Occlusions on implants

Occlusion on implants is a controversial topic. From the science of stress and load, to occlusal design, and quality of bone, everything is potentially responsible for implant failure, screws fractures and peri-implantitis.

This webinar will clarify the role of occlusion in implant restorations. We present a novel approach considering the position of the implant in the arch and the stability of the Dr. Michael Radu, occlusion. DDS, MS, FAGD Learning Objectives: • Discuss occlusion as a macro concept - intercuspation and mandibular position, rather than immediate side shift • Review of the common approach of occlusion on implants Primary Sponsor: • Discuss new protocols for occlusion based on the position of the implant in the arch • Compare analog to digital occlusal recordings • Discuss a simplified occlusal design for implant restorations

Dr. Michael Radu graduated in his native Romania, Dental School of Bucharest, in 1979 - Summa Cum Laude and valedictorian. He completed a three year general dentistry residency, at the same school. He Secondary Sponsor: received his Masters of Science degree from Lynn University in Boca Raton, Florida, in 2000. He practiced for six years in Düsseldorf, Germany and since 1989 in Boca Raton, Florida. He has a diverse practicing and teaching experience covering more than three decades and three countries. His passion for the science of dentistry translated into several articles and one patent that Dr. Radu authored.

Dr. Radu has a passion for dentistry and for teaching it to others. He believes dentistry is more of a science that can be taught, rather than an art one has to have a knack for. One of his most effective teaching techniques is breaking down complex ideas to their simple components so attendees can understand the concept and believe that they can achieve the same results.

Michael Radu, DDS, MS has a restorative and aesthetic oriented private practice in Boca Raton, Florida since 1992 and is an adjunct professor in the restorative department at Nova Southeastern University College of Dental Medicine since 1999.

He lectures nationally and internationally on aesthetic and reconstructive dentistry, with emphasis on treatment planning, implants, occlusion, the temporo-mandibular-joint and digital dentistry. He has lectured for groups ranging from dental societies and study clubs to Fortune 500 companies and dental industry manufacturers.

Comparing the efficacy of the chamfer margin and reverse margin systems at preventing submarginal cement

Intra-oral cementation is a common process for attaching crowns and bridges to natural teeth and dental implants. While fixed-prosthetics are cemented onto natural teeth, dental implants may also be restored using a screw-in system. It is unfortunate that this latter installation system has inherent problems that render patients susceptible to misfit joints and treatment related cantilevers that increase both mechanical loads and block access to care. Treatment for related peri-implant disease can be uncomfortable, unpredictable, Dr. Emil LA and expensive. Svoboda PhD, DDS Intra-oral cementation can reduce or eliminate several risk factors for complications related to the screw-in technique but has been plagued by complications related to poor prosthesis margins and residual subgingival cement. While removal of subgingival cement can often result in the resolution of related peri-implant disease, an intra-oral cementation Primary Sponsor: system that prevents this problem would be a game changer. Learning Objectives: 1. How to identify the longstanding root problems of installation related complications Secondary Sponsor: 2. How the Chamfer and related tissue facing margins fail to prevent both poor margins connections and submarginal cement 3. How the Reverse Margin System design and installation protocol helps the dentist to optimize the fit of implant connections easily and consistently, while preventing overhanging and open margins and submarginal cement

Dr. Svoboda graduated from the University of Toronto with a PhD, DDS. He is a Fellow of the AGD, an Honored Fellow of the AAID and a Diplomate of the ABOI/ID. He has received the Award of Merit from the ODA for his contributions to organized dentistry. He practices Implant Dentistry at ParkPlace Dental Centre in Brampton, Ontario. Dr. Svoboda has been granted Patent protection for his innovative Reverse Margin Abutment design. He is thankful to BioHorizons Canada, The Aurum Group® of Dental Laboratories and Core3D Milling Centres for helping to make Reverse Margin Products available to dentists across North America. Dr. Svoboda lectures Nationally and Internationally and loves to work with people that want to make implant treatment better.

Clinical and radiographic manifestations of vertical root fracture in endodontics

Vertical root fracture (VRF) is the result of complex restorative procedures such as as well as intra-radicular post placement. VRF exhibits a broad spectrum of signs and symptoms from no symptoms to obvious diagnostic signs of root fracture. The diagnosis of VRF is difficult at times because of the late appearance of the signs and/ or radiographic features; however, misdiagnosis is also common. Contrary to crack, a tooth with VRF is deemed to be non-restorable and should therefore be extracted. Early Dr. Farzad Danesh diagnosis can help preserve the bone needed for future prosthodontic rehabilitation. DDS, FRCD(C), Endodontist At the end of this webinar, the audience should be able to: • Diagnose VRF by gathering clinical and radiographic information • Differentiate VRF from entities with similar clinical and radiographic manifestations Primary Sponsor: • Implement CBCT in the diagnosis of VRF • Prevent VRF via appropriate procedural guidelines

Oral disease and systemic connections: the role of the dentist and oral pathologist

Very frequent and common oral diseases such as aphthosis, oral , migratory have a strong association with systemic pathologies and sometimes fall within syndromic pictures. The dentist and dental hygienist may be the first to diagnose even systemic conditions. Syndromic pictures such as Gardner’s syndrome, Crouzon’s, Kawasaki’s, Plummer Vinson’s syndrome also have oral manifestations which are important to know. The purpose of the report is to provide an overview of Dr. Cinzia Casu, these connections and the contribution that the dentist can make in the recognition of DDS conditions and syndromes starting from the observation of the oral cavity.

