropriate actions; and coordinated with the 8 th Infantry Division Philippine Army for dispatch of logistics to Abuyog, Leyte • PNP Units in Region 8 were placed on alert status and standby for deployment. • MDRRMO St. Bernard, Southern Leyte monitored the situation through radio and advised the public to conduct precautionary measures; conducted an ongoing evacuation to the flood and landslide prone barangays; closely monitored the swollen riverbeds located within the municipality; and alerted barangays to take appropriate actions • CDRRMC Ormoc provided ready to eat meals using mobile kitchen in coordination with CSWDO • WASAR trained personnel of 8 ID PA headed by Capt Cagabi were deployed to Abuyog, Leyte to augment MDRRMC Abuyog in the conduct of evacuation operations. Two (2) M35 trucks proceeded to Abuyog with the following

7 equipment: 3 rubber boats from 8ID, 1 rubber boat from OCD, 1 chainsaw, 2 nurses and 4 scout escorts • PAF-TOG 8 with six (6) WASAR Trained personnel and 1 rubber boat, PNP Ormoc, 8RCDG and Ormoc Rescue Team conducted rescue operations in Palo , Leyte • CDRRMO conducted overnight ocular inspection in identified flood/ landslide prone areas • DRTU of 19IB led by 1LT Palawan arrived at Brgy Liloan, Ormoc City and assisted the residents in crossing the flooded river. • Another DRTU under Cpt Tamarong arrived at Brgy Lao, Ormoc City and DRTU led by 1LT Aranez arrived at Brgy Cangumbang, Palo, Leyte to assist in the conduct of SAR operations. • 546 Engineering Battalion led by TSgt Gerona arrived at Abuyog, Leyte with one (1) rubber boat to augment the DRTU personnel of 78IB to reach the three (3) affected brgys of the municipality • PNP Ormoc, 8RCDG and Ormoc Rescue Team conducted an evacuation in the municipality • Leyte Early Warning Systems (EWS) reported that an ongoing evacuation is being conducted in Brgys Tacuranga, Cogon and Cangumbang, San Antonio, Palo, Leyte, rescue teams are conducting rescue evacuation • 802 Bde are conducting rescue evacuation • Tanauan ERT and PN Tacloban Station conducted search and retrieval operation for the missing person in Tanauan Leyte • OCDRC8 facilitated the release of financial assistance to the beneficiaries of flood victims

Region X • Valencia City requested for 1 chopper in to rescue trapped persons – request was granted by Davao Mayor and rescue efforts are ongoing • Bukidnon PDRRMC provided relief assistance in the form of rice, noodles, sardines, etc. • PHO Bukidnon extended medical assistance to the affected families • MDRRMC Quezon, Bukidnon sent their rescue team in the affected area. Families were evacuated at BUSCO Elementary School while Quezon rescue team is on standby at BUSCO Bridge to monitor the water level. • CDRRMC Valencia City Rescue Team conducted rescue operations. DANA team conducted Damage Assessment and Needs Analysis in the area affected with PDRRMC Bukidnon DANA Team • Responding Units: PA, Reservists, BFP, BJMP, PNP, PDRRMC Team-Bukidnon, CDRRMC , CDRRMC Valencia, PRC Bukidnon, Pangantucan Rescue, Karancho Malaybalay and Valencia, ABC Eagles Radio Club, City Traffic Management Center, City Office, CSWDO • Valencia City Mayor Leandro Catarata requested for 1 chopper in Davao City which was granted by Davao City Mayor Sarah Duterte for rescue of the trapped persons • LGU Valencia City was quick to setup evacuation centers; conducted medical services to flashflood victims; distributed 6 boxes of sardines, 100 boxes of noodles, 86pcs mats, 146pcs blankets, 30 mts “trapal” and 10 sacks rice; a community kitchen manned by Valencia LGU employees was also established • PDRRMC Bukidnon extended the following relief assistance to its affected constituents: 97 sacks rice, 30 boxes noodles, 19 boxes sardines, 8 boxes

8 beefloaf, 26pcs cooking pot, 26 pcs mats, 26 pcs basins, 369 pcs plates, 226 pcs blankets and 100 packs of used clothing • Barangay Council of Butong, Quezon in coordination with Rescue 601 Quezon, Bukidnon immediately conducted assessment and forced evacuation of affected households • MDRRMC San Jose, Quezon advised its constituents to transfer to higher ground and were also given relief goods

Region XI • PDRRMO and RDR Smart Davao del Norte, MDRRMCs Asuncion, Kapalong and BFP conducted rescue operations in the affected areas and assisted in the evacuation operations; PDRRMC Compostela Valley and Davao del Norte conducted pre-emptive evacuation • PDRRMCs of Compostela Valley and Davao del Norte, MDRRMCs and BDRRMCs provided food assistance to affected families • PDRRMC deployed heavy equipment for the restoration of the impassable roads caused by the flooding • Twenty five (25) families from New Visayas Montevista were evacuated to New Visayas Elementary school • PN Forces of Eastern with water assets were deployed to augment rescue operations in the Municipality of Sto. Tomas, Davao del Norte • MDRRMC Kapalong conducted an emergency meeting and recommended to the the declaration of the area under state of calamity • OCDX Director Liza Mazo led the ocular survey together with concerned LDRRMOs of affected areas • Responding Units: PDRRMO Davao del Norte, RDR Smart Davao del norte, 60IB PA, MDRRMOs of Asuncion and Kapalong Davao del Norte, BFP Kapalong, PCG – 2 Teams, NFEM, Compostela Valley Emergencies Response Team (COVERT), COERT of MDRRMC Compostela, COVERT of PDRRMC Compostela Valley Province • PCGA members proceeded to Capalong and Sto. Tomas, Davao del Norte and were able to evacuate 41 families and brought them to the Capalong evacuation center • PDRRMCs Comval and Davao del Norte directed all LDRRMC Chairmen to conduct pre-emptive evacuation if necessary; provided food assistance and deployed heavy equipment to clear/restore unpassable roads caused by the flooding • All affected LGUs conducted Damage Assessment

Region XII • MDRRMC T’boli conducted an ocular inspection in the area and organized ten (10) trained rescuers to conduct on site evaluation • Seventy (70) persons from Punol Island, Poblacion 7, evacuated to the Philippine Ports Authority (PPA) compound – Cotabato in the evening of 27 December 2011 due to fear of flood resulting from water releases of NIA dam • PNP PRO-12 reported no flooding incident occurred in Cotabato City as of 10:00 AM 28 December 2011.

ARMM • RDRRMC conducted an ocular inspection in the municipalities of Pagalungan and Montawal, Maguindanao • Conducted an Emergency Meeting on 27 December 2011 to assess the situation and plan next steps; alerted the DILG-ARMM and local authorities in low-lying

9 municipalities particularly those living near the Pulangi river (Municipality of Pagalungan, Datu Montawal, Maguindanao) who previously experienced flooding due to overflow of Pulangi river • OCDARMM BG Rirao made an ocular assessment and called up an emergency/ coordinative meeting with Mayor Datu Otto and Barangay Captains of the municipality of Montawal to assess the ongoing situation and plan actions to be undertaken; proceeded to six badly affected barangays in Montawal for data gathering and assessment of condition of affected families • LGU Montawal distributed 400pcs of sardines, 1,000 packs of noodles and 20 sacks of rice • PDRRMC Maguindanao distributed 1,185 relief packs and 1,00 kilos tilapia to the affected

CARAGA • DSWD–CARAGA has available standby funds of PhP356,630.14 and stockpile of relief commodities composed of food and non-food items worth PhP3,168,320.66 ready for augmentation for any eventuality within their area of jurisdiction. • LGU Surigao del Sur and Agusan del Sur provided P333,519.00 worth of relief assistance to the affected families • MDRRMCs concerned were activated to undertake precautionary measures and be alert for possible pre-emptive evacuation • The Quick Response Team (QART) of Surigao City was deployed to flood-prone areas in the city especially in Brgy. San Juan for actual survey of the situation • DPWH CARAGA monitored continuously the conditions of major roads and bridges; Its Disaster Preparedness Teams and Equipment were on stand-by ready for deployment • RDRRMC Operation Center disseminated NDRRMC Advisories to RDRRMC member agencies, local DRRMC’s and local radio/TV stations in the region through SMS; maintained 24/7 monitoring; and coordinated with local DRRMCs for situational update • PIA also assisted in disseminating warning advisories to its networks

Agusan del Norte • PDRRMC and CDRRMC Butuan City monitored continuously the water level of Agusan River with response teams and rescue equipment ready for mobilization when necessary • PDRRMC Agusan del Norte was convened to preposition resources. Emergency Rapid Assessment Team (ERAT) was deployed to Lake Mainit to conduct an ocular survey of the water level and provide warning and information in the community

Agusan del Sur • The Search and Rescue Agusan del Sur (SARAS) conducted SAR operations in Brgy Poblacion, Rosario. • PDRRMC Agusan del Sur dispatched its Search and Reascue team (SARAS) to assist the evacuation in the affected municipalities, dispatched PSWDO personnel to validate and document evacuation reports and conduct relief operations • CDRRMC of Bayugan provided relief assistance to affected families

Surigao del Norte • PDRRMCs Surigao del Norte and issued Alert Level 2 upon recommendation of PAGASA Surigao

10 • PDRRMO of Surigao del Norte inspected swollen river situation in Brgy. Jubgan, Malimono, Surigao del Norte • The province of Surigao del Norte and Province of Dinagat Islands have lifted Alert Level 2 upon recommendation by PAGASA Surigao station • PDRRMC Surigao del Norte and CDRRMC Surigao were convened to preposition their resources and activate response mechanisms. Local DRRM Operation Centers closely monitored the situation and coordinated with concerned agencies for appropriate action • DepEd Surigao Province was advised to prepare schools as evacuation centers • PSWD released family packs and distributed to families inside evacuation centers • The Active Local Emergency Response Team (ALERT) of Surigao del Norte, Quick Action Response Team (QART) of Surigao City and Dinagat Island Search and Rescue Team (DISART) were deployed to conduct response operations in affected areas. • QART conducted search and rescue for one (1) missing child of Brgy. San Roque, Surigao City • PRO 13 has activated the RDOC and ROPD personnel prior to the weather advisory from PAGASA and monitored continuously the situation in the AOR; PNP Sison MPS deployed 10 personnel with 20 BPATs and 4 vehicles (1 Hliux; 1 Dumptruck; 1 Ambulance; and 1 Multicab) to the barangay covered court in Mabuhay Sison, Surigao del Norte to assist the evacuees

Surigao del Sur ‹ PDRRMC conducted an emergency meeting to establish a 24/7 monitoring and updates of information on the effect of LPA in the province