Cinzia Casu graduated in 2010 and works at the moment at her Private Dental Practice in Cagliari (Italy). She had concluded a biennal Master on Oral Surgery and Pathology in 2015, and then others courses of Primary Sponsor: oral pathology. She is the author of several national and international articles and a monograph. The palatal approach in maxillary thin ridge: is immediate loading possible?

A very thin ridge at the upper jaw is not a rare issue in patients wearing dentures for decades, since their young age : a successful implant rehabilitation in these Patients , and especially the chance of an immediate loading fixed bridge placement, depends mainly on the density of the basal bone and not on the implants being completely surrounded by the hard tissue. Dr. Enrico If a good primary stability is attained, in a Type D1 or D2 bony tissue, allowing a good Rivarossa, 360° anchorage for the tip of the implant ( 50% lenght minimum) and for at least 4 fixtures DMD., DDS strategically placed, there is a reasonable likelihood of success for an “immediate laod” ,with an optimal osseointegration and a very low risk for the Patient developing peri- implantitis. Primary Sponsor:

This happens, in my opinion and according to my 10 years experience with this approach, in spite of the fact that 7 to 10 mm of threads may be partially in contact with, Secondary Sponsor: and covered only by the connective tissue of the palatal mucosa, wich is very thick : of course the osteotomy for the placement of each and every implant, must be accurately prepared on the inner side of the ridge in a particular manner ( Palatal Technique ) adding to the “hole” for the apical threads in the basal bone, a very precise “half pipe ” in the palatal slope , a task that can only be accomplished in a correct, and very easy way, by the use of a Piezo Surgical Instrument, wisely combined with the drills .

The fixture threads must have a very close contact with the osseous surface of this half pipe, in order to establish the conditions for Osseointagration, that will take place normally in the interface between the titanium and the bone: the thin ridge , external to the fixture, must be intact so that it works like a shield for the implant , or even better it provides a bed for the vestibular mucosae, preventing the keratinized and non- ketatinized tissues from getting in contact with the implant surface .

The vestibular gengiva , no matter its thickness, would not stick to, nor survive in contact with the titanium , and this circumstance if it happened would be shortly causing a” gingival retraction ” or a pocket , thus starting a process of perio-implant inflammation.

On the other ( or side literally, the palatal side of the ridge ) the thick mucosa , thanks to its huge amount of connective tissue, will still be abundantly nourished and easily bear the presence of the implant threads , probably adhering to its surface, without inflammation nor retraction .

Dr. Enrico Rivarossa, 67, D.M.D. in 1977 , D.D.S. in 1982, at University of Torino, ITALY. Private Practice since 1978, in Fossano and Cuneo, Piemonte, Italy Post-Graduate Continuing Education at Boston University H.Goldman School of Dentistry, Periodontics Department, in years between 1978 and 1988 Periodontal Education at PE:RI:CO , Swampscott Mass. in late 80’s and 90’s. Implant dentistry failures: how they affect the patient, surgeon, restorative and laboratory team

Implant failures unfortunately do occur and many of them are “late” failures i.e. failures after the insertion of the definitive prosthesis. While certain types of prosthetic designs allow for less complicated remediation, failures of all types present the patient and all members of the team with many stressful issues. This lecture will discuss different types of failures and the associated risk factors for them, as well as present many of Dr. Arlin’s Dr. Murray Arlin, cases and their remediation. This lecture will also address dealing with other non clinical D.D.S., M.B.A., issues including warranties and who bears financial responsibility for case remediation. D.D.S., Cert. Prostho., F.R.C.D.(C) Learning Objectives: • Understand the difference between “early” and “late” implant failures • Review many “risk factors” for “early” and “late” implant failures Primary Sponsor: • Review both “implant” and “prosthetic” failures • Illustrate techniques to minimize the risk for failure • Present many cases showing failures and their remediation Secondary Sponsor: • Discuss the effects of failures on the patient, surgeon and restorative team • Present the importance of contracts and warranties for the entire team • Discuss the “responsibility” factor for remediation costs

The effect of occlusal forces on peri-implant bone loss: myth or evidence?

Controversy persists regarding the factors causing pre-implant bone-loss. Over the past few decades, there have been so many attempts to show the direct link between mechanical loads and histological pre-implant osseous changes, in order to develop a simplified guide to the clinician for implant prosthetic design and occlusal therapy.
